Message from the Board President WHAT A YEAR WE VE HAD! It seems like just yesterday that I visited with you to share some of the wonderful things The Guidance Center has been doing within our Flagstaff community and in Northern Arizona. The team of caring professionals who make up The Guidance Center family continue to build upon a solid legacy of service that goes all the way back to 1969 when we first opened our doors. Our goal is to be there for our clients and their families when they are experiencing what may be the worst day of their lives. It is in these times of crisis when the quality care and compassion of our dedicated team shines the brightest and where we can truly make a difference between hope and despair for people who may be your family members, co-workers, neighbors, or friends. JOSH COPLEY Through our active engagement and participation with community partners, we have helped to improve the delivery of a broad range of behavioral health services. One such example is our Crisis Intervention Training (CIT) program that provides law enforcement officers and other first responders with the skills and techniques to effectively handle situations they encounter. The Guidance Center continues its leadership role in the Closing the Gap initiative which brings together a variety of community stakeholders who collaborate on strategies to mitigate the impacts of chronic alcoholism on our region. We have provided Teen Mental Health presentations to parents and teachers in the Williams Unified School District to help them better cope with challenges that face many of our young people today including suicide, depression, anxiety, and substance abuse. This year, The Guidance Center continued to strengthen its relationships with the Native American Tribes located within the northern Arizona region. We continually strive to bring value to the services we provide to our clients who live in remote areas of Arizona including Havasupai and the Hopi and Navajo Reservations. Please join me in celebrating the many other fine accomplishments and programs of The Guidance Center which are highlighted in this Annual Report. 2

Message from the CEO JACK CALLAGHAN, Ph.D. I AM DELIGHTED TO PRESENT the 2015 TGC Annual Report to you, the reader. In order to improve the delivery of mental health and substance abuse services to over 4,000 individuals in 2014, improvements have been made in many areas. There were infrastructure improvements: clinical space has been expanded as we have re-configured our footprint across the campus; a new electronic health record has been implemented to improve our clinical and billing efficiencies; and, our information technology capabilities have been brought up to industry and federal requirement standards, to name a few. Medical staffing has been increased to include new nurse practitioners. Additional clinical staff (mental health as well as substance abuse) have been hired across the Adult Service and Child and Family Service areas. The Psychiatric Acute Care Unit and our Alcohol Stabilization Unit have both had significant improvements made to their bricks and mortar infrastructure flooring, doors, painting, lighting, etc. And, our financial situation has been significantly improved through the refinancing of the original bonds that initially helped to finance two of our major buildings. We have a number of exciting program efforts that have also been recently initiated. We continue to look for ways to increase services that integrate both behavioral and medical services for our consumers. In our Child and Family Services Department we have introduced services in Tuba City at the Greyhills Academy; we have also implemented a new Strengthening Families Program, which helps at-risk families with children ages In our Adult Services Department, we have initiated Assertive Community Teams, which provide comprehensive inter-disciplinary service to adults with serious mental illness. Across all service areas (including substance abuse, child and family, and adult) we have begun Centralized Intake as well as new crisis stabilization services through the use of observation chairs. Looking into the future, we have a number of drivers both internally and externally. Our major funding source Northern Arizona Regional Behavioral Health Authority has successfully partnered with Health Choice Arizona to form Health Choice Integrated Care to begin to oversee integrated services (physical health and behavioral health) for members with serious mental illnesses in northern Arizona. This will provide new and exciting challenges as this unified approach begins on October 1, We continue to emphasize peer and family-based services, with Recovery being a prime focus. We will continue to strive to make TGC an excellent place to work and serve those in need in our community. Open forums for all staff are held regularly to allow real time discussions of timely happenings, and we have increased our community outreach efforts to ensure that the public knows how to access TGC services. And finally, we are constantly reaching out to our partners in the community (e.g., health care system, law enforcement, schools, social service agencies, and the courts) to make sure that TGC is making the best use of our resources as we strive to help our consumers be productive members of the Flagstaff and Coconino County communities. 3

Pathway to Recovery Winter of 2013 was life-changing for Emily Bean. Her life had been all about the bottle. Emily drank day in and day out. However, on December 5th her life changed forever. That day, after 40 years of marriage, Emily s husband left her. She continued to drink and it was draining her. She finally realized she needed to get help. Emily found The Guidance Center and was directed to the Alcohol Stabilization Unit. Her blood pressure was alarmingly high, so she was sent to the Emergency Room. Emily was determined to get well, so she returned to the Alcohol Stabilization Unit the next day and awaited an opening in the Substance Abuse Rehabilitation Unit. On January 2nd she was able to check herself into the rehab. She was scared, alone, and sick, but she was also determined to succeed. The first few weeks at the unit Emily listened to stories. She said, As I listened to everyone talking, I knew this is where I needed to be. Emily finished the program on February 2nd. Emily was still angry that her husband left her but she thought to herself everything happens for a reason, because she was on her way to recovery. She said, If he never left me, I would still be drinking. After she finished the rehabilitation program, Emily was scared to be alone. Thoughts of alcohol continued even though she successfully completed the Substance Abuse Rehabilitation program. As with many others, more work was needed, and The Guidance Center was there to help. Emily received transportation for an entire year, three days a week, to attend relapse prevention and family group. Assertive Community Treatment Team ACT stands for Assertive Community Treatment and is comprised of a team of dedicated case managers and behavioral health professionals who provide intensive wrap-around services to work with consumers who have severe and persistent mental illness, have continuous high service needs, and have experienced difficulty with traditional case management and office-based outpatient services. The team can travel to the consumers homes and assist with activities of daily living, employment, medications, counseling, and crisis management. therapy and counseling Adult Services Adult Services provides therapy, case management, and life skills assistance for adults living with mental health difficulties. Adult Services specializes in depressive disorders, anxiety disorders, serious mental illness, substance abuse, and post-traumatic stress disorder. In addition, Adult Services provides housing services and assistance for those who have a serious mental illness and who would otherwise be living on the streets or in shelters. Alcohol Stabilization Unit The Alcohol Stabilization Unit is a safe environment for people who are alcoholdependent and are seeking immediate stabilization. The Alcohol Stabilization Unit is open 24/7, and the team works to get the clients sober, to break the cycle of repeated inebriation, and to provide a permanent path away from substance abuse by encouraging participation in outpatient early recovery groups and admission to the Residential Recovery Program. case management Emily continues to stay sober today by attending social groups and therapy sessions. She said she knows she must continue to work on herself. The Guidance Center is where I found my life, my health, and my happiness. Emily is currently employed at The Guidance Center as a peer support specialist. 4

Toward a New Future Child and Family Services Child and Family Services provides a wide range of therapeutic services to children, adolescents, and their families. These include: counseling for physical, emotional, and sexual abuse; trauma associated with parental drug abuse, domestic violence, parental mental illness, or removal of child from home; emotional/behavioral problems at home, school, or community; adolescent alcohol and other drug use, abuse, and dependence. Other Child and Family Services include: assessment and treatment planning/ coordination, crisis intervention, home and community based services, and individual, group, and family therapy. Crisis Services The Crisis Department is open 24 hours a day, seven days a week for those needing immediate care. Many consumers in crisis can be stabilized in a short period of time. For those with more involved issues, The Guidance Center recently added a new Extended Crisis facility to this department. In this area, consumers can stay in a safe, comfortable, and calm environment for up to four hours for stabilization and in-depth assessment. They are also quickly connected to other services at The Guidance Center that will assist with their long-term recovery goals. life skills assistance HealthCare Connect HealthCare Connect is a partnership between North Country HealthCare and The Guidance Center to provide primary health care service to The Guidance Center s consumers. Located on The Guidance Center campus, HealthCare Connect Clinic provides easy access to primary health care services on-site for those who are receiving mental health and/or substance abuse treatment services at The Guidance Center. By working together, North Country HealthCare and The Guidance Center offer not only convenience, but also built-in coordination of care. Richard started receiving services at The Guidance Center when he was just 19 years old. His symptoms were so severe that he was in and out of the Psychiatric Acute Care Unit, which provides emergency hospitalization services. He attempted suicide by overdosing on medication. Quickly after this decision, he knew it was a bad idea and called 911. As he described this incident, Richard explained that it is hard to remember the events around this, but he does remember leaning on The Guidance Center for support. The Psychiatric Acute Care Unit gave him safety, safety from himself. It s hard for me to believe, looking back at all the difficulties, that I m here now and how many things had to fall into place at just the right time, in just the right way. Richard remembers the significance of his recovery and shares his milestones with the staff at The Guidance Center. At the suggestion of his Case Manager, Richard started volunteering at Riordan Mansion State Historic Park eight years ago. For years Richard was quiet and withdrawn, riddled with anxiety while working the front desk. Through the NAMI Wellness Project, Richard was able to join a gym and have the support from a personal trainer who helped build his self-confidence. He noticed the changes in not only his health but his attitude as well. Richard began to give public lectures at the Riordan Mansion, and he began to see changes in himself that he never experienced before Richard began to see his true self. Richard started to consider a future that he had written off, a future he never thought he had. His therapist at The Guidance Center noticed the changes and encouraged him to apply for a Peer Support position. Today, Richard is a successful peer support specialist who helps others through the path to recovery. Richard acknowledges the importance of his new position, This was a very important step towards a career I never envisioned. Richard continues to take the steps needed to leave the past behind and look to a future of success. 5

FYE September 30, 2014 Income Statement 12 Months Ended 9/30/14 REVENUES Healing through EMDR Therapy by Natalie Nixon When I was ten years old, I suffered a series of traumas. And the weight of those traumas has hung around my neck for the last 27 years. I have suffered a severe lack of self confidence for decades, as well as an over amplified sense of self-consciousness; in every activity in my life I find myself wondering, Am I going to fail at this? And what are the people around me thinking? I bet they can sense that I don t have what it takes to do this. I have tried counseling in the past, but then my life was complicated by being married to an emotionally abusive man, and the therapy that I received did not seem to penetrate deep inside me. The day my husband s abuse turned from emotional to a fist hovering in front of my nose and screams of how much he wanted to punch me, was the day I walked out. And found freedom. Total Revenues $19,943, The first year was a struggle, I won t lie. There were times when I was not sure where I would be living, or how I would be paying my bills or caring for my children s needs. But each day away from him I felt, deep down, incrementally happier. And once my life settled, and my basic needs were solidly met, I realized that I finally had the freedom to tackle deeper issues from my past. EXPENSES At the Guidance Center I have found healing, hope, and empowerment. And I have found these things through the combination of a highly qualified, genuinely caring counselor, and the EMDR (Eye Movement Desensitization and Reprocessing) therapy that my counselor specializes in. EMDR therapy has helped me re-process the trauma that I experienced. The pain that I went through is brought up, acknowledged, processed in a healthy way, and then, (in my experience) the pain is GONE. The memories of the trauma are not erased, but simply dealt with in a way that allows my brain to let go of the pain. And once again, I have found freedom. Total Expenses $19,893, Revenues over (under) Expenses $49, This time the freedom manifests itself in a new confidence. And a lack of worry about what others think of me. Several weeks ago I tried something new I went to the indoor climbing gym. In the past, I would have been stressed out about this. About trying something physically challenging in a room full of strangers. I would have worried about falling. Or looking like a fool while struggling up the walls. I would have thought that people might be snickering under their breaths at my efforts. But those worries did not plague me this time. I was excited about trying something new. I trusted that the employees there were truly friendly and helpful. And inside myself, I had the amazing thought: I can do this. No problem. I climbed those walls. And I reached the top! 7
