JACKSONVILLE, FLORIDA This event is hosted by The Kappa Alpha Psi Foundation of Jacksonville Florida a 501(c)(3) nonpartisan organization committed to improving the quality of life for male youth, seniors and families in need residing in the Northeast Florida Metropolitan area by providing financial support for social programs, scholastic achievement and health awareness. All members of the Foundation and the Jacksonville Alumni Chapter of Kappa Alpha Psi Fraternity, Inc. are volunteers. We rely on individual, corporate and foundation support to remain a relevant part of the community we serve. With your support we will continue to nurture and build the community with activities such as these. A portion of the proceeds from this event will also support The Sickle Cell Disease Association of Florida.. 1

We invite you to join us on Saturday, January 11, 2014 for the 2nd Annual Healthy, Fit and Smart Family 5K Run/Walk to benefit the Sickle Cell Disease Association of Florida. Your participation will not only help to improve and maintain your healthy body, but also provide financial support and awareness to those individuals battling Sickle Cell Disease (SCD) each and everyday. It is estimated that Sickle Cell Disease: Affects 90,000 to 100,000 Americans Occurs in approximately 1 out of every 500 African American births Occurs in approximately 1 out of every 36,000 Hispanic American births Trait is present in approximately 1 out of every twelve 12 African Americans Sickle Cell Disease creates a physical, emotional, and financial burden to the patient and families of those who suffer from this disease. Your contributions will help us support the mission of the Foundation to eventually find a cure as well as help those who suffer on a daily basis. 3 Saturday, January 11, 2014 Time: 5K at 9:00 AM, 1 Mile Fun Run at 10:00 AM Location: The Jacksonville Landing 2 Independent Drive Jacksonville, FL The Healthy Fit and Smart Family 5K Run/Walk is open to all runners and walkers. The purpose of this charitable event is to bring attention to the importance of living a healthy, fit and smart lifestyle for you and your families. Healthcare professionals will be on hand at the event to answer questions and provide you with insightful educational resources. We encourage the Northeast Florida community and the city of Jacksonville especially, to come out and enjoy a nice run or walk and get your 2014 year off to a healthy start! We will have fun activities for the kids and family as we all work together to promote a healthy, fit and smart lifestyle. The Jacksonville Alumni Chapter of Kappa Alpha Psi Fraternity, Inc., 1st Place Sports and the Kappa Alpha Psi Foundation of Jacksonville, its volunteers, and all of its officers and/or representatives shall be free from any liabilities or claims for damages arising by reason of injuries to anyone during the conduct of this event.
