2 General Liability Plicy Highlights InsureMyEquipment.cm ffers yu Camera Operatr General Liability prtectin frm third party claims fr prperty damage and/r bdily injury. The plicy serves t prtect the insured frm lawsuits where the insured has been deemed liable fr damages t a 3 rd party. This cverage is ften required by many rental huses and lcatins check yur cntract if yu aren t sure. When yu have purchased a plicy, yu ll be able t issue a certificate f insurance listing yur rental r lcatin vendr as Additinally Insured. This plicy will prvide the fllwing cverage: $2,000,000 General Aggregate (shrt-term) $2,000,000 Prducts / Cmplete Operatins $1,000,000 Persnal & Advertising Injury $1,000,000 Each Occurrence $100,000 Fire Damage / Any One Fire $5,000 Medical Expense Yu can add the fllwing ptinal cverage t yur plicy: $1,000,000 Nn-Owned & Hired Aut Liability (nt available in CT, FL, HI, LA, MA r VA) $100,000 r $200,000 Third Party Prperty Damage See mre under Additinal Cverage Optins Hw t Apply All applicants must have a permanent US mailing address, a valid US credit card, and a valid address fr lg-in purpses. The plicy is specifically designed t cver vendrs r subcntractrs invlved in the prductin, film r entertainment industry including but nt limited t: Cameramen, Phtgraphers, Set Designers, Sund Engineers & Crdinatrs, Sund Mixers, Vide Editrs, Lighting Specialists, Gaffers/ Grips, Character/ pht exhibits, Seasnal displays, Celebrity appearances, Tradeshw, fair, expsitin & cnventin exhibitrs, bk signings & autgraph bths, Fd vendrs, Flea market vendrs, Farmers Market Bths, Arts and Crafts vendrs, temprary exhibitrs f any king, etc. Q: What des this plicy exclude? All plicyhlders are respnsible fr reviewing their plicy and exclusins. The fllwing list is nt meant t replace the plicy language and/r exclusins in the plicy. The maximum length fr this plicy is 3 mnths. When the plicy expires, yu are able t purchase anther 3 mnth plicy. Yu wn t be able t renew a plicy. 4

5 Q: Hw can I cancel my liability plicy? Please nte that all shrt-term plicies are fully earned and nn-refundable upn binding. All plicies less than 12 mnths are cnsidered shrt-term. Please make sure t review ur cancellatin dcument and d nt purchase the plicy unless yu are very sure it ll be needed. Q: Will this plicy cver my prductin and everyne n set? N. This is nt a prductin liability plicy (DICE) and is intended fr use by individual camera peratrs, phtgraphers, etc. The plicy will nt adequately cver entire prductins but will meet the requirements t rent frm equipment vendrs and als t sht n lcatins which require 3 rd party prperty damage cverage. Additinal Cverage Optins Q: What is 3rd Party Prperty Cverage? This ptinal cverage insures physical damage t a lcatin r prperty that is nt wned by yu r yur cmpany. This cverage may be a part f the insurance requirements f a lcatin r rental peratin. Yu are able t set this cverage limit t either $100,000 r $200,000. Example: Yu are hired t sht a wedding and yur camera falls n a church pew, denting the wd. The church may request payment fr damage t their prperty and this cverage wuld prvide claim payment. T add this cverage after yu ve bund yur plicy, lg in t yur hmepage dashbard and Q: What is Hired & Nn-Owned Aut liability? This ptinal cverage insures 3 rd party bdily injury r prperty damage resulting frm an accident invlving a hired/nn-wned aut perated by the insured r an insured s emplyee fr business purpses. $1,000,000 is the ttal plicy limit fr the plicy perid. Example: Yu ask an emplyee t run an errand, visit a client, pick up lunch, etc, in his r her wn car. The emplyee causes an accident and the injured party lks t yu r yur cmpany t pay fr the damages, since the emplyee was n cmpany time. This cverage 5

6 culd als apply if yu ask a nn-emplyee t casually run an errand befre stpping in t visit and an accident ccurs. This ptinal cverage insures physical damage resulting frm an accident invlving a hired/ nn-wned aut perated by the insured r an insured s emplyee fr business purpses. Physical damage is a brad term that includes cllisin cverage (eg: yu are in car accident with a fixed bject r ther aut) and cmprehensive cverage (eg: an accident nt cnsidered cllisin that is ut f yur cntrl, like hitting a deer.) $1,000,000 is the ttal limit fr the plicy perid. T add this cverage after yu ve bund yur plicy, lg in t yur hmepage dashbard and Q: What is a Waiver f Subrgatin? This ptinal cverage prhibits the insurer (Atlantic Specialty Insurance Services) frm seeking restitutin frm a 3 rd party that causes the insured/plicyhlder t experience any claim-related lss. Sme rental huses r prductins will require yu t purchase a waiver f subrgatin. T add this cverage after yu ve bund yur plicy, lg in t yur hmepage dashbard and Q: What is a Primary and Nn-Cntributry Wrding? Primary wrking n a plicy means that the plicy with this endrsement will pay fr the claim befre any ther plicy. The nn-cntributry wrding n a plicy means nt nly will the plicy with this endrsement pay first, but the plicy will pay the full amunt f the claim until the limits are exhausted befre any ther plicy cntributes t the lss. Example: A subcntracted camera peratr is required t prvide separate GL insurance by a prductin that is primary and nn-cntributry in relatin t a prductin s GL plicy. This means that the camera peratr s plicy must pay befre all ther applicable plicies (primary) and withut seeking cntributin frm the ther plicies that are als endrsed as primary (nn-cntributry.) This means the subcntractr s plicy will be the ne that pays first and t its fullest extend fr any claims invlving the camera peratr befre any ther plicies prvide cmpensatin. Nte: An insurance plicy that states it is bth primary and nn-cntributry is bligated t respnd first in the event f a claim. 6

Q: What is the minimum plicy price? Premium Payment Liability plicies start as lw as $455. Cverage is quted n a flat-rate basis and des nt fluctuate based n plicy length. Q: What shuld I d if my credit card is nt wrking? Almst all f ur credit card declinatins are due t the billing address nt matching the address entered. Duble check that all CC infrmatin is crrect and send an t internetbusinessdivisin@heffins.cm if yu cntinue t have issues. We can nly accept credit card payments r debit payments frm cards with addresses within the United States. Q: D I pay my premium nce r thrughut the year? We dn t ffer any payment plans. Plicies must be paid fr, in full, at the time f purchase. Q: Can I pay with a check r mney rder? All payments must be made nline with a credit card r debit card with a US mailing address. Our prgram n lnger accepts checks, mney rders, wire transfers r any frm f payment ther than a credit r debit card. All payments must be made thrugh ur website. We dn t prcess payments ver the phne. We d nt keep yur credit card infrmatin n file after yur plicy has been purchased. 7

Yu can als change yur passwrd, submit a claim r apply fr a rented r wned equipment plicy. Q: Hw d I issue certificates? Certificates Upn cmpletin f yur applicatin, yu ll be asked t designate a certificate hlder. Yu can click n ne f ur rental huse partners in the drp-dwn menu, r add a custm cert hlder. Nte that yu are able t issue mre certificates f insurance when yu ve purchased 8

9 yur plicy. Just click the link t Issue a Certificate under Plicies n yur hmepage dashbard. Make sure t list yur vendr as the certificate hlder and nt yurself. Q: I have t list my rental huse as Additinal Insured as well as Lss Payee. Hw d I d this? This plicy will allw yu t list yur vendr as Additinal(ly) Insured. T list yur vendr as Lss Payee, yu ll need t carry a rented equipment plicy r add rented equipment cverage t yur wned equipment plicy. Yu can apply fr either f the tw plicies frm yur hmepage dashbard. Q: Can I issue certificates f insurance t cmpanies that are lcated utside f the United States? N. Our plicy is written n the law f the land and will nly wrk t cver rentals frm cmpanies that are dmiciled within the United States. We can make exceptins fr Canadian cmpanies n case-by-case basis fr a fee. Please cntact InternetBusinessDivisin@heffins.cm if yu need a certificate fr a Canadian cmpany. Claims Q: Hw lng des a claim nrmally take t get paid? In rder fr the claims adjusters t mve frward, they need all infrmatin relevant t yur claim. Because the circumstances f Camera Operatr General Liability claims vary frm claim t claim, the timeframe fr each claim varies substantially. We will maintain clse cntact with the claims adjuster and be sure t keep yu up t date n any develpments surrunding yur claim. Q: Hw d I file a claim? Lg in t yur hmepage dashbard. Click the link t submit a claim; Yur claim applicatin will be received by the claims department at Atlantic Specialty Insurance Services. With ur assistance, they will assign a claim number and a claims adjuster t assist yu with clsing yur claim. This adjuster will send yu an and call yu n the phne within 72 hurs f yur applicatin. Make sure t keep all receipts, invices and plice reprts available t share with the adjuster. Yu will receive a cpy f the claims reprting guidelines when yu submit yur claim.
