1 American Academy of Neurology 2016 Annual Meeting April 15 21, 2016 GROUP REGISTRATION POLICIES AND CONTRACT Welcome to the American Academy of Neurology s (AAN s) Group Registration for the 2016 Annual Meeting. The AAN is pleased to offer the services of Convention Management Resources (CMR) once again as its official meeting registration service provider. The 68th AAN Annual Meeting will take place April 15-21, 2016 at the Vancouver Convention Centre in Vancouver, BC, Canada. AAN/CMR Registration In partnership with CMR, the AAN will again be offering a competitive and group-friendly service package to meet the needs of groups attending the 2016 Annual Meeting. Group Registration is defined as 10 or more paid registrations using the same credit card or check for individuals who do not register and pick up badges and tickets individually. IMPORTANT: An administrative fee of $300 USD per attendee will be charged if you require Group Registration only and do not secure housing with AAN Housing. To secure Housing Services, contact the AAN International Housing Office at aan@cmrglobalgroupservices.com. Thursday,, 2016 is the Early Bird registration deadline. All registration fees must be paid in full by this date in order to avail of this lower fee. March 30, 2016 is the official Group registration deadline. If your group desires to secure Group Registration Services, an authorized group representative should review and complete this document for submission to AAN Group Registration. A signature on this document will serve as a binding and contractual agreement between the AAN and the Group represented by the signatory. Fees and Description Meeting Registration (AMREG) What s included in your Annual Meeting Registration Most education courses*, Integrated neuroscience Sessions, Plenary Sessions, Invited Science, Scientific Platform and Poster Sessions, Exhibit Hall, Experiential Learning Areas, Neurology Career center, breakfast and lunch for attendees all week long, and much more! All Annual Meeting Registrants will receive one ticket to the Opening Party and Closing Party at no charge. 6 Payment Instructions 1. AAN Registration accepts American Express, Diners Club, MasterCard, Visa and checks. Checks must be drawn on US Funds, from a US Bank, include the ABA encryption code at the bottom of the check, and be payable to American Academy of Neurology Institute. Wire transfers are not accepted by the AAN. 2. No administrative fee shall be assessed when housing has been secured with AAN/CMR Global Group Services along with Group Meeting Registration. A fee of $300 USD per attendee (payable by credit card or check only) will be charged if your group requires only Group Registration and does not secure housing with AAN/CMR Global Group Housing. The $300 USD fee will not be refunded should you decide to dissolve your group. 3. Full Meeting Registration payment must be received by March 30, Full payment must be received before any meeting materials will be distributed. 5. Per the American Medical Association (AMA) Gifts to Physicians from Industry Opinion 8.061, the AAN is unable to accept payment from any pharmaceutical company for meeting registration fees for any physician not employed by that pharmaceutical company. Required Attendee Information 1. You will register your group by completing an attached pre-formatted spreadsheet. Once it has been fully completed, return it via through the AAN Support Message Center at 2. Provide individual contact information for each registrant. This includes the complete mailing address, telephone and fax numbers, and individual address. This information is used to track continuing education credits and is required by the AAN without exception. 3. Spreadsheets with missing or incorrect information (such as the repeated use of an address, phone, fax or the use of the tour company's information) will be considered incomplete and therefore subject to the next rate levels if not fully confirmed and paid by March 30, Group Registration Changes and/or Cancellations 1. AAN Registration will review education program lists and contact the group representative regarding any discrepancies. AAN Registration will not be responsible for typographical errors or other errors related to calculation costs. The AAN reserves the right to final authority on questions pertaining to membership and other fee related issues. 2. All changes or cancellations must be sent in writing to AAN Group Registration via fax at (415) or through the AAN Support Message Center at Requests sent to any other location by any other means will not be considered. 3. AAN Registration must receive all group registration cancellations by, 2016 in order to receive a refund, less a $50 USD administrative fee. Cancellations after, 2016 through April 7, 2016 will receive a refund less $100 USD administrative fee. No refunds will be provided after April 7, AAN Group Registration Contract Page 6 of 9

7 4. No deposit/payment monies will be moved between AAN/CMR Global Group Services (The Housing Office) and the Group Registration Office. Contracts and monies must be submitted separately for housing and for meeting registration. All applicable refunds will also be issued separately. 5. AAN Registration must receive all group registration cancellations by, 2016 in order to receive a refund, less a $50 administrative fee. Cancellations after, 2016 through April 7, 2016 will receive a refund less $100 administrative fee. After April 7, 2016 group registrants unable to attend will have their registrations designated as cancellations. Name substitutions will only be allowed until April 7th. After April 7th name substitutions will not be allowed and no exceptions will be made for any reason. In the event of cancellation, the group representative must cancel all meeting registration, housing, and travel reservations separately. The AAN will not cross-check lists. If your group paid the $300 USD outside of the block fee per attendee and you decide to dissolve your group, those monies will not be refunded. 6. The AAN is not responsible for airfare, hotel, or any other costs incurred by participants in the event of program or registration cancellation. Group Registration Materials 1. Upon group registration confirmation, the group representative is required to advise each participant within the group, in writing, that the group representative will be picking up their registration materials. This letter will be provided to the group representative by AAN Registration and will specify the location, date, and time of the distribution of these materials. A copy of the letter sent to group members must be received by AAN Housing and Registration by March 30, IMPORTANT: Groups that fail to send this letter to the group members or fail to provide a copy of the letter to the Group Registration Office will have their onsite appointments cancelled. Attendees from such groups will be asked to pick up their registration materials individually. 2. If delegates from your group request to pick up their registration materials prior to the scheduled group appointment, AAN staff will show them a copy of your letter informing them of the date and time of the materials distribution and a copy of the receipt signed by the group representative at the time the materials were picked up from AAN staff. The AAN will not reprint badges. 3. Upon receiving the group s registration materials, you must sign confirming receipt of the materials. You are responsible for the transportation and distribution of the materials. AAN will not have staff available to assist in transporting materials.
