In vitro studies have shown that pemetrexed inhibits thymidylate synthase (TS), dihydrofolate reductase (DHFR), and glycinamide ribonucleotide formyltransferase (GARFT), all folate-dependent enzymes involved in the de novo biosynthesis of thymidine and purine nucleotides. Pemetrexed is transported into cells by both the reduced folate carrier and membrane folate binding protein transport systems. Once in the cell, pemetrexed is converted to polyglutamate forms by the enzyme folylpolyglutamate synthetase. The polyglutamate forms are retained in cells and are inhibitors of TS and GARFT. Polyglutamation is a timeand concentration-dependent process that occurs in tumor cells and, to a lesser extent, in normal tissues. Polyglutamated metabolites have an increased intracellular half-life resulting in prolonged drug action in malignant cells. Preclinical studies have shown that pemetrexed inhibits the in vitro growth of mesothelioma cell lines (MSTO-211H, NCI-H2052). Studies with the MSTO-211H mesothelioma cell line showed synergistic effects when pemetrexed was combined concurrently with cisplatin. Criteria: CWQI HCS-0097 I. Alimta is medically necessary for patients with One or more of the following: a. Bladder Cancer with ALL of the following i. For a second line of treatment of metastatic disease b. Bladder Cancer Primary Carcinoma of the Urethra with ALL of the following: i. For second line treatment of metastatic disease; and c. Bladder Cancer upper genitourinary (GU) tumors with ALL of the following i. For second line treatment of metastatic disease d. Bladder Cancer Urothelial carcinoma of the prostate with ALL of the following i. For second line treatment of metastatic disease
2 e. Primary central nervous system lymphoma and ALL of the following: i. For progressive or recurrent disease; and. f. Malignant pleural mesothelioma with ONE or more of the following i. Must be used in combination with cisplatin or carboplatin ii. May be used as a single agent g. Nonsquamous non-small cell lung cancer with ONE or more of the following i. Must be used in combination with cisplatin or carboplatin ii. May be used as single agent (if not already given) as subsequent therapy for metastatic disease following progression on a cytotoxic regimen in patients with performance status 0-2 May be used for continuation maintenance therapy and ALL of the following: 1. Patient has recurrent or metastatic disease 2. Patient s disease has not progressed (achieved tumor response or stable disease) 3. Patient has a performance status of 0-2 and ONE or more of the following: a. Must be used in combination with bevacizumab if bevacizumab previously used with a first-line pemetrexed/platinum chemotherapy regimen b. Must be used as a single agent if given previously with chemotherapy h. Thymic malignancies with ALL of the following i. For second or third line treatment of locally advanced unresectable disease i. Ovarian Cancer with ALL of the following i. For persistent or recurrent disease II. Renewal of Alimta with ALL of the following a. Patient continues to meet criteria for specific disease; and b. Tumor response; and c. Absence of unacceptable toxicity Dosage/Administration: Indication All indications Dose 500mg/m 2 every 21 days
