1 600 South Bois d Arc * Forney, Texas * * Fax: * FISD Field Study Guidelines and Procedures Note: FISD Field Study Request Forms must be submitted to the principal at least six weeks (30 school days) prior to the departure date. Principals must submit the FISD Field Study Request Form to the Superintendent, or designee at least five weeks (25 school days) prior to the departure date. Instructions The administrator should carefully consider each request for a field study and approve only those studies with instructional value outweighing any loss of regular instructional time. Field must be directly related to subject matter being studied and be selected from the approved FISD list of acceptable Field per grade level located at: All school policies shall apply at all times. Generally, field studies are limited to one out of district field study per class/grade level. Spring weekday field studies will not be approved until after state mandated testing is complete. Teachers should ensure proper preparation of the field study, including appropriate pre-visit and follow-up activities, following district guidelines and timelines, as well as proper lunch and medical procedures. Teachers and principals need to make sure students do not duplicate field studies from year to year. For example, if the first grade class goes to the Science Place in the school year, that group of second grade students could not go to the Science Place in the school year. The Field Study Guide provides various destination opportunities. Students in ISS or DAEP may not participate in the field study. Transportation Bus Requests must be submitted by the campus secretaries. Bus Request Forms should be submitted (online through the intranet) and can be found on the Field Study Guide page or at On the website, click on Forms under the Administration tab and then select Employee Forms. Locate the Transportation Request Form at the bottom of the list. Field studies should not be scheduled to depart until buses have completed their morning routes and must return prior to afternoon route departure. Students must ride the bus to and from the field study destination. In the case of a doctor s appointment, prearrangements must be made with the campus administrator. Chaperones In advance of any field study, any and all chaperones, persons interacting with students, must have completed the Volunteer Applications. They must have been cleared through Human Resources and be on the approved Volunteer List for the campus. Chaperones may not bring siblings on the field study. The ratio of student to chaperone will depend on the age of the student, the destination, and the length of the field study. Principals should make the final determination. Volunteers may not ride on the bus. An administrator must accompany any overnight field study and the cost of the administrator shall be covered by the group sponsoring the trip. Permission Requirements Students will not participate in a field study without the FISD Field Study Permission Form completely filled out and signed by a parent. Any changes, made by the parent, to the Field Study Permission Form nullify the form, and the child will not be allowed to participate in the field study. Parents must be notified of each field study and have the opportunity to deny their student s participation. Safety Safety is always a primary focus. Campuses must have clearly written safety procedures with at least 2 points of contact for cell phones of teachers or chaperones on the field study. Administrator cell phone numbers must be made available to teacher/chaperones. Teachers should always take contact information for each student, emergency medical information, and student medication, if needed. 2 600 South Bois d Arc * Forney, Texas * * Fax: * Planning the Field Study FISD Field Study Checklist Choose a Field Study experience from the FISD Field Study Guide and select a tentative date for the Field Study Check the District Assessment Calendar to make sure the date does not interfere with state and local assessments Discuss financial arrangements with campus administrator in at least six weeks prior to the departure date (30 school days) Tentative location & date: Complete and submit FISD Field Study Request form to principal six weeks (30 school days) prior to the departure date Once approved for the Field Study, confirm reservation with destination choice Complete the Bus Request form online at least two weeks (10 school days) prior to the departure date Complete pre-visit and post-visit activity form and submit to principal Copy FISD Field Study Permission Form and send home with students to be completed by the parent and returned Request for chaperones to attend the Field Study, and make sure each has completed a Criminal History Background Check Check to make sure the chaperones are on the FISD approved Volunteer List; Criminal History Checks must be turned in at least three weeks (15 school days) prior to the departure dates Complete the List of Approved Chaperones to take on the field study An administrator must accompany the group on all overnight field studies. The administrator s expenses shall be paid by the group sponsoring the trip. Collect field study money and permissions slips make sure deposit is paid (if needed) Complete FISD Field Study Lunch Form and provide notification to the cafeteria at least three weeks (15 school days) prior to the departure date Remind the cafeteria staff that the students will be away during the lunch hour one week (5 school days) prior to the departure date Complete FISD Field Study Medication Request Form for the school nurse verify at least one week (5 school days) prior to the departure date Have the students develop a set of standards of conduct for the field study Make chaperone assignments for student groups Inform other teacher about the field study if their schedules will be affected one week (5 school days) prior to the departure date

3 Field Study Preparation & Follow Up--Academic Details Discuss with the class the purpose of the Field Study and how it relates to the current unit of study Prepare activities for students to complete before and after the field study Conduct appropriate pre-visit activities to prepare students for the field study Conduct appropriate post-visit activities to reinforce student learning from the field study Have the class compose and send a thank-you letter to the Field Study host site (optional) Evaluate the field study in terms of educational purpose, strengths, weaknesses, and suggestions for improvement The Day of the Field Study Get the medical form and medicine from the nurse Gather permission slips and take them with you on the field study Take student contact information on the field study Take a roster of students and approved chaperones attending the field study Gather lunches, coolers, and ice for drinks from the cafeteria Introduce your chaperones to the students they will be responsible for on the field study Count your students to make sure you know how many students you are taking and returning from the campus Give your cell phone number to the office staff and administrator Take administrator and campus phone numbers on the field study Have students use the restroom before getting on the bus Provide directions to the destination to the chaperones-remember they are not allowed to ride on the bus, nor are they to bring siblings with them Out of State Field Study Guidelines Sponsor/Director shall receive approval from the principal before soliciting information pertaining to an out of state field study. Request must be submitted to the Superintendent for approval. 1 adult chaperone for every ten participants Chaperones are required to have background criminal history checks. An administrator must accompany the group on the out of state field study. All expenses must be paid for by the group sponsoring the trip. No field study trips before or during state or national assessments. No trips between the day after Spring Break and the end of the year. Preference would be that out of state field studies take place during non-instructional time like Spring Break, Thanksgiving Break, Christmas Break and Summer. No more than 2 school days may be missed. Dates must be approved by the principal & Superintendent. Method of raising money for the trip shall be limited to voluntary contributions by parents and fundraising projects. Parents/Guardians must fill out the Field Study Parent Permission Form Out of state trips must be taken every other year o (13-14) Choir, Drill, Cheer and any other organization o (14-15) Band
