3 Health Savings Accounts Frequently Asked Questions (FAQ) TPA Advantage Administrators A health savings account can be a powerful financial tool to cover medical expenses and save for the future. An HSA gives you a triple tax break: Your contributions are sheltered from income taxes, the money grows taxdeferred, and the funds can be withdrawn tax-free for medical expenses. It s like a supercharged flexible spending account that never expires, and it can even serve as an extra retirement-savings fund. Below are answers to frequently asked questions about how HSAs work and how to make the most of them. How do I qualify for an HSA? You need a high-deductible health insurance policy, whether it s through an employer or on your own. CBE s HDHP option is the only plan that allows participation in an HSA as a pre-tax, payroll deduction option. How much can I contribute? You can make pretax contributions each payroll. You choose the amount you want payroll deducted. In 2015 you can contribute up to $3,350 a year if you have individual coverage, or up to $6,650 if you have family coverage. People age 55 and older can save an extra $1,000 per year. (Contribution limits are set each year by the IRS and usually increase due to cost of living increases) Can I change my payroll deduction HSA contribution amount? CBE s HSA plan allows you to change your payroll deduction amount at the beginning of a calendar quarter. (January 1, April 1, July 1, and October 1) Contact Human Resources to make the change. How can I use the money? You may spend the HSA money tax-free on out-of-pocket medical expenses, such as your deductible, co-payments for medical care and prescription drugs, or bills not covered by insurance, such as vision and dental care. CBE HSA plan provides a debit card and an online reimbursement request option through Advantage Administrators on-line website. Unlike a flexible spending account, you don t have to use the money by the end of the year it can grow taxdeferred in your account for later use. There s no deadline for making a withdrawal: You can reimburse yourself in future years for medical costs you incur now, as long as you have records of past bills. You can use HSA funds to pay Part D or Medicare Advantage (but not medigap) premiums, or for a portion of your longterm-care insurance premiums. If you use HSA money for nonmedical expenses, you ll have to pay taxes on it (plus a 20% penalty before age 65). A complete list of eligible expenses can be found at click on Health Savings Accounts. Can I use the HSA to pay for family member s medical expenses? You may use HSA funds to pay for medical expenses incurred by dependent children or a spouse even if you have an Employee only HDHP plan. However the IRS does not allow you to use HSA funds to pay for adult children s medical bills if they are over age 24. How do I access my HSA money? You will be issued a pre-paid benefits debit-card or you can request a distribution directly from Advantage Administrators. An annual $15 fee is charged for the debit card. No claim form or proof of expense is needed for reimbursement. Keep proof of medical expenses in case of an IRS audit. You cannot pre-spend, but you can reimburse yourself once the money is deposited. For example if you need a prescription for $50, but you only have $25 in the HSA account your HSA debit card will not work at the time of purchase, however you can use your regular checking account to pay for the prescription and then request a reimbursement after your HSA balance reaches $50. How do I invest the HSA money? HSA accounts through CBE are administered by Advantage Administrators with your money deposited into Healthcare Bank. If you maintain a minimum balance of $2,000 your additional funds may be invested in mutual funds yielding tax-free earnings. If I set up an HSA through CBE, what happens if I switch jobs? You can keep the money in an HSA account even after you leave CBE, similar to a 401(k). But you will get stuck with a 20% penalty -- plus an income-tax bill -- if you use any of the money for nonmedical expenses before age 65.
4 Flexible Spending Accounts (FSA) TPA Advantage Administrators Enrollment required January 1 for FSA and HSA each calendar year. Flexible Spending and Health Savings Account Plans are effective January 1 through December 31, Employees can add Health Savings Account at Open Enrollment time if switching to the HDHP Plan. Caution: If you already have a Medical FSA and open a HSA in the same year your Medical FSA can only be used for dental and vision expenses and will change to a Limited Purpose FSA. Limited Purpose FSA allowed in tandem with a health savings account (HSA) Reimbursement of qualifying dental or vision expenses only Use HSA for medical expenses Must incur expense within calendar year Must submit a claim form with proof of expense for reimbursement Reimbursement for FSA Submitting a Claim: Wellness Plan Access to accounts using a convenient Consumer Portal available 24/7/365 File claims easily online or via fax to Advantage Administrators Throughout the year there will be fun-filled wellness activities that will promote camaraderie, healthy habits, and stress relief. Schedule of Wellness Events: February /30/2015; Healthy Choice Wellness Incentive Program Full-time employees employed February 1, 2015 through November 30, 2015 can earn up to $150 by acquiring 100 Wellness Points by completing the Healthy Choice Verification Form. Documentation is required for approval. The Healthy Choice Verification Form can be found on CBE on-line Human Resources\Benefits\Wellness. Submit completed form and documentation to Human Resources by November 30, April 2015 Poker Walk Employees will walk outside on a pre-designated route for up to 15 minutes. Each time they reach a Wellness representative on their walk, they will receive a playing card until they collect five (5) of them. The employee with the best Poker hand at the end of the Poker Walk will receive a wellness prize. There will be a winner of a $25 gift certificate at each location. June 1 through August 1, 2015 Walk, Walk, Talk, Talk! Grab a buddy or two and walk 15 minutes outside over lunch break three (3) times a week for six (6) weeks during June and July. Log your minutes and dates walked, have your buddy sign your log and turn your journals into your HR office. Winners from each location will be drawn from those that turn in journals. September 2015 Be Fruitfull, Veggiefull and Waterfull! Learn about healthy habits, record your fruit, vegetable, and water consumption for the month of October and win! Winners from each location will be drawn from those that turn in journals. November 20, 2015 January 2, 2016 Maintain Don t Gain Conservative eating habits over the Holidays will get you a $10 gift card. Weigh in prior to Thanksgiving and do not gain weight through New Year s Day at the final weigh in.

Benefit allowances provide no remaining balance for future use within same benefit period. Lost or broken lenses, frames, glasses, or contact lenses will not be replaced except in the next benefit period. Underwritten by Fidelity Security Life Insurance Company. Getting an eye exam is more than just testing your vision. Eye exams can assist in the early detection of vision conditions and health conditions such as: Glaucoma Diabetes Cataracts High Blood Pressure Astigmatism That s why it s important to get an eye exam on a regular basis. Children need eye exams, too! Did you know the American Optometric Association recommends that children receive an eye exam as early as six months of age? Our nationwide provider network will be happy to assist you in servicing your vision care needs. **LASIK and PRK Benefit EyeMed is pleased to provide all EyeMed Vision Care members with a laser vision correction benefit. EyeMed and LCA-Vision have arranged to provide his benefit to all EyeMed Vision Care members through the U.S. Laser Network. Members are entitled to a 15% discount on the usual and customary fees for LASIK and PRK procedures, or a 5% discount on any promotional pricing, whichever is the greater benefit. One easy phone call to LASER6 begins the process of using your benefit. No claim forms are needed, making it a hassle free process for members. Contact Lens By Mail Program Effective January 1, 2004, you may order replacement contact lenses for great prices via the internet, and have the contacts mailed directly to your home. The service is for replacement contact lenses only, and your core benefit allowance or discount will not apply to the service. Your initial pair of contact lenses must still be purchased from your eye care provider to ensure proper fit and follow-up care. Simply visit for details, and a link to the order site.
