2 When there is a work-related accident or illness, procedures must be taken to ensure the employees needs are met with respect to treatment and claims processing. Certain workers compensation documents must be provided to the employee, and an injury report must be filed accordingly. The following outlines the employee s responsibilities with respect to this process. S i S h l N d Ri k M Supervisors, School Nurses, and Risk Management representatives ( ) can assist in providing these documents to the employee.
3 The employee must immediately report all work-related accidents to their supervisor whether or not an injury is apparent. Injuries requiring emergency medical attention: a) Employee should go to the closest emergency room or urgent care facility. b) Employee must submit to drug testing as allowed by the nature of the injury. c) Employee must give or send to their immediate supervisor any paperwork received from the Emergency Room. d) Employee must select a physician from the approved panel of physicians as soon as the emergency care has been completed.
4 Injuries not requiring emergency medical attention: a) Employee must select a physician from the approved panel of physicians b) Employee may be financially responsible for any medical treatment sought or received from a physician not on the panel. c) Employee must submit to drug testing as directed by their supervisor and/or Personnel.
5 All injuries a) Employee shall inform the physician/medical facility the injury is work related and the employer is Portsmouth Public Schools. b) Employee must accept and cooperate with the medical services provided by the panel physician or on his referral. Failure to do so may bar entitlement to workers compensation benefits. c) Employee must submit a doctor's status report on the injury after the first five consecutive calendar days of absence to employee's supervisor, and monthly thereafter if unable to return to work or six month limitation. d) Employee must submit a doctor's status report upon returning to work indicating release to duty with or without limitations. A doctor s status report will be required after all follow up visits i as well, and shall indicate release to duty with or without limitations.
6 Any school employee who is injured while participating in school duties must adhere to the procedures listed below. All workers compensation forms are located at: Then choose Departments> Finance > Benefits and Retirement (forms are at the bottom of the page). Injured employees should immediately inform their supervisor and/or school nurse. Failure to report a workplace injury could result in denial of Workers Compensation coverage. At the time of incident notification, the employee will receive certain documentation, an injury report will be completed, and the employee will sign for a copy of School s approved panel of physicians from which he or she may choose a medical doctor. In the case of an emergency that requires immediate hospital care, the employee will receive the panel information after reporting the injury, in person, to his or her supervisor. All follow up medical treatment must be completed by a panel doctor or approved through one. Medical care not completed directly through a panel doctor or recommended specialist might not be covered by workers compensation.
7 The employee shall provide a detailed written account of the reported incident and identify any other personnel present at the time. This signed statement is to be completed as soon as possible after the incident not to exceed two business days unless the employee is incapacitated. Upon visiting a panel physician, the employee shall notify the office staff that the visit is in connection to a workplace injury with the School. Physician visits as a result of a workplace injury should not be billed to the employee s group health insurance. Employees will be reimbursed for expenses associated with approved medical care as defined d by the Workers Compensation Law. Any employee who visits a physician for a workplace injury shall present to his or her supervisor, and School appointed personnel written documentation from that physician specifically detailing any restrictions (or none) for a (if any) specified time. No employee will be permitted to return to work in any capacity without such written documentation.
