8 WELCOME Welcome to OISE/UT s ADDITIONAL QUALIFICATION COURSES Welcome to Professional Learning at OISE/UT. We thank you for your interest in our Additional Qualification courses. OISE/UT has a long and proud history of offering quality professional learning for educators. Again in 2005/6 we are offering expanded offerings of courses in both inclass or online formats. Our online courses are increasingly popular and provide timely learning designed to work within educators busy schedules. We continue to offer the full range of classroom programs including the largest summer program for the arts in North America! Locations include our OISE/UT campus and many other parts of the Greater Toronto area and beyond. Our courses can qualify candidates to teach in another division or become a principal, as well as deepen or enhance present skills. Additional qualification courses help educators meet the needs of a variety of students, adapt to the changing curriculum, incorporate current research into their daily teaching practices and to integrate technology effectively. Because educators for various school districts participate in both our inclass and online courses, dialogue is enriched by the sharing of a variety of practices, ideas and resources. Distinguished faculty from OISE/UT and curriculum leaders from district boards instruct and link theory to practice. Additional qualification courses consist of 125 hours of work as defined by regulation and accredited by the Ontario College of Teachers. Applicants should note that Additional Qualification courses are not considered for degree credit at the University of Toronto. 10 APPLYING FOR ADDITIONAL QUALIFICATION COURSES We have developed procedures to enable you to complete your application and registration quickly and simply by mail. You can pay by MasterCard, VISA, certified cheque, money order or in person by debit card. Please read pages 4-6 in this Calendar for information on completing the application form, and on required documents. PLEASE DO NOT FAX APPLICATIONS 1. Applicants should carefully complete the application form and submit it with the required tuition fee and supporting documentation to the Registrar s Office by the deadline specified for each session. Certified cheque, money order, completed credit card information or in person by debit card are acceptable forms of payment. 2. The Registrar s Office receives and processes applications on a first come, first served basis. Applying by the deadline does not guarantee a place in the course of your choice. Applications received without the required tuition fee payment or complete credit card information will be returned to applicants. Missing information may delay the processing of an application. The Registrar s Office will notify applicants by mail of their status: Accepted and Registered ; Accepted on Condition and Registered or Refused. Due to the high volume of applications we receive, please allow at least 3-4 weeks for notification. OISE/UT will begin processing Fall/Winter applications on July 18, Please note that for teachers who have completed their education outside Canada, academic/transcript assessment may take longer, because external services are often consulted for clarification of foreign transcripts and this process takes time. 3. Note that if you are applying for a Part 2, Part 3 or an Honour Specialist course, you must have an appropriate Supervisory Officer certify that you have the required length of successful teaching experience. Please use the Certification of Teaching Experience form included in this Calendar and available on our Web site at: Where applicable, you must also ensure that your application is supported by official transcripts of all your university work. 1. UNIVERSITY OF TORONTO STUDENT NUMBER If you have previously attended or applied to the University of Toronto, list your assigned student number. This allows speedy identification of your previous records. 2. ONTARIO COLLEGE OF TEACHERS REGISTRATION NUMBER This information is required by the Ontario College of Teachers for the purpose of updating your Certificate of Qualification. 13. SOCIAL INSURANCE NUMBER This information is required by the Ontario College of Teachers for the purpose of updating your Certificate of Qualification. 14. OFFICIALLY RECOGNIZED SURNAME Give the surname by which you are legally and currently known. If there has been a change of name, provide supporting documentation. See page 8 for information on Change of Name. 15. FORMER SURNAME (IF APPLICABLE) Name changes subsequent to a previous application must be supported by official documentation (e.g., marriage certificate). This is a University of Toronto requirement. See page 8 for information on Change of Name. 16. GIVEN NAMES Please give the FIRST name by which you are legally and currently known. 17. TITLE Indicate your title: Miss, Mr, Mrs, Ms, Dr. 18. MARITAL STATUS Indicate by a ( ) mark. 19. DATE OF BIRTH Use numbers only (e.g., 1970/03/15). 10. GENDER Indicate by a ( ) mark. 11. FIRST LANGUAGE Identify the first language you speak. 12. Please notify aq@oise.utoronto.ca immediately of changes to your address. Some settings automatically route larger-volume mailings to bulk or junk folders. Please print your address clearly and legibly on the application form. 17. POST SECONDARY INSTITUTIONS ATTENDED OR CURRENTLY BEING ATTENDED List all post secondary institutions attended or being attended, including dates and degree sought or earned. This information assists us in the assessment of your application. 18. COURSE SELECTION Indicate the course(s) you wish to take by listing: The full course code. It is important that there be no errors in recording this code (list all 13 characters). Each location has a different code. Ensure you have selected the correct one. For example CODE: EAQ9020Y-Y the course title and Part 1, 2 or 3 (if applicable) Listing a second course choice is strongly recommended. 24. CREDIT CARD INFORMATION Complete all credit card information. Expiry date of the card is required. Credit card numbers cannot be submitted by telephone. Please note debit cards can only be used in person. Please note that OISE/UT is not responsible for any interest charges incurred on credit card payments of Additional Qualification course fees. DOCUMENTATION REQUIRED When returning a completed application form, be sure to include all required documentation. While we maintain candidates Additional Qualification files for two years, some documents on file may not be appropriate to a current application. Documents submitted to OISE/UT cannot be returned to candidates. Most courses require some or all of the following: a legible photocopy of your current Ontario College of Teachers Certificate of Qualification or Interim Certificate of Qualification showing the required prerequisites. No other teacher qualification document is acceptable. A replacement Certificate of Qualification can be obtained from the Ontario College of Teachers upon payment of $ Telephone: x 330 (Client Services). 19. PREVIOUS APPLICATIONS If you have previously attended or applied for an AQ course at OISE/UT, indicate the session and year. This information helps with the identification of your previous records. 20. PREVIOUS FRENCH AQ COURSES AT OISE/UT If you are applying for an AQ course in French, this information helps us determine if you are exempt from writing the pre-admission test in French. 21. ATTACHED DOCUMENTS Indicate all documentation you have attached to your application. Be sure to include your certified cheque, money order or complete the credit card information (see number 24 following) to cover full tuition. Where applicable, you must also enclose your official transcripts in sealed envelopes (see page 6) or have them sent directly from the issuing institution to the Registrar s Office. 22. APPLICANT SIGNATURE Your signature on the application form is a University of Toronto requirement. 23. DATE State the date on which you completed the application form. If the prerequisite qualification is not entered on your Certificate of Qualification, please provide a photocopy of your letter of acknowledgement or from the Ontario College of Teachers that indicates the prerequisite course has been successfully completed and that the qualification will appear on your next Certification of Qualification. Applicants should also note that it is their responsibility to ensure that they are enrolled in the appropriate course(s) required to satisfy conditions, if any, set by the Ontario College of Teachers and stated on their Certificate of Qualification or Interim Certificate of Qualification. 13 APPLICATION INFORMATION for all courses which require a university degree and/or specific prerequisites, official transcripts of your academic record from each university attended must be included with the application. Each transcript must be received in a sealed envelope, signed/stamped by the issuing institution. Some institutions will not provide you with transcripts in sealed envelopes. In this case, official transcripts should be sent directly from the issuing institution to the OISE/UT Registrar s Office, Additional Qualifications Unit, Room Transcripts received in unsealed envelopes are not considered official, nor are faxed copies of transcripts. Please note that the Additional Qualifications Unit does not have access to transcripts from other OISE/UT files or other faculties at the University of Toronto. Official English language translations must accompany all documentation not written in English. An official translation is one received in the OISE/UT Registrar s Office in a sealed, signed/stamped envelope directly from an accredited Translation Service. Official translations in English facilitate the assessment of admission documents and avoid unnecessary and unacceptable delays in the assessment process. At the same time, candidates also have the opportunity to verify the accuracy of translations (with their own copy) to eliminate any discrepancy or disputes about our assessment. proof of teaching experience certified by the appropriate Supervisory Officer, for courses which require such experience as a prerequisite (see Certification of Teaching Experience form at back of calendar or available from applications for Technological Studies Courses must include a record of skilled wage-earning experience as detailed on page 55. This experience should not include teaching experience but, rather, any other wage-earning experience within the specific field of study. a completed Technological Studies Applicant Information Sheet for AQ Courses in Technological Studies. TB Test: Any candidate attending a program in which children are participating, and who has not, during the preceding year, either taught for a school board or attended a program leading to basic certification as a teacher in Ontario, must provide evidence of freedom from active tuberculosis. tuition fee payment must accompany your application in the form of: a certified cheque or money order, payable to the University of Toronto OR VISA or MasterCard, with all relevant information on the application form completed OR debit card, only in person. The Office of the Student Accounts does not authorize the Registrar s Office to accept cash or uncertified cheques. 14 APPLICATION INFORMATION Applicants who attend or complete a course for which they do not have the prerequisites and for which they have not received an official letter of ACCEPTANCE from the Registrar s Office do so at their own risk. A CONDITIONAL LETTER OF ACCEPTANCE does not satisfy this requirement. The Registrar s Office reserves the right to withdraw any candidate who submits documentation and who is subsequently found to be unqualified. Admission and course requirements will not be altered or waived. ADMISSION ELIGIBILITY The eligibility of each applicant will be determined by OISE/UT, subject to the regulations of the University of Toronto, and in accordance with the requirements of the Ontario College of Teachers (Regulation 184/97, The Education Act). Only teachers holding a current Ontario College of Teachers Certificate of Qualification or Interim Certificate of Qualification may apply for admission. EQUIVALENT STANDING OISE/UT does not grant equivalent standing. Applicants seeking equivalent standing for any additional teacher s qualification must apply to the Ontario College of Teachers. Written evidence of equivalent standing granted must be submitted when applying for OISE/UT courses. UPDATING EXISTING TEACHER QUALIFICATIONS To update existing teacher qualifications, candidates follow normal application and registration procedures. Upon successful completion of all course requirements they will be recommended to the Ontario College of Teachers for the qualification. The College will add the update to the Certificate of Qualification showing the current year of the update. HONOUR SPECIALIST PRE-ASSESSMENT The Additional Qualifications Unit of the Registrar s Office will conduct academic pre-assessments between November 1st and the end of January upon receipt of your written request, official university transcripts, a copy of your current Certificate of Qualification and the required non-refundable administrative fee, currently $7.00. Thus, any academic course deficiencies could be corrected in preparation for registering in the respective Honour Specialist course. There is a separate charge for each pre-assessment. Assessment of eligibility for Honour Specialist Courses usually takes 6-8 weeks. VERIFICATION LETTERS FOR HONOUR SPECIALIST COURSES Requests for verification letters listing courses used towards the academic admission requirements of an Honour Specialist will be processed only: after the successful completion of the course(s); upon receipt of a written authorization from the teacher to release this information. The authorization must be accompanied by a copy of the letter from the appropriate organization confirming that this information is required from OISE/UT; upon payment of a non-refundable administrative fee, currently $7.00. While every attempt is made to send these letters promptly, please allow at least 6 to 8 weeks for processing. COURSE TIMES, DATES AND LOCATIONS Always check our Web site - for specific times, dates and locations as this is our most efficient means for you to access any changes to our courses. These changes will be posted on the Web site as they become available. CANCELLATION OF COURSES OISE/UT may combine and/or cancel a course due to insufficient enrolment. Decisions regarding cancellations will be made following the application deadline. 15 CANDIDATE RESPONSIBILITY Candidates are responsible for knowing the rules, regulations and deadlines established by OISE/UT and for complying with them. AQ courses are governed by Regulation 184/97, made under the Ontario College of Teachers Act, and as such conditions cannot be waived. Candidates who attend or complete a course for which they do not have the prerequisite and for which they have not received an official letter of acceptance from the Registrar do so at their own risk. CHANGE OF NAME The University of Toronto policy on Name Changes states that changes to an officially recognized name in the student records of the University can only occur at the written request of the name holder, who must present to an authorized University official one of the following supporting documents together with the request for the change: a birth certificate a baptismal certificate a marriage certificate a change-of-name certificate issued by the Registrar General of Ontario CHANGE OF ADDRESS/PHONE NUMBER/ It is the responsibility of the candidate to notify the Registrar s Office in writing (by mail, , or fax) of any changes to their mailing address, phone number and/or address. WITHDRAWAL/NON-ATTENDANCE Candidates who are unable to attend a course for which they have applied or in which they have been officially registered, must notify the OISE/UT Registrar s Office promptly in writing. Candidates must either submit a completed Change/Withdrawal Form or send a signed and dated letter or that states inability to attend or intention to withdraw from the course. Withdrawals are effective on the date upon which written notice is received by the Registrar s Office. Never having attended, ceasing to attend, or advising an instructor that you will no longer attend classes does not constitute official withdrawal and will result in academic and financial penalty. Candidates who withdraw from a course are refunded the portion of their tuition fee as dictated by the Office of Student Accounts, and as stated in the fees refund schedule on page 9. 17 ONTARIO COLLEGE OF TEACHERS SPECIAL STUDENTS Teachers seeking Ontario teaching certification who have been directed by the Ontario College of Teachers to take professional courses at a faculty of education may apply for admission as Special Students to enrol in Additional Basic Qualification courses or basic level Technological Studies courses only. ORIENTATION SESSIONS Applicants must first attend an Orientation Session at OISE/UT. Orientation Sessions for the 2005 Summer Session will be held during April and May For the Fall/Winter Session , these will be held in August Specific dates and times of the Special Student Orientation Sessions are available from the Additional Qualification Unit, OISE/UT Registrar s Office, Room or by calling or Information relating to course offerings, application procedures and an explanation of the letter issued to the applicant by the Ontario College of Teachers, will be provided during these sessions. Applications for a required course will be evaluated at an Orientation Session, thus applicants will be required to present the following: transcripts of all university work, with translation if applicable. Only official transcripts, bearing the institution s seal will be accepted. Unofficial transcripts or grade reports will not be accepted if applicable, a photocopy of marriage certificate or other proof of any change of name photocopy of proof of citizenship/permanent residency/landed immigrant status in Canada photocopy of the letter from the Ontario College of Teachers designating the individual as a special student. If the letter is in French, a translation in English will be required proof of freedom from active tuberculosis satisfactory evidence of an acceptable level of proficiency in both written and oral English. For further details, please refer to OISE/UT s English Language Proficiency Policy. Official English Language translations must accompany all documentation not written in English. Translations will only be accepted from: the consulate, high commission or embassy to Canada of the country which issued the documents a Canadian embassy, consulate or high commission in the country from which the teacher emigrated a translator accredited by a professional association of translators in Canada. To obtain the name of an accredited translator, contact the Association of Translators and Interpreters of Ontario (ATIO) at or or info@atio.on.ca At the time of application, applicants will be required to pay the tuition fee of $ per course, by certified cheque or money order made payable to the University of Toronto. ENGLISH LANGUAGE PROFICIENCY POLICY English is the language of instruction for all courses at OISE/UT, with the exception of some Modern Language courses. Consequently, to be successful in these courses and in the Practicum if required, candidates must demonstrate a high degree of proficiency in both oral and written English. As well, successful candidates receive certification to teach in schools where English is the language of instruction, and where the primary obligation of the elementary and secondary school teacher is to meet the needs of the learner. To do this, the teacher must be able to communicate effectively in English, the students language of instruction. OISE/UT is committed to producing graduates who possess strong English language skills to meet these needs, and who will also be competent and effective role models in the use of oral and written English. Thus, all applicants to the Bachelor of Education and Diploma in Technical Education/Ontario Teachers Certificate of Qualification program, as well as Ontario College of Teachers Special Student applicants to the Continuing Education Program are required to give evidence of their oral and written proficiency in English at the time of application. Acceptable Levels of Proficiency in English for Ontario College of Teachers Special Students Applying to the Continuing Education Program All teachers seeking Ontario certification who have been directed by the Ontario College of Teachers to take professional courses at a faculty of education, and who apply for admission to the Continuing Education Program to complete such requirements, must satisfy one of the following criteria: a) their mother tongue or first language is English OR b) they have studied full-time for at least three years (or equivalent) in a university where the language of instruction and examination was English and which was located in a country where the first language is English OR c) they have achieved the required level of proficiency on one of the tests in English as outlined in Section B (i-v) below. B. Acceptable Tests and Required Scores Note: All tests must have been taken within the three years prior to the date of application to OISE/UT. i) An official statement of the results of TOEFL (Test of English as a Foreign Language) for either the new computer-based test or the previous paper-based test. 19 ONLINE LEARNING AND COURSE FORMATS INTRODUCTION TO ONLINE LEARNING For many, online learning is a new experience. In our online AQs you will find familiar aspects of in-class courses, such as group work and research, set into an easy-to-access online context. Knowledgeable instructors, passionate about developing online learning communities, teach our courses. These courses are continuously revised and upgraded, as is the technology upon which these courses rely, and we continue to expand our online course offerings each year. Please visit our online learning pages for our most current information, ATTENDANCE POLICY FOR ONLINE LEARNING While online learning provides flexibility for candidates, the time commitments outlined by the Ontario College of Teachers remain in effect. For Summer candidates, the 100 online hours are compressed into approximately four weeks. This translates into daily computer time. For Fall/Winter Session courses, candidates spend approximately 7 hours per week on their course. Since all course work must be completed and online interaction is integral, you are not able to compress your learning into substantially fewer days or weeks. 20 PRE-ADMISSION TESTING PRE-ADMISSION TESTING FOR ALL FRENCH COURSES, INTERNATIONAL LANGUAGES (GERMAN, ITALIAN AND SPANISH) HONOUR SPECIALIST AND MATHEMATICS INTERMEDIATE BASIC Please note that candidates MUST have registered for the appropriate course through the Additional Qualification Unit PRIOR to writing the pre-admission test. Test results will be forwarded to the Registrar s Office by the Principal of the course. FRENCH MANDATORY ASSESSMENT FOR FRENCH COURSES Applicants are required to successfully pass a pre-admission test for all French courses. The pre-admission test has been designed to evaluate your ability to understand, speak, read and write in French. It is structured in three parts: 1. Comprehension: Listening to a tape, reading text and answering questions. 2. Written: Cloze test (filling in the blanks with the right word, composing short memos, faxes, notes, reports, letter to a parent). 3. Oral: Interview with a teacher and 2 students (15 minutes duration). Dictionaries are not permitted. The test is two hours in length. To pass the test you need to achieve at least 75% on each part.
