1 Equipment and Services: Mississippi Bend Area Education Agency Cell Phone Service Nationwide plan with roaming included Request for Bid 3-Year contract from July 1, 2014-June 30, 2017 Mississippi Bend Area Education Agency is seeking bids for a 3-year Nationwide plan with roaming included cell phone service contract. We have an estimated 102 cell phones with an average of 24,000 combined minutes per month. 12 lines must have an unlimited plan, which includes talk in/out, voice mail, texting, web and data (not part of the 24,000 minutes). 30 lines with unlimited texting and no data. 60 lines just phone. Quantities may change. Please provide proposals that include Voice Mail, Text Messaging, 3-way calling, Call Waiting, Wireless Internet Access, and Caller ID and features/options. *All areas within the geographical district must have 100% connectivity minimum three bars. *All plans proposed must include detailed billing, and online account access. In Addition, Please provide the following information: 1. Length of time business has provided this type of service 2. 3 reference sites using your service 3 years or more 3. The Service Level Agreement (SLA) for your proposal 4. Indicate any options available 5. Trouble reporting and escalation procedures 6. Hours of operation for help or trouble reporting 7. Describe maintenance and trouble notification (to us) procedures 8. Costs: Detail all service costs with cost of supported cell phones separate 9. Please show applicable discounts separately, if applicable 10. Your E-Rate Service Provider Identification Number (SPIN) on your proposal 11. Corporate customer E-Rate contact information 12. An implementation timeline proposal with all service fully implemented starting July 1, 2014.

2 Special Conditions: 1. Nation-wide service (roaming included) and coverage to include calling, data and text are mandatory requirements for this bid 2. Ability to deliver quality service in both voice and data 3. All training costs must be provided. If training is provided at no additional cost, the hours required per user must be quantified 4. All equipment costs must be included and identified separately 5. Prices to remain firm through SLD approval, execution, and duration of the proposed contract. In the event of a price decrease for service or from the manufacture, said decrease shall be passed on to the Mississippi Bend AEA and documented with new price sheet sent to: jcoon@aea9.k12.ia.us 6. The RFB must cover the total cost of successfully converting all users to the new service 7. The RFB must include a detailed project plan for a seamless transition and uninterrupted service for all users 8. All existing telephone numbers must remain the same 9. The Board of Mississippi Bend AEA reserves the right to reject any and all bids/proposals, or any or all items of any bid/proposal 10. Any add on plans will need to end on June 30, In case of un-used plans they can be deleted after 1 year Notes: We currently have 12 unlimited plans that included talk in/out, voic , text and web (data) and 30 have talk and text, and 60 talk only. 3 The timeline for this proposal is as follows: All bids must be delivered to the following address: Mississippi Bend Area Education Agency, Administrative Services, attention Jennifer Klindt, st Street, Bettendorf, Iowa before 10:00 a.m. (CDT) on Thursday, January 16, 2014 and the envelope must be marked "Cell Phone Bid". Faxes will not be accepted. Vendors should insure that their bids proposals arrive at the correct location on or before the stated deadline. Late bid proposals or proposals delivered to the incorrect location will not be considered and will automatically be disqualified from further consideration. Service start date will be July 1, Bids will be opened at 10:00 a.m. on Thursday, January 16, All below bid proposal information should be contained in the written material submitted. Device features should include call waiting, voice message center and call display. The unlimited plan devises must have access to on-line applications. All device and services should provide for recurring audible alarm for missed calls and voice mail. The proposed cellular technology must have the ability to have features deactivated (i.e. web, texting, etc.). If so, state which features can be deactivated.
