22 Certificate Renewal Renewal yearly. Submission requirement: - Renewal form (TAC/FOR/05-1) - Original Certificate Of Conformity -Renewal fee: RM50.00 per year per COC The maximum validity period is not more than 5 years from the current year Additional Host Additional host is only allowed for the same approved product and has valid COC. The certificate holder shall submit: Application form for add host; -Original COC; - Product Specification; -Additional Host Fee: RM per model. 22

23 Submission Import : Valid import permit or importation documentation under the applicant s name Local : Declaration letter from the manufacturer/assembler. Sample (Max : 4) Marking : model number and brand Sampling : Import : Production.Local, prototype may be accepted. Complete : In good working condition and supplied with accessories, peripherals and test jig, where applicable Responsibility & Technical Clarity : Applicant or appointed consultant On-site (field) inspection Upon official request, on-site (field) inspection may be carried out due to sample is : huge, expensive, high logistic cost, customised product, order based on tender approval etc;. Additional costs related to the inspection shall be borne by the applicant. Collection Shall collect the sample within two (2) weeks from the date of issuance of COC Notify through e-comm. Uncollected sample will be disposed. 23

39 Electronic Permit (epermit) is a web based service provided by Dagangnet that enables importers to apply for permit from SIRIM and obtain approval online. Approved permit swill then be electronically transmitted to SMK for validation and cross reference against Customs Declaration. 39

40 Scope: ALL radio communications apparatus capable of being used for telecommunication in the frequency band lower than 3000 GHz or their motherboards and apparatus or equipment to be attached or connected to Public Network or System imported into Malaysia must be declared to Royal Customs, irrespective whether they are dutiable or not, inclusive of equipment for licensed network.
