5 Bella Cosmetology College was founded in September of 2009 by Cindy Rhea and then purchased by Mrs. Berry. Mr. and Mrs. Berry founded Touch of Class School of Cosmetology in June of 1993 and sold TOC in After missing the excitement of seeing students learn a new career Mrs. Berry bought Bella Cosmetology College in September of Mrs. Berry graduated from Marinellos Beauty College in 1964 and ran her own salon for 30 years before giving it up to run the school full time. She has attended many continuing education classes during those years. Mr. Berry ran a market for several big chain grocery stores and managed 6 to 8 employees. He did this for about 25 years until Mrs. Berry convinced him to go to Cosmetology school. He was very reluctant, but it has proven to be an excellent move. Mr. and Mrs. Berry became licensed instructors and completed two years of vocational education training at TAMU of Commerce to be able to teach all phases of Cosmetology. They received training from NACCAS by attending many of their workshops and have studied financial aid for at least 14 years. Their vision was to open a school and to teach something they both are passionate about, to others. They have that same vision today. MISSION STATEMENT 1. To establish well organized courses of study to aid in preparing graduates for licensure and competitive job entry level positions. 2. To consistently offer a supervised clinic with appropriate equipment for students to perform and consistently improve their cosmetology skills and performance ability. 3. To continually develop current teaching procedures and educational practice for each course of study. Bella Cosmetology College is located at 410 West Broadway St. Winnsboro, Texas. Our institution has 7000 square feet of space with modern stations. Bella College offers the latest in teaching techniques, including braiding, hair extensions and chemical services. Advanced classes are offered frequently throughout the year by different product companies such as Ez flow and Farouk. Bella College is a clock hour school with four semesters. We report hours to Texas Department of Licensing and Regulations. Bella Cosmetology College 2

6 believes the integrity of the profession rest in the quality of skilled practitioners. We strive to educate students to portray a positive attitude towards life, themselves and the profession.. ENROLLMENT SCHEDULE AND SCHEDULED CLOSINGS Bella College enrolls students monthly using the first Monday of each month as class start date. We strive to assign students class schedules that best meets their needs; however Bella College reserves the right to change or rearrange enrollment schedules. The institution is open for training Monday through Friday 9:00 am to 4:00 pm, on a year round basis. Bella College is closed Saturday and Sunday. The following holidays are observed: Memorial Day the week of the 4 th of July, Labor Day, Thanksgiving Day and the week of Christmas. The school, at its option may choose to close the facility for in house training or other special events. Such closings will be posted 10 days prior to the actual closing. ADMISSIONS REQUIREMENTS For enrollment at Bella College, all applicants must provide copies of the following: proof of education (high school diploma, high school transcript, GED certificate, certificate of homeschool completion), proof of age, and proof of citizenship, two small pictures, a non-refundable permit fee of $25.00, and a $ application fee. Applicants must attend, prior to enrollment, 1 day of orientation prior to enrollment, be beyond the age of compulsory education (17 years in the state of Texas), and have met all financial requirements. Applicants for Instructor Training must have a high school diploma, GED certificate or homeschool certificate and a copy of their state license. All students will be admitted without regards to race, creed, religion, age, color, ethnic origin, military status or sex. Transfer Students: Students accepted for transfer will receive credit for documented hours as confirmed by their transcript from the Texas Department of Licensing and Regulation, or other state agencies up to 50 % of their program length, any hours past 50% the school will only accept 70%of those hours as transferable. The school does not recruit students already attending or admitted to another school offering a similar program of study. ABILITY TO BENEFIT Bella Cosmetology College does not offer an Ability to Benefit test. DRESS CODE 3

7 In order for our students to look professional while attending classes at Bella College, they will wear all black -shirts and all black pants. This attire must be worn at all times while in attendance. Shoes must be clean, enclosed, soles of material that will not slip in water and no heels over 1 inch. STUDENTS CREATIONS We teach our students to use their imagination, while teaching them the basics. We allow them to explore possibilities in styles and color. Year Rape Burglary Theft Assault Robbery Drug Violations Murder Arson Hate Crimes CAREER OPPORTUNITIES ARE LIMITED ONLY BY YOU In the program of Cosmetology you could become a Salon Owner, Salon Manager, Stylist, Color Technician, Make up Artist, Platform Artist, Skin Care Technician, Cosmetology Instructor, Product Educator, Cosmetology School Owner, if you choose the Cosmetology program you will be provided 54 weeks of instruction in hair, skin and nails with a total of 4 semesters for 1500 clock hours. DESCRIPTION: The primary purpose of the Cosmetology 4

8 course is to train the students the basic skills, proper work habits, safety and sanitation procedures and positive attitudes necessary for licensure and job entry level positions in Cosmetology or related fields. FORMAT: The clock hour education is provided through a regimented set of learning steps which address specific tasks necessary for graduation, state licensure and job entry level skills. Clinic equipment, implements, and products are comparable to those used in the industry. Each student will receive instruction that relates to the performance of useful, creative, and productive career oriented activities. The course is presented through well-developed lesson plans which reflect current educational methods. Subjects are presented by means of lecture, demonstration and student participation. Audio-visual aids, guest speakers, field trips, and other related learning methods are used in the course. COURSE GOALS: At the completion of the course the graduate will be able to: 1. Project a positive attitude, a sense of personal integrity, and self-confidence. 2. Practice proper grooming and effective communication skills. 3. Understand employer-employee relationships and respect the need to deliver a worthy service for value received. 4. Perform the basic skills in hair styling, shaping, bleaching, tinting, chemical reformation, scalp/hair conditioning, facials, and manicures. If you choose the instructor program, you would be provided 25 weeks of instruction, in a total of 2 semesters for 750 clock hrs. With 1 yr. Salon experience you could qualify for the 500 hr. Instructor program, which is 2 semesters of 8.5 weeks. PROFESSIONAL CONSIDERATIONS Bella College would like you to be aware of factors that could affect your success in the field of Cosmetology, or instructing. The work associated with these fields could be strenuous and physically demanding, sometimes requiring long hours of standing. 7

11 There is almost always exposure to various chemicals and fumes that could cause allergic reactions. The practice of safety and sanitation is extremely important and must be practiced at all times for the safety of your self and the consumer. Employers will expect you to arrive on time and stay until clients have departed. Compensation will vary in the profession; it can be on straight commission, sliding scale commission, salary and commission, straight salary, hourly wage or independent contracting. You should expect on making an investment to start, depending on what tools you will use, paying for your license, advertisings and uniforms, these will vary from one salon to another. TDLR will be inspecting you salon at least twice a year and can issue fines if found you are not following the rules. You will be required to have an updated rule book on site when inspected. Every individual who applies for a license with the Texas Department of Licensing and Regulation ( the Department ) is subject to a criminal background check to determine his or her suitability for the license. In 2009, the Texas Legislature enacted new provisions to allow a person to find out, before applying, whether he or she would likely be denied a license due to his or her criminal history. This was due to the time and expense involved in applying for a license, which in some cases includes completing required education and taking an examination. See Section and Chapter 53, Subchapter D, of the Occupations Code, which allows a person to request a criminal history evaluation letter from the Department, prior to actually applying for a license. The Department has issued Criminal Conviction Guidelines for each occupation the Department licenses. These guidelines list the crimes which are considered to relate to each occupation, as well as other factors that affect the decisions of the Department. When a request for a criminal history evaluation letter is filed, the Department will review the requestor s criminal history with reference to these guidelines, the same as if an actual license application had been filed. View the Criminal Conviction Guidelines To request a criminal history evaluation letter, an individual must: a. submit a request form, b. complete a criminal history questionnaire 8

12 for each crime for which he or she was convicted or placed on deferred adjudication, and c. pay a fee ($25.00). COSMETOLOGISTS Crimes involving prohibited sexual conduct or involving children as victims. Reasons: Licensees have direct contact with members of the general public often in settings with no one else present. A person with a predisposition for crimes involving prohibited sexual conduct or involving children as victims would have the opportunity to engage in further similar conduct. Crimes against the person such as homicide, kidnapping and assault. Reasons: Licensees have direct contact with persons in situations that have potential for confrontational behavior. A person with a predisposition of a violent response would pose a risk to the public. GRADUATION REQUIREMENTS Students must pass all exams given by Bella College with a minimum score of 70 % average, an equivalent of a C, have completed all practical assignments and completed all required clock hours for the specific program of study pursued by the student. All fees owed to the institution including any late charges that may have accrued must be paid in full unless other arrangements have been made. All items belonging to the school must be returned in satisfactory condition or the student will be responsible to pay for those items. A diploma will be awarded upon completion of the conditions outlined and if a student loan has been received the student must receive an exit interview. LICENSING REQUIREMENTS Once you have graduated school you must pass a written and practical exam given by PSI a national testing service. You must pass the written portion of the exam first which will cost $55.00, then the practical, which will cost $ After you have passed the exam you will need to pay TDLR, your state licensing agency, for a license to practice Cosmetology or related fields ( instructing.) These examinations are given in several locations around Texas and are scheduled by TDLR, the school has no control over the scheduling dates. Any cost related to travel, lodging and other expenses related to the exam are solely the 9

13 responsibility of the student. EMPLOYMENT ASSISTANCE The institution does not guarantee employment. However Bella Cosmetology College will offer employment assistance by placing job openings on the bulletin board. In a 2007 a job demand survey, conducted by NACCAS, results indicated that salons in Texas plan on hiring 26,574 new employees in the next 12 months. Most importantly, 69% of Texas salons owners who attempted to hire new employees in 2006 said they were unable to find properly trained applicants. This would mean that jobs would be immediately available for salon professionals. GRADING POLICY Students are given a theory study schedule and a minimum number of practical requirements. Diplomas will be issued upon satisfactory completion of the program in both academics and attendance. Students failing to achieve 70% in theory or laboratory assignments may request and be granted time to make up work in a 30 day period. SATISFACTORY ACADEMIC PROGRESS POLICY The Satisfactory Academic Progress Policy is consistently applied to all students enrolled at the Bella Cosmetology College. It is printed in the catalog to ensure that all students receive a copy prior to enrollment. The policy complies with the guidelines established by the National Accrediting Commission of Career Arts and Sciences (NACCAS) and the federal regulations established by the United States Department of Education, partial funding assistance, or self-pay. New SAP definitions went into effect on July 1, Bella Cosmetology School developed policies to determine the academic standards that students are expected to meet and built a means and schedule of measuring the achievement of those Pace and Quantitative standards. SAP standards are established by the Office of Academic Affairs. The relevant SAP policies are summarized below. All students must maintain Satisfactory Academic Progress according to the following standards in order to continue enrollment. Satisfactory Academic Progress is measured at the end of each 10

14 payment period, and will be checked prior to disbursement of aid. Same As or Stricter Than: Bella Cosmetology School s SAP policy for Title IV, HEA students is the same as Bella Cosmetology School s standards for students enrolled in the same educational programs who are not receiving Title IV, HEA funding. The Director in the financial aid office reviews the Title IV, HEA SAP policy to ensure it meets all federal requirements. The Director Education notifies financial aid office if Bella Cosmetology School changes its academic policies. EVALUATION PERIODS Students are evaluated for Satisfactory Academic Progress as follows: Cosmetology 450, 900, 1200 (actual) clock hours Instructor 375 (actual) clock hours Instructor 250 (actual) clock hour *Transfer Students- Midpoint of the contracted hours or the established evaluation periods, whichever comes first. Evaluations will determine if the student has met the minimum requirements for satisfactory academic progress. The frequency of evaluations ensures that students have ample opportunity to meet both the attendance and academic progress requirements of at least one evaluation by midpoint in the course. Academic Year Definition: 900 clock hours and 26 weeks of instruction for Title IV, HEA funding. ATTENDANCE PROGRESS EVALUATIONS Students are required to attend a minimum of 70% of the hours possible based on the applicable attendance schedule in order to be considered maintaining satisfactory attendance progress. Evaluations are conducted at the end of each evaluation period to determine if the student has met the minimum requirements. The attendance percentage is determined by dividing the total hours accrued by the total number of hours scheduled. At the end of each evaluation period, the school will determine if the student has maintained at least 70% cumulative attendance since the beginning of the course which indicates that, given the same attendance rate, the student will graduate within the maximum time frame allowed. MAXIMUM TIME FRAME Quantitative Requirement The maximum time (which does not exceed 143% of the course length) allowed for students to complete each course at satisfactory academic progress is stated below: COURSE Cosmetology (Full time, 30 hrs/wk) Hours Cosmetology (Part time,20 hrs/wk) Hours 11

15 Instructor Full time, 30 hrs/wk Instructor Part time, 20 hrs/wk- 750 Instructor Full time, 30 hrs/wk Instructor Part time, 20 hrs/wk- 500 MAXIMUM TIME ALLOWED WEEKS SCHEDULED HOURS The maximum time allowed for transfer students who need less than the full course requirements or part-time students will be determined based on 70% of the scheduled hours. ACADEMIC PROGRESS EVALUATIONS Qualitative Requirement The qualitative element used to determine academic progress is a reasonable system of grades as determined by assigned academic learning. Students are assigned academic learning and a minimum number of practical experiences. Academic learning is evaluated after each unit of study. Practical assignments are evaluated as completed and counted toward course completion only when rated as satisfactory or better (the computer system will reflect completion of the practical assignment as a 100% rating). If the performance does not meet satisfactory requirements, it is not counted and the performance must be repeated. At least two comprehensive practical skills evaluations will be conducted during the course of study. Practical skills are evaluated according to text procedures and set forth in practical skills evaluation criteria adopted by the school. Students must maintain a written grade average of 70% and pass a final written and practical exam prior to graduation. Students must make up failed or missed tests and incomplete assignments. Numerical grades are considered according to the following scale: A EXCELLENT 80-89B VERY GOOD 70-79C SATISFACTORY 0-69D UNSATISFACTORY DETERMINATION OF PROGRESS STATUS Students meeting the minimum requirements for academics and attendance at the evaluation point are considered to be making satisfactory academic progress until the next scheduled evaluation. Students will receive a hard-copy of their Satisfactory Academic Progress Determination at the time of each of the evaluations. Students deemed not maintaining Satisfactory Academic Progress may have their Title IV 12

16 Funding interrupted, unless the student is on warning or has prevailed upon appeal resulting in a status of probation. Financial Aid Warning Students who fail to meet minimum requirements for attendance or academic progress are placed on warning and considered to be making satisfactory academic progress while during the warning period. The student will be advised in writing on the actions required to attain satisfactory academic progress by the next evaluation. If at the end of the warning period, the student has still not met both the attendance and academic requirements. APPEAL PROCESS: If the student does not make SAP at the end of the Financial Aid Warning period, they lose their Title IV, HEA financial aid eligibility and will be placed on Academic development Status, with a loss of Title IV, HEA funding, with the right to appeal. The student may have the opportunity to have their Title IV, HEA financial aid eligibility reinstated by appealing the Academic development Status, with a loss of Title IV, HEA funding decision and placed on Financial Aid Probation if the appeal is granted. The student has five (5) days to institute an appeal. The appeal must be given to the School Director, who in turn will meet with the Financial Aid Director to make a decision on the appeal. The student must describe any unusual circumstance(s) that the student believes deserve special consideration. The basis on which a student may file an appeal: death of a relative, an injury, or illness of the student or other special circumstance. The student must provide supporting documents and describe in writing any unusual circumstance(s) that the student believes deserve special consideration. The student must provide information as to why they did not make SAP and what has changed that will allow them to make SAP by the next evaluation point. Once the School Director receives the appeal, they will evaluate the appeal and provide a decision within ten (10) business days. The School Director will notify the student in writing of the decision and that decision is final. Financial Aid Probation: If Financial Aid Probation Status is granted, the student will regain Title IV, HEA eligibility for the next eligible payment period only. The student must be making SAP at the end of the 13

17 payment period to regain Title IV, HEA funding for the next payment period. When a student is placed on Financial Aid Probation status, he or she will be required to do the following: 1. Agree to a written academic plan that specifies how the student will regain SAP. The plan may include but is not limited to mandatory tutoring, scheduled advisement sessions, extra course assignments, repeating a course for which the student received a failing grade, and/or repeating a course from which the student withdrew. 2. Sign the academic plan (a copy will be kept in the student s file). A student on Financial Aid Probation because of a successful appeal is eligible for Title IV, HEA funds for ONE payment period only. Students who regains SAP at the end of the next payment period will have regained full eligibility for Title IV, HEA funding. The student on financial aid warning, probation or Title IV, HEA funding suspension for a payment period may not receive Title IV, HEA funding for the subsequent period unless the student makes SAP Reinstatement of aid is limited to the period under evaluation. Students making SAP by the conclusion of the Warning or Probation period will be removed from the warning/probation status and will regain eligibility for Title IV, HEA funding. Reinstatement of Title IV, HEA Aid: A prior student requesting to be reinstated as an active student, based on whatever reasons or circumstances, should do so in writing to Bella Cosmetology School Director. Supportive documentation and/or information concerning any mitigating circumstances should be noted in the request. The requesting prior student shall be notified of the Reinstatement Review within five (5) days following the decision of School Director. INTERRUPTIONS, COURSE INCOMPLETES, WITHDRAWALS If enrollment is temporarily interrupted for a Leave of Absence, (not to extend beyond 30 days) and documentation to support the required leave of absence is submitted to Bella Cosmetology School director (i.e. Doctor s excuse, etc.), the student will return to school in the same progress status as prior to the leave of absence. Hours elapsed during a leave of absence will extend the student s contract period and maximum time frame by the same number of days taken in the leave of absence and will not be included in the student's cumulative attendance percentage calculation. Students who withdraw prior to completion of the course and wish to reenroll will return in the same satisfactory academic progress status as at the time of withdrawal. Reinstatement before 180 days Students who withdraw prior to completion of the course and wish to re- 14

18 enroll within six months (180 days) of the original official withdrawal date will return in the same satisfactory academic progress status as at the time of withdrawal. Students who have been terminated or withdrew from school may re-enroll (if determined eligible) within 180 days and will not incur additional charges, however these students will be responsible to pay any remaining balance from the previous enrollment that cannot be covered with reinstated federal funds. Students who have been terminated or withdrew from school and re-enroll (if determined eligible), after more than 180 days will pay a $100 application fee and will be charged for contracted hours at the current tuition rate. All reenrolling students will be provided Bella Cosmetology School s reenrollment policy and will be evaluated by Bella Cosmetology School Director for placement. Students applying for reentry or transfer-in from other schools will be required, as a condition of enrollment, to bring delinquent prior student loans to a current status. Course Incompletes, Repetions and Non Credit Remedial Courses Course incompletes, repetitions and noncredit remedial courses do not apply to this institution s form of instruction. Bella Cosmetology School does not issue incompletes and does not offer non-credit remedial courses. Students do not repeat courses. All hours attended are considered attempted. It is not possible for students to withdraw from individual subjects. Students withdrawing from school will receive credit for all work successfully completed up to the point of withdrawal. Transfer clock hours received will not be included in the calculation of the student's GPA; however these hours will be counted toward the quantitative requirements. TRANSFER HOURS With regard to Satisfactory Academic Progress, a student s transfer hours will be counted as both attempted and earned hours for the purpose of determining when the allowable maximum time frame has been exhausted. TERMS OF RE-ENTRY Students failing to complete or one who withdraws will have notice placed in his or her file noting their progress at the time of withdrawal. Students making satisfactory progress at the point of withdrawal may apply for reentry and will be considered in satisfactory progress when they return. Students who withdrew or terminated who were not making satisfactory progress at the last day of attendance will be re-entered in unsatisfactory progress and could be reestablished in one month. LEAVE OF ABSENCE A student returning from a leave of absence or other official interruption will return in the same satisfactory progress status they were in when they were granted an approved leave of absence. An approved leave of absence must fall into the guide lines set by the Department of education. An approved leave of absence must be requested in writing prior to the 15

19 leave on a form provided by the school, signed and dated by the student. The leave will start on the last day of attendance for a period no longer than 30 days, if the students needs an additional 30 days they will need to request it in writing or by speaking to a director over the phone. If the student does not return from the leave, the last day of attendance will be used to calculate the refund. If it is impossible for a student to come in to sign a leave, due to circumstances, a family member or friend may pick up a form and bring it back signed with documentation. Should a student not be in attendance for 14 days with out a leave the student will be considered with drawn and the last day of attendance will be considered the withdrawal date. When the student returns from the leave of absence he or she will be allowed to complete the course work that the student began prior to the leave at no additional cost. INSTITUTIONAL REFUND POLICY Mandated by the state of Texas (a) The holder of a private beauty culture school license shall maintain a refund policy to provide for the refund of any unused part of tuition, fees, and other charges paid by a student who, at the expiration of the cancellation period established under Section : (1) fails to enter the course of training; (2) withdraws from the course of training; or (3) is terminated from the course of training before completion of the course. (b) The refund policy must provide that: (1) the refund is based on the period of the student's enrollment, computed on the basis of course time expressed in scheduled hours, as specified by an enrollment agreement, contract, or other document acceptable to the department; (2) the effective date of the termination for refund purposes is the earliest of: (A) the last date of attendance, if the student is terminated by the school; (B) the date the license holder receives the student's written notice of withdrawal; or (C) 10 school days after the last date of attendance; and (3) the school may retain not more than $100 if: (A) tuition is collected before the course of training begins; and (B) the student fails to withdraw from the course of training before the cancellation period expires. WITHDRAWAL OR TERMINATION OF STUDENT IN EACH PROGRAM 16

20 (a) If a student begins a course of training at a private beauty culture school that is scheduled to run not more than 12 months and, during the last 50 percent of the course, withdraws from the course or is terminated by the school, the school: (1) may retain 100 percent of the tuition and fees paid by the student; and (2) is not obligated to refund any additional outstanding tuition. (b) If a student begins a course of training at a private beauty culture school that is scheduled to run not more than 12 months and, before the last 50 percent of the course, withdraws from the course or is terminated by the school, the school shall refund: (1) 90 percent of any outstanding tuition for a withdrawal or termination that occurs during the first week or first one-tenth of the course, whichever period is shorter; (2) 80 percent of any outstanding tuition for a withdrawal or termination that occurs after the first week or first one-tenth of the course, whichever period is shorter, but within the first three weeks of the course; (3) 75 percent of any outstanding tuition for a withdrawal or termination that occurs after the first three weeks of the course but not later than the completion of the first 25 percent of the course; and (4) 50 percent of any outstanding tuition for a withdrawal or termination that occurs not later than the completion of the first 50 percent of the course. (c) A refund owed under this section must be paid not later than the 30th day after the date the student becomes eligible for the refund. Institutional refunds apply to all terminations for any reason, by either party, including student decision, course or program cancelation, or school closure. Students who do not return from a leave of absence, the last day of attendance will be used for all calculations. The institution will perform a refund calculation and all refunds will be made in a timely manner. GROUNDS FOR TERMINATION The students failure to comply to the rules and regulations and policies of the school regarding satisfactory attendance and academic progress and standard of conduct or non payment are grounds for termination. A termination fee of $100 will be charged. Items such as books and kit items purchased through the school become the property of the student and are non-refundable. Items issued to the student for use while in school must be returned in good shape when they graduate or drop. If a student wishes to withdraw he/she must first: 1. Put it in writing 2. List reasons for withdrawal 3. List date of official withdrawal 4. Summit to director s office 5. Turn in all equipment belong to the school and make arrangements to pay any outstanding debits. RECORD OF ATTENDANCE A time clock with individual student time cards is used for general attendance records. Attendance records are maintained in a permanent file by the school. Attendance records are recorded and kept in a data base that generates monthly hour reports and an individual attendance history is always available. 17

21 TARDINESS Students are encouraged to be punctual as a means of promoting professional job enhancement. Students may not enter the theory class after the class is in session. Students arriving after 9:15 may not clock in until 10:15, after break. Students who are late and scheduled for theory, May not clock in until after theory is over. RETURN OF TITLE IV, HEA POLICY When you apply for financial aid, you sign a statement that you will use the funds for educational purposes only. Therefore, if you withdraw before completing your program, a portion of the funds you received may have to be returned. Bella Cosmetology College will calculate the amount of tuition to be returned to the Title IV, HEA Federal fund programs according to the policies listed below. RETURN TO TITLE IV FUNDS POLICY This policy applies to students who withdraw official, unofficially or fail to return from a leave of absence or dismissed from enrollment at Bella Cosmetology College. It is separate and distinct from the Bella College refund policy. (Refer to institutional refund policy) The calculated amount of the Return of Title IV, HEA (R2T4) funds that are required for the students affected by this policy, are determined according to the following definitions and procedures as prescribed by regulations. The amount of Title IV, HEA aid earned is based on the amount of time a student spent in academic attendance, and the total aid received; it has no relationship to student s incurred institutional charges. Because these requirements deal only with Title IV, HEA funds, the order of return of unearned funds do not include funds from sources other than the Title IV, HEA programs. Title IV, HEA funds are awarded to the student under the assumption that he/she will attend school for the entire period for which the aid is awarded. When a student withdraws, he/she may no longer be eligible for the full amount of Title IV, HEA funds that were originally scheduled to be received. Therefore, the amount of Federal funds earned must be determined. If the amount disbursed is greater than the amount earned, unearned funds must be returned. The Payment Period for Clock Hour Schools is one-half of the academic year or program length (whichever is less). The Date of Determination is the date that Bella Cosmetology School determines is the withdrawal date. For schools that are required to take attendance, the date of determination is no longer than 14 days after the Last Date of Attendance. The Date of Determination starts the clock for timely refunds of Title IV funds, within 45 days after the Date of Determination. The Withdrawal Date for schools required to take attendance is the Last Date of Attendance (LDA). The institution has 45 days from the date that the institution determines that the student withdrew to return all unearned funds for which it is responsible. The school is required to notify the student if they owe a repayment via written notice. The school must advise the student or parent that they have 14 calendar days 18

22 from the date that the school sent the notification to accept a post withdraw disbursement. If a response is not received from the student or parent within the allowed time frame or the student declines the funds, the school will return any earned funds that the school is holding to the Title IV, HEA programs. Post-withdraw disbursements will occur within 90 days of the date that the student withdrew. WITHDRAWAL POLICY Official Voluntary Withdrawal A student is considered to be Officially withdrawn on the date the student notifies the Financial Aid Director or School Director in writing of their intent to withdraw. The date of the termination for return and refund purposes will be the earliest of the following for official withdrawals: 1. Date student provided official notification of intent to withdraw, in writing. 2. The date the student began the withdrawal from the School s records. A student will be permitted to rescind his notification in writing and continue the program, if so chosen. However, if the student subsequently drops, the student s withdrawal date is the original date of notification of intent to withdraw. Upon receipt of the withdrawal information the School will complete the following: 1. Determine the student s last date of attendance as of the last recorded date of academic attendance on the school s attendance record, and 2. Perform two calculations a. The students ledger card and attendance record are reviewed to determine the calculation of Return of Title IV, HEA funds the student has earned, and if any, the amount of Title IV, HEA funds for which the school is responsible. Returns made to the Federal Funds Account are calculated using the Department s Return of Title IV, HEA Funds Worksheets, scheduled attendance and are based upon the payment period. b. Calculate the school s refund requirement (see school refund calculation). 3. The student s grade record will be updated to reflect his/her final grade. 4. The School will return the amount for any unearned portion of the Title IV, HEA funds for which the school is responsible within 45 days of the date the official notice was provided. 5. If applicable, the School will provide the student with a letter explaining the Title IV, HEA requirements. To include, 19

23 a. The amount of Title IV, HEA assistance the student has earned. This amount is based upon the length of time the student was enrolled in the program based on scheduled attendance and the amount of funds the student received. b. Any returns that will be made to the Federal program on the student s behalf as a result of exiting the program. If a student s scheduled attendance is more than 60% of the payment period, he/she is considered to have earned 100% of the Federal funds received for the payment period. In this case, no funds need to be returned to the Federal funds. c. Advise the student of the amount of unearned Federal funds and tuition and fees that the student must return, if applicable. 6. Supply the student with ledger card record noting outstanding balance due to the school and the available methods of payment. A copy of the completed worksheet, check, letter and final ledger card will be kept in the student s file. In the event a student decides to rescind his or her official notification to withdraw, the student must provide a signed and dated written statement indicating he/she is continuing his or her program of study, and intends to complete the payment period. Title IV, HEA assistance will continue as originally planned. If the student subsequently fails to attend or ceases attendance without completing the payment period, the student s withdrawal date is the original date of notification of intent to withdraw. Unofficial Withdrawal Any student that does not provide official notification of his or her intent to withdraw and is absent for more than 14 consecutive calendar days, fails to maintain satisfactory academic progress, fails to comply with the school s attendance and /or conduct policy, does not meet financial obligations to the school, or violates conditions mentioned in the School contractual agreement, will be subject to termination and considered to have unofficially withdrawn. Within two weeks of the student s last date of academic attendance, the following procedures will take place. 1. The education office will make three attempts to notify the student regarding his/her enrollment status. 2. Determine and record the student s last date of attendance as the last recorded date of academic attendance on the attendance record. 20

24 3. The student s withdrawal date is determined as the date the day after 14 consecutive calendar days of absence. 4. Notify the student in writing of their failure to contact the school and attendance status resulting in the current termination of enrollment. 5. The School calculates the amount of Federal funds the student has earned, and, if any, the amount of Federal funds for which the school is responsible. 6. Calculate the school s refund requirement (see school refund calculation). 7. The School s Controller will return to the Federal fund programs any unearned portion of Title IV funds for which the school is responsible within 45 days of the date the withdrawal determination was made, and record on student s ledger card. 8. If applicable, the School will provide the student with a refund letter explaining Title IV requirements: a. The amount of Title IV aid the student has earned based upon the length of time the student was enrolled and scheduled to attend in the program and the amount of aid the student received. b. Advise the student in writing of the amount of unearned Title IV, HEA aid and tuition and fees that he/she must return, if applicable. 9. Supply the student with final student ledger card showing outstanding balance due the school and available methods of repayment. 10. A copy of the completed worksheet, check, letter, and final ledger card will be kept in the student s file. Withdraw Before 60% The institution must perform a R2T4 to determine the amount of earned aid through the 60% point in each payment period or period of enrollment. The institution will use the Department of Educations prorate schedule to determine the amount of the R2T4 funds the student has earned at the time of withdraw. Withdraw After 60% After the 60% point in the payment period or period of enrollment, a student has earned 100% of the Title IV, HEA funds he or she was scheduled to receive during this period. The institution must still perform a R2T4 to determine the amount of aid that the student has earned. Bella Cosmetology College measures progress in Clock Hours, and uses the payment period for the period of calculation. The Calculation Formula: Determine the amount of Title IV aid that was disbursed plus Title IV, HEA aid that could have been disbursed. Calculate the percentage of Title IV,HEA aid earned: a.) Divide the number of clock hours scheduled to be completed in the payment period as of the last date 21

25 of attendance in the payment period by the total clock hours in the payment period. HOURS SCHEDULED TO COMPLETE TOTAL HOURS IN PERIOD = % EARNED TOTAL HOURS IN PERIOD = % EARNED (rounded to one significant digit to the right of the decimal point, ex = 44.9%.) b.) If this percentage is greater than 60%, the student earns 100%. c.) If this percent is less than or equal to 60%, proceeds with calculation. Percentage earned from (multiplied by) Total aid disbursed, or could have been disbursed = AMOUNT STUDENT EARNED. Subtract the Title IV aid earned from the total disbursed = AMOUNT TO BE RETURNED. 100% minus percent earned = UNEARNED PERCENT Unearned percent (multiplied by) total institutional charges for the period = AMOUNT DUE FROM THE SCHOOL. If the percent of Title IV aid disbursed is greater than the percent unearned (multiplied by) institutional charges for the period, the amount disbursed will be used in place of the percent unearned. If the percent unearned (multiplied by) institutional charges for the period are less than the amount due from the school, the student must return or repay one-half of the remaining unearned Federal Pell Grant. Student is not required to return the overpayment if this amount is equal to or less than 50% of the total grant assistance that was disbursed /or could have been disbursed. The student is also not required to return an overpayment if the amount is $50 or less. Bella Cosmetology College will issue a grant overpayment notice to student within 30 days from the date the school s determination that student withdrew, giving student 45 days to either: 1. Repay the overpayment in full to Bella Cosmetology College or 2. Sign a repayment agreement with the U.S. Department of Education. Order of Return Bella Cosmetology College is authorized to return any excess funds after applying them to current outstanding Cost of Attendance (COA) charges. A copy of the Institutional R2T4 work sheet performed on your behalf is available through the office upon student request. In accordance with Federal regulations, when Title IV, HEA financial aid is involved, the calculated amount of the R2T4 Funds" is allocated in the following order: Unsubsidized Direct Stafford loans (other than PLUS loans Subsidized Direct Stafford loans Direct PLUS loans Federal Pell Grants for which a Return is required Federal Supplemental Educational Opportunity Grant 22
