2 Purpose To evaluate the association of baseline knee cartilage T2 relaxation time values and focal knee lesions in subjects from the Osteoarthritis Initiative (OAI) incidence cohort with future osteoarthritis (OA) related pain medication intake. Methods and Materials 140 subjects (64 males, 76 females) with risk factors for knee OA (such as history of knee injury, history of knee surgery, or family history of knee replacement), years of age, BMI of kg/m² and without knee pain in either knee (WOMAC pain score of zero) at baseline were included in this study. They were selected from the Osteoarthritis Initiative (OAI) incidence cohort. Baseline bilateral standing posterior-anterior fixed flexion knee radiographs were acquired. Right knee radiographs were graded by two radiologists in consensus by using the Kellgren-Lawrence (KL) scoring system Baseline 3T MR images of the right knee were assessed for presence and grade of abnormalities using the whole organ MRI score (WORMS) by two radiologists. Cartilage segmentation and T2 measurements were performed in five compartments: patella, medial/lateral femur/tibia (Figure 1, 2, 3). Knee pain associated medication intake was monitored with a standardized questionnaire over a period of one year after baseline MRI. Multivariate linear and logistic regression models were used to determine significant differences between subjects with versus without pain medication intake, adjusting for gender, age, OA risk factors, and KL-score. 7 Results Pain medication intake for OA over a period of one year was reported for 39 of the 140 subjects (28%). The severity, but not the prevalence of cartilage lesions at baseline was significantly higher in subjects with pain medication intake (p<0.05). Subjects with pain medication intake also showed significantly higher T2 values in all compartments (p<0.05; 44.14ms versus 45.65ms averaged over all compartments). Conclusion More severe cartilage lesions and higher T2 values at baseline are associated with future OA related pain medication intake in middle-aged subjects from the OAI incidence cohort. Knee cartilage T2 relaxation time values and severity of focal cartilage lesions might be indicators for future osteoarthritis related pain and consequential pain medication intake.
