2 1 What is the Small Business Marketplace? An online Marketplace where you can select from a variety of health and dental plans to offer your employees. Through the Marketplace you can offer your employees multiple carriers to choose from while setting contribution levels that make sense for your business. Depending on your size, tax credits may also be available for eligible businesses. You can set up a plan on your own or with the help of a broker or assistor. What are the benefits of the Small Business Marketplace? The best place to find low-cost, quality health plans Right for small businesses with 50 or fewer employees Lets you select one plan, several plans, or all plans offered where a person lives or works Gives you the option to choose how much of the cost you want to share Has brokers and other assistors to guide you and your employees through the enrollment process Helps you attract and keep workers who want health coverage for themselves and their families

3 2 Is my business eligible to participate? Yes, if your business: Is located in, or has a primary business location in New York State Has 1-50 eligible employees Employs at least one common law employee as defined below Offers health insurance coverage to all of your eligible employees Do I have to offer coverage to all employees? You must offer coverage to all eligible full-time employees. You as the employer define who is full-time. You can also choose but are not required to offer health insurance to part-time workers, defined as working 20 hours or more per week. Business owners are eligible to enroll. Sub-contractors and independent contractors are not considered employees, and cannot enroll. A note about sole proprietors: Under federal law, sole proprietors or businesses that do not employ at least one common law employee are not eligible to purchase coverage as a small group. A common law employee is any employee who is not an owner, a partner or the spouse of an owner or partner. Sole proprietors or other types of businesses without a common law employee may buy health insurance coverage through the Individual Marketplace.

4 3 What health and dental plans are available in 2015? The following health insurance carriers are participating in the Small Business Marketplace in 2015: The following dental carriers are participating in the Small Business Marketplace in 2015: DELIVERY SYSTEMS, INC. How much do health plans cost? To learn more about the cost of each health plan, please visit our website at and use our get quotes tool. Rates are updated quarterly. The rate you get when you sign up is the rate you ll keep for 12 months. What do I need to know about tax credits? You may be able to get tax credits based on the amount you pay toward your employees monthly premiums if you: Employ fewer than 25 full-time workers (two half-time workers count as one full-time worker) Pay your employees an average of less than $50,000 a year Share at least half of employees premium costs You should work with a tax professional to determine if you qualify for this credit.

5 4 How can I enroll my business? Contact a broker or assistor. They can help you: Set up your online account Compare health and dental insurance options Create an enrollment offering for your employees Assist employees with enrollment, making changes, and understanding enrollment periods Brokers and assistors can also help you understand the eligibility criteria for the Small Business Health Care Tax Credit. Search for a broker or assistor at hx_brokersearch Go online at nystateofhealth.ny.gov. You can set up your own account and decide which health plans to offer to your employees. Call the Customer Service Center at Representatives can assist you over the phone. When can I enroll my business? You can choose to start a plan at any time. Once a plan effective date is chosen always the first of the month it becomes the start date for your group s plan year. The chart below indicates what a group plan s coverage effective date will be, based on the open enrollment end date.
