CBI Home Health is a multidisciplinary team of almost clinical and support professionals that includes physiotherapists, occupational therapists, massage therapists, kinesiologists, chiropractors, occupational physicians, medical specialists, family physicians, nurses, disability management professionals, psychologists, exercise physiologists, dietitians and much more. THE OPPORTUNITY CLINICAL PROGRAM MANAGER Reporting directly to a CBI Home Health Director of Operations, the Clinical Program Manager is accountable for the management of the Clinical department. Responsibilities include planning and operational activities related to the education and supervision of regulated and non-regulated staff. The Clinical Program Manager provides effective leadership, clinical advice and timely problem solving, monitors contractual performance through analysis, tracking and trending and then implements strategies for performance improvement. Key Result Areas The Clinical Program Manager abides by CBI Home Health s vision, mission, objectives, standards, policies, procedures and protocols and where appropriate, the policies and procedures of the AHS and other customers of CBI Home Health. Specifically, the Clinical Program Manager will be responsible as follows: Ensuring the privacy of clients/caregivers/families, the health care team and fellow colleagues. Reporting any client safety concerns or evidence of client abuse or child abuse as required by law. Reporting immediately any safety concerns as per CBI Home Health s policies and procedures and the Occupational Health and Safety Act. Participating in all mandatory Health and Safety activities. Ensuring proper use of routine practices and use of personal protective equipment when required. Working safely at all times according to CBI Home Health standards. Identifying own learning needs and seeks out resources to meet those needs. Maintaining professional growth and development through ongoing education. CBI HOME HEALTH Clinical Manager 1
3 Performing other tasks/responsibilities as assigned, other than those stated. Key Roles & Responsibilities Department Planning and Procedures Participates in the identification and development of the Clinical Program goals, policies and procedures consistent with current professional and legal standards of practice; Identifies, implements and sustains best-in-class Clinical Program practices and procedures; Plans and coordinates Clinical Program professional activities to achieve department goals and priorities including financial and performance standards; Develops, implements and evaluates Home Care Program training and education activities; Provides professional expertise related to clinical treatment and departmental planning activities; Develops and evaluates the Clinical Program position profiles; Participates in RFP and accreditation activities. Communication Facilitates and maintains effective communications strategies to ensure quality client care according to service excellence guidelines; Facilitates open lines of communication both internally and externally between teams and partnerships; Provides input to the development, utilization and evaluation of the QCare Scheduling Program as it relates to the Clinical Program. Risk Management Ensures effective management of client and customer issues and concerns; Assists in the planning, evaluation and improvement of the Clinical Program s risk management strategies through managerial input to CBI Home Health Risk Management Plan; Ensures Risk Management Program implementation within the Clinical Program; Equipment Purchase/Service: Maintains appropriate equipment and PP&E inventories and materials; Equipment maintenance management. Records Information Management Develops effective records information management protocols for the Home Care Program; Ensures compliance and ongoing evaluation of Records Information Management through routine client chart audits, both off and on site. 4 Quality Management Responsible for quality improvement activities in a leadership role, including accreditation; Collects and analyzes data to identify trends within department and initiates and executes on action plans to improve quality. Including but not limited to results of chart audits, incident/event reporting and staff competency; Monitors CBI Home Health s performance indicators and initiates and executes on action plans to improve performance as required within department related to the Clinical Program to ensure compliance with AHS and other CBI Home Health contracts; Supervises/coaches/disciplines Clinical Program personnel. HR responsibilities include recruitment & hiring, orientation, training & education and staff retention. Service Delivery Operations Market knowledge participates in AHS and other community care activities utilizing information for department planning, development and evaluation Intake/Service Requests monitors and analyzes the intake process for essential services and refusals and in collaboration with the Program Manager of Home Care implements processes to improve acceptance rates Service/Care Delivery Assessment oversees all aspects of the client assessment process, as applicable Service/Care Delivery Monitoring monitors and analyzes the achievement of client care goals and outcomes Service/Care Delivery Coordination oversees all aspects of the coordination process as it relates to Clinical Program personnel and ensures adequate coverage during and after regular business hours for both direct care and supervision requirements. Service/Care Delivery Implementation ensures that services are delivered according to best practices resulting from established procedures and protocols THE PERSON The ideal candidate will possess the following qualifications and experience: Graduate from an approved nursing or equivalent degree program Current registration in good standing with the College and Association of Nurses of Alberta (CARNA) or equivalent professional body Minimum 5 years experience in community health care Minimum 5 years in health care management, supervision or leadership role Valid Alberta driver s license, reliable vehicle CBI HOME HEALTH Clinical Manager 3
