A comprehensive wellness program with incentive management capabilities. Our History MBA is operated by Emerson Reid the largest wholesale employee benefits General Agent in NY, NJ, CT, and PA. Emerson Reid was founded in 1972 as a Statutory Disability wholesale distributor serving the entire state of NY and grew to be the largest DBL broker in New York. In 1998, Emerson Reid entered the medical wholesale business to support independent insurance brokers in the sale and service of employee benefits products. Today, Emerson Reid has over 150 employees and manages over $3 billion in health care premium, over 10,000 group statutory disability policies and 5,000+ ancillary line cases. In 2006, Emerson Reid established My Benefit Advisor to provide employers with best-in-class service on behalf of a select group of insurance brokers. 5 Advisory Team The MBA client advisory team consists of talented professionals with wide-ranging areas of expertise to provide clients with first-class support in all aspects of their employee benefits program. Advisor Account Executive The client s trusted advisor for long term consulting in establishing a successful employee benefits program, inclusive of appropriate coverage, cost control, contribution strategies, plan design, and ancillary products. A seasoned insurance veteran who brings the unique perspective of having consulted hundreds of benefits programs for employers of all sizes and has valuable working relationships with carrier representatives. Account Manager Implementation Specialist A licensed Account Manager is responsible to assist with the daily administration of the employee benefits package. Areas of assistance include, but are not limited to, claims support, general questions on policy and procedure, billing inquiries and transaction processing. An Implementation Specialist works with the client and account manager to ensure new clients get on board properly and are maximizing our client resources and services for the most positive customer service experience. Benefits Counsel Ancillary & Worksite Specialist Our on-staff counsel monitors industry changes, legislative news, and health care reform requirements to prepare useful communication bulletins that help simplify these complicated issues for our clients. An ancillary expert will educate the client on how to best complete the employee benefits program through employer sponsored and/or voluntary products and then will take the lead on negotiating and securing the desired plans. Technology Specialist Disability/TDB Department Our Technology Specialist provides demonstrations and in-depth training on our online enrollment and benefits administration system, client portal, and our vendor solutions including wellness, payroll, HRIS, and employee shopping rewards program. Considered the premium DBL agent in NY and NJ, our staff can access up to 18 carriers and specializes in competitive pricing, administration, and servicing of state mandated disability plans across all statutory states. 7 Benefits Consulting In today s market, a basic presentation of rates and benefits is not enough. Strategies around funding, contributions, voluntary offerings, and wellness are an integral part of the process. We tailor a comprehensive benefits program specific to our client s organizational culture and financial objectives. Licenses Our Benefits Consulting Services: Complete analysis of your existing benefits program Proactive renewal process to include carrier negotiation, large claim review, contribution analysis, provider match (disruption), and recommendation of carrier and plan alternatives Education and advice on alternative funding strategies: fully insured vs. self-funding or minimum premium My Benefit Advisor is licensed in nearly all states providing convenience for our clients with multi-state locations. Review of third-party administrator options, including: Health Savings Accounts, Health Reimbursement Arrangements, Flexible Spending Accounts, Section 125, Dependant Care Accounts and Transit Communication strategy of market, carrier and plan changes to include quarterly on-site reviews and various technology based tools to ensure proper administration and usage of benefits programs Multi-year cost containment strategies Best practices to include ancillary and voluntary benefits analysis Advice on wellness strategies with premium incentive programs My Benefit Advisor 7

8 Enrollment Experience To help facilitate new relationships, an Implementation Specialist will coordinate the installation of a series of tools to help employers with the open enrollment process, including: On-Demand Benefits Video On-Demand Benefits Video Our enrollment videos are hosted online and can be password protected. The table of contents enables viewers to skip or revisit sections as desired. The attachment tab includes supporting documents and web links. Employees can view the video from their computer, tablet, or mobile device. In addition to traditional in-person and webinar enrollment meetings, MBA can replicate the presentation in an on-demand video format complete with web links and supporting documents. This is useful for employees who were unable to attend, were hired at a later date, or those who want to re-review the information. Employers have the option of recording an introductory message, which is done easily via telephone. Benefits Guidebook MBA can assist employers with developing a benefits guidebook for distribution to their employees. The number of employees determines eligibility and guidebook format. Our guidebooks help improve the communication of benefits programs and plan details to employees. Employee Hotline MBA can provide wallet-sized cards to distribute to employees highlighting an 800 number for answers about their benefits package. The line is activated 30 days before and remains on 30 days after their renewal date. This resource helps to ease the burden away from the HR Administrator regarding general employee questions about the benefit plans offered. Online Enrollment System MBA s enrollment system is available for mid-to-large size employers to improve and streamline benefits communication complete with plan summary documents, side-by-side comparisons, plan rates, and carrier resources. Employees will be delighted with a faster, more convenient and paperless enrollment experience. HR Administrators benefit from the ability to track the completion of employees enrollment and an easy tool to generate HR and payroll deduction reports in Excel.

10 Benefits Guidebook Employee Hotline Our benefits guidebooks help employers communicate their benefits program to employees. The MBA Implementation Specialist will obtain the plan information from the Account Manager and coordinate with the employer for any additional information needed. The number of employees determine the appropriate format and whether electronic delivery or printed copies are appropriate. At a minimum, the guidebooks feature an attractive cover, opening statement, and benefit plan designs. In NY, the Department of Financial Services may require MBA to charge a fee to include content outside the area of the benefits programs. The employee hotline is a direct, confidential resource for employees to call with questions related to their upcoming renewal options. If employees have questions on how a specific benefit will be covered, what their contributions will be, or simply how to choose a Primary Care Physician (PCP), they can call us directly, thus removing the burden from the HR Administrator. The hotline remains opens for 30 days after the renewal date. Employees who call after that period will be directed to the Benefits Administrator or the appropriate insurance carrier s member service number.

11 Online Enrollment System My Benefit Advisor can provide an employer with an employee benefits communication portal and/or a full employee self-service enrollment system to update personal information, add dependents, and make benefit elections. The system features a class-based rules engine enabling employees to only see what is offered to them. Employees can view side-by-side plan comparisons, rates per pay period, and benefit summaries to assist with making decisions. Employers can track the completion of each employee s enrollment and assist where needed. The reporting tool features custom report building, the ability to save report templates, and easy export to Excel. After the open enrollment period, the system can be used to manage life events, new hires, and dependents. Enrollment System If the carrier accepts electronic enrollment data, MBA will configure the enrollment system with employer s logo, benefit plans, and company information to conduct the open enrollment process. Communications System If the carrier requires hard copy enrollment forms with wet signatures, the system can be set up to communicate benefits and provide access to their appropriate forms for the employee to complete and return to the Benefits Administrator. Step-by-Step Enrollment Process The system guides employees through the enrollment process with a step-by-step checklist. Each section contains all plan information, rates, and documents. The compare plan feature is available when multiple plans are offered. My Benefit Advisor 11

12 Benefits Management The MBA approach to managing our clients benefits programs focuses on providing in-depth consultative advice with an ease of doing business and regular proactive communication. HR & Benefits Administration System Our HR & benefits administration system is provided to clients as part of our communication and enrollment system. Post open enrollment, employers can leverage this HR database to easily access and report on HR data, accurately report payroll deductions, reconcile carrier bills faster, manage dependents and emergency contacts, and give employees access to submit life event changes and new hire enrollments. A free mobile app (Apple and Android) is available which enables employees to access any posted company documents directly on their smart phone. Reporting: Quickly access system reports or create a report featuring any data field in the system. Recent Activity: A dashboard summarizes recent activity with quick links to employee records. Designated Account Manager Clients are assigned a designated Account Manager who is available to assist with the daily administration of their employee benefits package. Areas of assistance include, but are not limited to, claims support, general questions on policy and procedure, and billing inquiries. Our Account Managers attend regular training and continuing education meetings. These include carrier product updates, carrier policy and procedures, industry updates and legislative news. Transaction Processing Within our team is a dedicated resource to handle all transactions to carriers on the client s behalf. This includes new hire enrollments, terminations, life event changes, and COBRA coordination. Employee Management: Store, manage and report on numerous employee record data fields.

13 Communication In addition to discussions with Advisors and Account Managers, proactive services calls, and periodic onsite visits, we keep clients well informed of benefit and legislative news through several ongoing communication methods. These methods include our client portal, newsletters, notifications, webinars and on-demand videos. Client Portal Through our secure website, HR Administrators can retrieve benefit plan information, access compliance bulletins and forms, view carrier news, and interact with other HR Administrators through the Community Portal. Claims & Healthcare Advocacy Employees may contact MBA for assistance with claim resolution and related issues. When the issues become clinical in nature, such as medical procedure approvals and healthcare decisions, we ll connect the employee with a registered nurse to guide them in the right direction. Proactive Renewal Process Account Managers have a structured 12-month renewal calendar with automated reminders built into our account management system to help ensure we are proactively managing and communicating the renewal process to our clients. 14 Compliance Assistance MBA provides compliance assistance to employers through a combination of our on-staff benefits counsel and several key third party vendors. In addition we can help employers research complex topics of benefits administration. Self-Help Compliance Tools MBA has developed several self-help compliance tools featuring yes/no questions and informational guidance for our clients. Answering no to a question indicates the client may be out of compliance and should seek legal advice. The topics include COBRA, ERISA, FMLA, HIPAA, and USERRA. COBRA Administration MBA offers complementary COBRA service which provides a number of services that are designed to relieve the burden that COBRA currently places on employers*. Our COBRA-Care Advisors Help By: Providing a real time, full-featured COBRA administration system with web based access Monitoring eligibility and qualifying events for all employees enrolled in COBRA qualifying health plans Offering immediate access to our reporting system Preparing and mailing all required notices to comply with all regulations Handling carrier enrollment/term notifications for COBRA participants Assuming administration for all current employees on COBRA Collecting all premiums from COBRA participants Training all necessary Human Resource personnel on the system Timely Guidance on Important Topics As new compliance and reform topics come forward, our on-staff counsel will prepare bulletins to provide guidance on these complicated benefit and legislative issues Document Preparation As part of our standard service component, we can arrange for employers 5500 documents to be prepared, utilizing state-of-the-art software to help ensure compliance with all filing requirements. For New York Employers, a small fee must be applied per the New York Department of Financial Services Service Includes: Development of a filing schedule for each plan Timeline management tool with built-in follow up notices Assistance in gathering Schedule A & C data Sample client letter with detailed instructions Signature ready Form 5500 s with Schedules Summary Annual Reports (SAR) Consulting advice regarding ERISA Reporting Assistance responding to DOL / EBSA letters Preparation of Form 5558 as necessary Communication of new rules and regulations * The service is complementary for clients with 10 or more lives. For clients under 10 lives, the service is available for a negotiated, below market fee. In New York the Department of Financial Services requires a fee to be charged. If no one is eligible for COBRA then there is no charge.

16 Employee Programs Wellness Strategy MBA offers employers the choice of providing their employees with access to an MBA sponsored Wellness Benefit to boost employee morale and company goodwill, or implementation of a comprehensive incentive based Wellness Program to help manage health premiums. The Wellness Benefit Employees are invited to register on the MBA sponsored wellness website for access to telephonic health coaching, fitness & nutrition planner, personal wellness journal, healthy recipes, and to compete in our pedometer tracking contest for chances to win prizes. The Wellness Program Large group employers can engage the vendor to design, implement and manage a comprehensive wellness program with an incentive management reporting system to help manage health care costs. Services available include: Health risk assessments Onsite health & biometrics screenings Fitness classes Health fair management Lunch & learns Health coaching Vaccination clinics Reward programs and more... Employee Rewards MBA has partnered with an innovative technology company that offers an e-commerce portal to leading merchants where employees can earn cash back, incentives and instant rewards just for shopping. From Best Buy and Target to thousands of other retailers, this portal lets employees shop where they re already shopping online today. Same stores, same merchandise. But now they can leverage the power of cash back shopping to receive a tangible benefit. Employers can position this as an exciting addition to an existing benefits program, at no cost to the employee. How It Works To drive online sales, retailers of all sizes have created programs that offer incentives of 1% to 40% cash back for shopping online. This powerful e-commerce engine searches all these retailers and accesses the cash back for the employee. Retailers get new online sales and employees get cash back and other incentives for shopping. Extend The Program To Your Customers Employers have the choice of directing their employees to the MBA rewards portal at no cost to them or their employees, or for a nominal one-time fee, the employer can receive a private labeled version of the portal to share with both their employees and their customer base.
