1 Project H.E.A.L.T.H Certified Nursing Assistant Program for Youth PROGRAM INFORMATION The Certified Nursing Assistant Program is an accelerated curriculum that prepares students to pass the Massachusetts Certified Nursing Assistant exam and begin a career in the healthcare industry. This course is specifically designed to work with a cohort of high-school aged youth. It is made possible through a joint collaboration of the Berkshire County Regional Employment Board s Project HEALTH and Sweet Brook Transitional Care Center. The course, valued at over $700 per participant, is offered at no charge, thanks to the generosity of Sweet Brook and funding from the Workforce Competitiveness Trust Fund. Twelve (12) slots are available through a competitive selection process. Eligibility: Applications will be accepted from individuals age living in Northern Berkshire County, including students currently attending Hoosac Valley, Drury, McCann, Mount Greylock, and BArT, as well as out-of-school youth. Application & Selection Process: To apply for this program, a student must submit the following: Application for Admission Essay Question Responses Recommendation from non-relative adult (such as Teacher, Guidance Counselor, Mentor, Employer, etc.) High School Transcript with Attendance Record Deadline: All applications must be postmarked by May 29, Applications will be reviewed and candidates for interviews will be chosen. Not all applicants will be invited to sit for an interview. Interviews will take place on June 9, 2009 from 2:30-5:00pm at Sweet Brook. Interviews will be conducted, and a Basic Reading/Math Test will be administered. Not all interviewees will be selected to participate in the program, as there are only 12 slots. Twelve students will be notified of initial acceptance into the program by June 26, Program Requirements: A TB test, physical examination and CORI check will be required upon initial acceptance to the program since students will have clinical work experiences in a long-term care setting. For health and safety reasons, official acceptance into the program will be based on the results. Program Participation Deposit: There is $100 fee which must be submitted upon official acceptance into the program. It is refundable at the end of the program, provided that the student has completed all coursework and clinical experiences, and has taken the state examination.

2 Course Schedule: This course will run for two (2) consecutive weeks, July 13-24, 2009, Monday through Friday, 8:00 a.m. to 5:00 p.m, for a total of ten (10) days. All classes and clinical are held at the Sweet Brook Transitional Care Center in Williamstown. During the clinical portion, students will have the opportunity to work under the direction of a Registered Nurse in a long-term care facility. Attendance: Students are required to attend every class and clinical session. Failure to do so will result in termination from the program and forfeiture of deposit. Course Completion and Exam: Following completion of the program, the student will be granted a certificate of completion which then allows the student to sit for the state exam. The cost of the exam is covered by the program. Program Required Books and Supplies: Textbooks and necessary supplies for the course will be loaned at no cost to the student. Scrubs: Students are required to wear medical scrubs to all classroom and clinical sessions. One set of scrubs will be provided free of charge to each student. Transportation: Transportation to the site of the classes and clinical experience from a location in North Adams will be provided by Sweet Brook. QUESTIONS?
