CA021694L, of Reading, Berks County, was assessed a civil penalty in the amount of $6,000 for practicing with a lapsed license. (10/24/14) PA Architects Licensure Board No disciplinary actions taken. PA Board of Auctioneer Examiners Montgomery County Ah Ram Lee, of Norristown, Montgomery County, was provisionally denied licensure based on his misdemeanor conviction. Morgart, license number AA019137, of Coudersport, Potter County, was revoked and assessed a $1,000 civil penalty for being convicted of theft by unlawful taking or disposition. (11/12/14) PA Board of Barber Examiners Bucks County Boss Barber Shop, license no. BO433210, of Warrington, Bucks County was suspended for failing to pay a previously imposed civil penalty. (11/19/14) Butler County Donald R. Ferraro, license no. BM010969L, of Butler, Butler County, was suspended for failing to pay a previously imposed civil penalty. (10/21/14) Cumberland County Bryant K. Mathes, license nos. BL050938L, BM and BT000478, of Enola, Cumberland County, was suspended pursuant to the Order of the Court of Common Pleas of Dauphin County dated November 17, 2014, which the court issued under section 4355 of the Domestic Relations Code. BM011819L, of Harrisburg, Dauphin County, was ordered to pay a $750 civil penalty and cease and desist from charging a student tuition in a licensed barbershop because he charged a student for tuition in his licensed barbershop. 4 Lancaster County Jorge Valentini t/d/b/a Who s Next Barber Shop, license no. BO432497, of Lancaster, Lancaster County, was ordered to pay a $1,000 civil penalty and to cease and desist from employing unlicensed individuals and aiding and abetting unlicensed practice of the barbering profession, because he continues to employ unlicensed persons and aided and abetted the unlicensed practice of the profession by permitting an unlicensed person to practice barbering. BO433250, of Bethlehem, Northampton County, was ordered to pay a civil penalty of $450 because it operated as an establishment without the supervision of designated manager barber or other designated licensee in charge; and moved said barbershop before filing an application with the board for the inspection, approval and registration of such shop. DF003952, of Tunkhannock, Wyoming County, voluntarily surrendered her expanded functional dental assistant certificate along with any other licenses, registrations, certificates, approvals, authorizations or permits issued to her by the board because she failed to practice dentistry or as a dental hygienist or as an expanded function dental assistant with reasonable skill and safety to patients by reason of illness, drunkenness, excessive use of controlled substances, chemicals or any other type of material, or as the result of any mental or physical condition. (12/12/14) Out of State Ronald Kermit Creecy, license no. DS028378L, of Moorestown, New Jersey, was placed on probation until such time the $2,500 civil penalty is paid to the Commonwealth of Pennsylvania. DS031119L and DN031119A, of St. Louis, Missouri, was revoked based on disciplinary action being taken by the proper licensing authority of another state. (10/29/14) Clyde Knox, license no. DS022033L, last known at Sicklerville, New Jersey, was revoked because of failing to provide for the disposition of patient records. (10/29/14) Zuhair Sayany, license no. DS027365L, of Cherry Hill, New Jersey, was ordered to pay a civil penalty of $7,000 because he practiced dentistry or as a dental hygienist or as an expanded function dental assistant or held himself out as a practitioner of or entitled or authorized to practice dentistry or as a dental hygienist or as an expanded function dental assistant, or assumed any title of dentist, dental surgeon, dental hygienist, expanded function dental assistant or other letters or titles in connection with his or her name which in any way represented him or her as being engaged in the practice of dentistry or as a dental hygienist or as an expanded function dental assistant, or authorized so to do, without being duly licensed or certified, and authorized to engage in such practice. (12/12/14) PA Board of Professional Engineers, Land Surveyors and Geologists Lackawanna County E. Kenneth Acker, license and registration nos. PE024042E and SU000401A, of Moscow, Lackawanna County, had his certificates of licensure to practice engineering and land surveying suspended, such suspension immediately stayed in favor of probation, subject to certain terms and conditions, and was ordered to pay a civil penalty in the amount of $1,000, enroll in and successfully complete a board approved ethics course for professional engineers/professional land surveyors, and to provide 15 hours of community service because he failed in his fiduciary duty as a partner in an engineering firm. (11/12/14) Schuylkill County Merlyn J. Jenkins, license number SU007784E, of Pottsville, Schuylkill County, was assessed a civil penalty in the amount of $3,000, and indefinitely suspended until he completes 60 hours Page 8 of 22

9 of continuing education, because his license was disciplined by the proper licensing authority of other states. (11/14/14) Out of State Southeastern Reprographics, Inc., now known as Davey Resource Group, Alpharetta, Georgia, was ordered to pay a civil penalty of $2,000, for practicing and offering to practice land surveying while not being properly licensed, and for representing itself without employing a licensed directing head or responsible charge credentialed as professional land surveyor. (11/14/14) PA Board of Funeral Directors Lehigh County John M. Sunday, license and registration nos. FD011616L and FS015298, of Allentown, Lehigh County, had a public reprimand placed on his permanent disciplinary record with the board and was ordered to pay a civil penalty of $2,000 and costs of investigation of $ because he failed to fulfill the professional responsibilities of a funeral director. (12/03/14) PA Board of Landscape Architects No disciplinary actions taken. PA Board of Massage Therapy Allegheny County Weiguang Liu, unlicensed, of Pittsburgh, Allegheny County, was ordered to immediately cease and desist from utilizing in connection with its business name the word "massage" in advertisements describing the services provided, immediately remove from the business name as well as any and all advertisements, signage, etc., the words massage or any derivative thereof, and submit proof of the required changes within 60 days of approval of the order and was ordered to pay a civil penalty of $500 and costs of investigation of $ because Liu, as the responsible owner/officer of Chinese Back Rub, utilized the word "massage" in advertisements describing the services provided which implied directly or indirectly that massage therapy services were being provided even though the services were not being performed by licensees. (12/01/14) Centre County Feng Xian He, unlicensed, of State College, Centre County, was ordered to cease and desist hiring or employing unlicensed individuals to perform or offering to perform activity requiring a license to practice massage therapy services and pay a civil penalty of $1,000 and costs of investigation of $ because she, as the responsible officer of Joma Chinese Massage, employed an unlicensed individual to provide massage therapy/bodywork services to customers even though the individual was not licensed by the board. (12/01/14) Chester County Lin Jun Jiang, license no. MSG007511, of Downingtown, Chester County, was ordered to pay a civil penalty of $500 and costs of investigation of $1,243.37, was placed on probation for one year, ordered to complete 12 hours of continuing education on the topic of professional ethics, and ordered to cease and desist from hiring or employing unlicensed individuals because she, as the responsible officer of Green Tea Spa, knowingly aided, assisted, and hired individuals to perform or offer to perform massage therapy Page 9 of 22

10 services when those individuals were not licensed by the board and she practiced massage therapy between August 5, 2012 and February 14, 2013 without having at the time of so doing, a valid, unexpired, unrevoked and unsuspended license. (12/01/14) Dauphin County Jizhen Fan, license no. MSG000325, of Hershey, Dauphin County, was ordered to cease and desist hiring or employing unlicensed individuals and ordered to pay a civil penalty of $1,000 and costs of investigation of $ because she knowingly aided, assisted, or hired individuals to perform or offer to perform massage therapy services at Oriental Massage without being licensed by the board, and she, as the responsible officer of Oriental Massage, employed unlicensed individuals to perform massage therapy services. (12/01/14) Delaware County Xueli Zhang, license no. MSG007866, of Wayne, Delaware County, was ordered to cease and desist from hiring or employing unlicensed individuals to perform massage therapy services requiring a license, ordered to pay a civil penalty of $1,000 and costs of investigation of $80.58, and complete four hours of remedial education in professional ethics, because he as the responsible officer of the Chinese Tai Chi Bodywork Center, employed unlicensed individuals to provide massage therapy/bodywork services even though none of the aforementioned individuals were licensed by the board or were members of any national or local bodywork organization. (12/01/14) Montgomery County Yong Qiang Li, unlicensed, of North Wales, Montgomery County, was ordered to immediately cease and desist from hiring or employing unlicensed individuals to perform massage therapy and was ordered to pay a civil penalty of $1,000 and costs of investigation of $ because Li, as the responsible officer of Health Power, employed individuals to provide massage therapy/bodywork services to customers even though none of the individuals were licensed by the board. (12/01/14) PA Board of Medicine Chester County Michelle S. Montague, license no. MA051498, of West Grove, Chester County, was ordered to pay a civil penalty of $2,000 because she had disciplinary action taken by the proper licensing authority of another state against her license to practice the profession. (12/02/14) Erie County Deborah Elaine Bishop, license no. MD056630L, of Girard, Erie County, was indefinitely suspended for no less than three years, retroactive to April 22, 2014, based on her violation of a previous Board order. (10/30/14) Lackawanna County Tracy Lynn Liberatore, license no. MA051327, of Scranton, Lackawanna County, was indefinitely suspended based on findings that she violated a previous Board order. (10/30/14) Lehigh County Maryann Theresa Weisman, license no. MD043771E, of Whitehall, Lehigh County, was indefinitely and actively suspended until such time as she has submitted to a board-approved psychiatrist indicating that she can practice medicine with reasonable skill and safety and can demonstrate to the satisfaction of the Page 10 of 22

11 board that she can safely and competently practice medicine, because she is unable to practice the profession with reasonable skill and safety to patients by reason of illness. (12/06/14) Montgomery County Donna Glover, license no. MD021901E, of Gladwyne, Montgomery County, was indefinitely suspended based on findings that she violated a previous Board order. (10/30/14) Tami L. Berry, license no. MD440815, of Penn Valley, Montgomery County, was suspended based on her felony under the Drug Act. (09/11/14) Philadelphia County Gregory M. Braccia, license no. MD048926L, of Philadelphia, Philadelphia County, was suspended for no less than one year, immediately stayed in favor of probation for being unable to practice with reasonable skill and safety to patients. (11/03/14) Ronald Jay Haberman, license no. MD042475E, of Philadelphia, Philadelphia County, permanently voluntary surrendered his license to practice in the Commonwealth of Pennsylvania, along with any other authorizations to practice the profession, because he had a license or other authorization to practice the profession suspended, revoked, refused or otherwise disciplined by the proper licensing authority of another state, territory, possession or country, or a branch of the federal government. (12/02/14) Out of State Kenneth Mwatha, license no. MD449345, of Baltimore, Maryland, had his Pennsylvania license placed on probation indefinitely based on findings he was disciplined by the proper licensing authority of Maryland. (07/15/14) Alison Nicole Kinlaw, of Pennington, New Jersey, was denied Pennsylvania licensure based on her multiple felony convictions and disciplinary action taken by another state. (11/03/14) Bill G. Sekulovski, license no. MD023851E, of Thompson Station, Tennessee, and Ravenna, Ohio had his Pennsylvania license suspended for not less than 102 months, with the suspension stayed in favor of probation for not less than 102 months, based on discipline taken against his license by the proper licensing authority of Tennessee. (11/03/14) Dharam Paul Garg, license no. MD033125E, of Depew, New York, permanently and voluntarily surrendered his license, along with any other licenses, registrations, certificates, approvals, authorizations or permits issued to him by the board, because he had disciplinary action taken against his license to practice the profession by a proper licensing authority of another state. (12/2/14) Farid Tanios Fata, license no. MD072100L, of Grosse Pointe, Michigan, agreed to the permanent voluntary surrender of his license and any other authorizations to practice the profession because he had a license to practice the profession suspended, revoked, refused or otherwise disciplined by another state or territory and failed to report said disciplinary action within 60 days after its occurrence. 12 Anita K. Connor (Taylor), license no. MA001820L, of Nashville, Tennessee, had her Pennsylvania license automatically suspended based on her felony under the Drug Act. PA Board of Nursing Allegheny County Theresa Ann Pressler, license no. RN599443, of Pittsburgh, Allegheny County, voluntarily surrendered her license to practice as a registered nurse in the Commonwealth of Pennsylvania because she had disciplinary action taken against her license by the State of Florida Board of Nursing.
