4 Executive Summary This quarterly report includes information on all patients that received NHS dental care in England for the second quarter period to 30 September Subjects covered include: Dental activity, including the number of courses of NHS treatment Patients seen, number of patients seen by an NHS dentist for the 24 month period up to 31 December 2013 This information is used by the Department of Health to inform government policy on NHS dentistry and by local NHS managers in the planning and delivery of local dental services. The report is based on data collected on each course of NHS dental treatment by NHS Dental Services, part of the NHS Business Services Authority. Key Facts A total of 29.9 million patients were seen in the 24 month period ending 31 December 2013, an increase of 1.8 million on the March 2006 baseline. This represents 55.9 per cent of the population compared with the March 2006 baseline of 55.6 per cent. The number of children seen by an NHS dentist is 1.0 per cent above the March 2006 baseline of 7.8 million although the percentage of children seen (69.0 per cent) is below the baseline (70.2 per cent). Figure 3a: Percentage of patients seen in the previous 24 months by region, March 2006 and December 2013 Percentage of patients seen in the previous 24 months, March 2006 and December 2013 There were an estimated 9.9 million CoT in Q2 2013/14, an increase of approximately 29,000 (0.3 per cent) on Q2 2012/13. CoT increased for each treatment band in Q2 of 2013/14, except Other which saw a decrease of approximately 174,000 (78.7 per cent) when compared with the corresponding quarter in 2012/13. This decrease reflects the phasing out of UDA credits for prescription only CoT, which was announced as part of the 2012/13 dental contract uplift 1. 5 arrangements 1, and came into force on 1st November As a consequence, no CoT or UDA are counted for prescription only activity from 1st November 2012 onwards. During Q2 2013/14 paying adults account for the largest percentage of patients receiving treatment in each band, with the exception of band 3 treatments, where non-paying adults received 53.2 per cent of these treatments. The greater provision of more complex treatments (band 3) to non-paying adults, compared with other patient types, may reflect a lower standard of dental health among these patients. This is supported by findings from the Adult Dental Health Survey Further Information This report covers work carried out by high street dentists, who account for the majority of dental activity and work undertaken by dentists under vocational training. The publication does not cover dental services provided privately. The total number of patients seen reached its lowest point in June 2008, and has generally increased gradually since this point (slight decreases in June 2012 and September 2013). In December 2009 the number exceeded the March 2006 baseline, when the current dental contract was introduced. 6 Introduction This quarterly report brings together information on NHS dental activity in England, based on data collected through FP17 forms by NHS Dental Services, part of the NHS Business Services Authority. This information is used by the Department of Health to inform government policy on NHS dentistry and by local NHS managers in the planning and delivery of local dental services. The report includes information on: Dental activity data on the number of Courses of Treatment (CoT) and Units of Dental Activity (UDA) The number of patients seen by an NHS dentist This report covers dental activity carried out by high street dentists, including work undertaken by dentists under vocational training. It does not, however, cover dental treatment carried out in hospital under Hospital Dental Services, nor under services provided privately. For an introduction to the terms and measures used in this report, we recommend reading our Beginner s Guide to Dental Data available at: In addition, full technical information is available in Annex 3 of this report. This report covers activity data under the current dental contract arrangements, including data (provisional and estimated) at specified dates from 2006/07 to the second quarter (30 September) of 2013/14. Numbers of patients seen by NHS dentists are slightly more up-to-date, including figures for the 24 month period to the end of the third quarter (31 December) of 2013/14. Dental activity data are provisional each quarter until final figures are available at the year end. The Health and Social Care Information Centre provides national year-end estimates for each quarter available based on these provisional data. Population based measures use population estimates provided by the Office of National Statistics (ONS) based on their revised 2010 methodology. ONS has published revised population estimates back to pre-2000; this is reflected in the patients seen section of this report. The historic percentage of the population will differ to those previously published. Due to the changes in the NHS structure, historical data (pre 2013/14) at subnational level are no longer included in the report annexes. From this report onwards data in Annex 2 will be presented at National, Regional, Area Team, Clinical Commissioning Group (CCG) and Local Authority (LA) levels. Historical data at sub-national level, under the pre-2013/14 NHS structure can be found in previous iterations of this report, available via the link below: Click for previous versions of NHS Dental Statistics for England While neither CCGs nor LAs are directly accountable for commissioning NHS primary care dental services (Area Teams hold this responsibility), in order to provide a suitable level of geographic granularity and relevance to the health service we have opted to provide these data at CCG level. 7 In addition, Local Authorities have responsibility for commissioning public health services, which includes dental and oral health programmes; so for this reason we considered that it would be relevant to present data at this level also. In order to present data at these levels, we have mapped data to these organisations on the basis of the postcode of the practice which claimed for the activity. This mapping requires use of additional datasets which define which postcodes reside in each organisation. Further details on this mapping are provided in the Geographic Granularity section of Annex 3. The following annexes are available with this report; Annex 1 includes all report tables and charts in Excel format. This annex also contains patients seen tables by adult/child at Region level. Annex 2 shows patients seen figures at Region, Area Team, CCG and Local Authority levels in Excel format. Annex 3 includes full technical information on the measures and definitions used in the report. Annex 4 contains flat csv files of unformatted activity and patients seen data, suitable for further analysis. Annex 4 (Patients Seen) contains patients seen data for each month from April 2013 onwards. The population estimates in this annex allow the calculation of population based patients seen figures. Annex 5 contains the csv file used to map PCTs to the regions in the new NHS structure. We are still able to assign these data to an area team, as this information is included in the raw data provided to us by the BSA, and thus we can also allocate them to a Region. 8 Activity Courses of Treatment Dental activity is measured through the number of CoT delivered. For a full definition of CoT and treatment bands, please refer to Annex 3. Tables 1a and 1b show that there were an estimated 9.9 million CoT in Q2 2013/14, an increase of 29,064 (0.3 per cent) on Q2 2012/13. A further increase for CoT during Q2 follows an overall decrease in CoT over the course of 2012/13 compared with 2011/12, and may indicate a recovery of activity levels. Were it not for the impact of the phasing out of UDAs for prescription only CoT (see below), it is likely that the overall increase in quarter 2 would have been higher. Table 1b shows that in both absolute and relative terms, Other treatments experienced the greatest change compared with the same quarter the previous year; a decrease of 174,036 (78.7 per cent). This decrease can be attributed to the phasing out of UDA for prescription only CoT, with this treatment band substantially reducing throughout 2012/13. The phasing out of UDA credits for prescription only CoT, was announced as part of the 2012/13 dental contract uplift arrangement 4 for prescription only activity from 1 st November 2012 onwards 5. 14 Units of Dental Activity Units of Dental Activity (UDA) are weighted CoT. The measure is used in the funding of NHS dental activity and in setting contracts with NHS dentists. More information on UDA is available in Annex 3. Table 2a shows that an estimated total of 21.9 million UDA were carried out during Q2 of 2013/14, an increase of 170,669 thousand (0.8 per cent) on Q2 2012/13. As UDA are weighted CoT, changes in UDA by each band is similar to those for CoT, with the exception of Other (as there are multiple weightings in this band). The largest absolute change in UDA between Q2 2012/13 and Q2 2013/14 was for Band 1; increasing by 145,905 (2.7 per cent). As weighted CoT, the factors underlying the reduction in Other activity (e.g. phasing out of UDA credits for prescription only CoT), also apply in the same way to the UDA figures. 19 Patients Seen This section shows the number of individual patients who received care or treatment from an NHS dentist at least once in the most recent 24-month period. Note that orthodontic patients are included in the patients seen measure. The National Institute for Health and Care Excellence (NICE) recommends that patients are recalled for check ups at intervals of 3 months to 24 months depending on the individual s oral health status. The patients seen measure is more up-to-date than the activity figures. In this report, we show the number of patients seen up to the end of the third quarter of 2013/14. In the 24 month period ending 31 December 2013: The total number of patients seen remains above the March 2006 baseline figure. In the 24 month period up to 31 December 2013, 1.8 million more patients were seen by an NHS dentist compared with the March 2006 baseline. Table 3a shows that the number of adult patients seen has risen by 1.7 million (8.3 per cent) since the baseline date. The number of children seen by an NHS dentist is 1.0 per cent above the March 2006 baseline of 7.8 million though the percentage of children seen (69.0 per cent) remains below the baseline (70.2 per cent). Table 3b shows that a total of 29.9 million patients were seen, increasing 0.4 per cent on the previous quarter. As in Q2, the percentage of the population seen by an NHS dentist exceeds the March baseline in the North and South regions (note that the distribution of data between regions for March 2006 is estimated).
