JOB IDENTIFICATION Job Title: Responsible to: Department(s): Directorate: Dental Hygienist Senior Dental Officer Dental Clinical Services Date of last update: October JOB PURPOSE Perform a wide range of clinical and preventive dental procedures on referral from a dentist or other specialist practitioner in any of the Board s Dental sites, or domiciliary and other settings, including the mobile dental unit and satellite clinics, on a wide range of patient groups including routine and special care. Provide oral health advice and instruction to a wide range of patients. 3. DIMENSIONS NHS Shetland s salaried dental service employs over 60 wte staff is operates from 2 bases, the Montfield Dental Health Centre in Lerwick and the Brae Dental Health Centre in Brae; there are 9 chairs in Lerwick: 4 in the Montfield Dental Health Centre, 2 in the Gilbert Bain Hospital Dental Suite, and 3 in the St Olaf Street Dental Surgery. A single chair practice on the Island of Whalsay is also managed from Montfield. Brae has 3 surgeries and manages a single chair practice on the Island of Yell, which also provides dental services for the residents of the Isles of Unst and Fetlar. Occasional visits are carried out to other outer islands. The post holder may be required to work in any of the Board s Dental Centres.

ROLE OF DEPARTMENT The Dental Department assists NHS Shetland in the maintenance and improvement of the oral health of local communities within the area through: Delivery of routine and emergency dental services to local communities Delivery of the epidemiological function through the National Dental Inspection Programme Delivery of National Childsmile Programme Delivery of Comprehensive Dental Service for Older People Delivery of various health improvement initiatives Delivery of specialist services (e.g. general anaesthetics & sedation, orthodontics) Delivery of care in a variety of settings (e.g. domiciliary care & care from mobile surgeries)

KEY RESULT AREAS Provide dental hygiene care to a wide range of patients Plan the delivery of care for patients to improve and maintain their periodontal health Obtain a detailed dental history from patients and evaluate their medical history Complete periodontal examination and charting and use indices to screen and monitor periodontal disease Provide preventive oral care to patients and liaise with dentists over the treatment of caries, periodontal disease and tooth wear Undertake supragingival and subgingival scaling and root debridement using manual and powered instruments Use appropriate anti-microbial therapy to manage plaque related diseases Adjust restored surfaces in relation to periodontal treatment Apply topical treatments and fissure sealants Give patients advice on how to stop smoking Take, process and interpret various radiographic film views used in general dental practice Identify anatomical features, recognise abnormalities and interpret common pathology, and carry out oral cancer screening If necessary, refer patients to other healthcare professionals Referring of radiographs to other operators with appropriate skill Remove sutures after the wound has been checked by a dentist Within the scope of practice, following relevant training, the job holder will: - Give infiltration and inferior dental block analgesia - Place temporary dressings and re-cement crowns with temporary cement - Take impressions - Placement of rubber dam 7a. EQUIPMENT AND MACHINERY Responsible for the safe use of complex equipment, e.g. dental units, computer and digital technological hardware. Daily use of high speed rotary instruments and sharp instruments which the potential for inflicting severe bodily injury to the operator/patient/staff. The use of emergency equipment as and when required e.g. oxygen, defibrillator. 7b. SYSTEMS Awareness of Health and Safety at Work legislation and how it relates to self and staff. Completion of Datix reporting maintaining Clinical Governance. Putting into effect Occupational Health Systems for nurses e.g. sharps injuries Use of Kodak R4 System Understand and aware of the issues surrounding confidentiality and Caldicott.

5 8. ASSIGNMENT AND REVIEW OF WORK Work will be assigned by Senior Dental Officers, Dental Officers and Vocational Trainees. Work will be reviewed by the Senior Dental Officer. 9. DECISIONS AND JUDGEMENTS To provide appropriate clinical preventive interventions commensurate with qualifications, GDC regulations and personal experience, to patients attending the Boards Salaried Dental Service at various sites, on referral from dental practitioners within the Service. This will involve working with the elderly, those with special care needs medically compromised, learning difficulties, mental health issues, physical disabilities and phobic patients. The provision of clinical and preventive interventions to paediatric patients on referral from a dentist. This may include patients who suffer from dental anxiety. The provision of dental interventions and preventive care and advice to the housebound on a domiciliary basis under the direction of a dentist. The provision of dental interventions and preventive care and advice to those attending day centres, schools for children with special needs, residential care centres etc. under the direction of a dentist. Supervision of the clinical site and staff when acting as the sole practitioner ensuring efficient and effective management of resources. Participate in the development of, and ensure compliance with, all clinical protocols, e.g. infection control, health and safety 10. MOST CHALLENGING / DIFFICULT PARTS OF THE JOB Working in cramped or confined space in some clinics, mobile dental units and in domiciliary settings. Daily exposure to noxious odours, saliva, blood, pus due to close proximity. Treating patients with a high level of anxiety; severely challenging behaviour; or physical, medical or mental disability. Dealing with patient complaints, and facing hostility.

Direct communication: (Clinical) On a one to one basis, with patients, or with carers; Communication in the clinical setting with due regard to confidentiality between staff and patients; Liaising with dentists with regard to combined treatment needs of patients. Direct communication: (Non-Clinical) undertake teaching to groups, with the use of computer-aided presentations, visual aids, and practical demonstration. Various groups of patients are treated including patients with: physical disability; dental anxiety or phobia; mental illness; behavioural problems; learning difficulties, patients with blood-borne virus infections. Examples of care requiring communication skills. The provision of tooth-brushing instruction, dietary advice and other preventive advice to a wide range of patients and those who care for others; parents, support services etc. Ensuring all information is accurate, evidence based and is conveyed in the most appropriate form to ensure understanding and is mindful of social, religious and cultural needs.

7 Ensure that complex and sensitive information relating to treatment options; treatment provided and the need for compliance with treatment and preventive regimes is given to patients and those who care for others; parents, support services etc. Ensuring all information is accurate, evidence based and is conveyed in the most appropriate form to ensure understanding and is mindful of social, religious and cultural needs. Recognise and overcome barriers to the acceptance of given advice with tact and persuasion, using appropriate visual aids and support from other agencies where appropriate. Provide smoking cessation advice to appropriate service users involving multi-disciplinary teams when needed. Communicate with the dental team and other health care professionals e.g. social services, health visitors and school nurses, paediatricians and general medical practitioners to ensure efficient and effective working and appropriate patient care pathways. 12. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB Physical: Working in cramped or confined space in some clinics, mobile dental units and in domiciliary settings; As some patients cannot be treated in the normal supine position due to ill health or disability, the post holder may compromise the ideal working position; Highly developed physical and precision skills are required to use instruments safely; Long periods of intricate work requiring a high level of dexterity, movement and stability with leverage a major component approximately 3.5 hours per time; Manual handling of patients with disability; Weight of equipment to be carried for domiciliary visits; Driving. Mental The job requires concentration, empathy and an ability to deal with patients with complex disabilities. Emotional: Daily exposure to noxious odours, saliva, blood, pus due to close proximity, airborne infection etc. Daily exposure to hazardous materials Frequently treating patients with a high level of anxiety; severely challenging behaviour; or physical, medical or mental disability. This can be very emotionally distressing. Dealing with patient complaints, and facing hostility.

8 The post holder undertakes to provide domiciliary care (home visits) to elderly and housebound patients. Delivery of treatment to patients (especially children) living in relative deprivation and with high disease levels. 13. KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED TO DO THE JOB The post holder will be in possession of a Diploma in Dental Hygiene or equivalent qualification, be registered with the General Dental Council and have a clean full driving licence to enable travel across Shetland. Significant evidence of Continuing Professional Development, including knowledge of any advances in extended duties (as laid down by the General Dental Council), clinical techniques and treatments. Other essential knowledge areas include: COSHH guidelines; Knowledge of Internet and ; Statutory training, including; moving and handling, cross infection, basic life support. 14. 9 PERSON SPECIFICATION General Education/ Professional Qualifications Experience Skills and Abilities Knowledge Personal Attributes ESSENTIAL Degree / Diploma in Dental Hygiene/Therapy. Current GDC registration. Competency in permitted extended duties Experience in working/dealing with special needs groups including the elderly, young patients, medically compromised patients and those patients with differing abilities and behaviour. Assessment of own workload in order to accommodate service need. Supervision and direction of dental nurse providing chairside and clerical support. Participate in and evaluate audit and research. Highly specialist fine manual dexterity. Concentrate for long periods of time. Work under pressure and on own initiative. Be competent in dealing with emotional and distressing situations and possible verbal aggressive behaviour. Literate and numerate. Maintain and update theoretical and practical knowledge in order to obtain professional qualification. Knowledge of the specific needs of patients with HIV disease, organ transplants and those with complex dermatological conditions (e.g., Epidermolysis Bullosa) to enable safe practice.
