1 NEW JERSEY MARKET Nomaan Ashraf, MD (Orthopedic Surgeon) Dr. Nomaan Ashraf is a spine surgeon at Comprehensive Spine Care, PA in New Jersey. He completed his college education with honors at The University of Pennsylvania in Philadelphia. Following his Ivy League college education he completed his medical education while simultaneously getting a business degree as part of the Tufts University School of Medicine's MD/MBA program in Boston, MA. After medical school he completed his internship in Surgery at New York Presbyterian Hospital (Cornell) in Manhattan. He then completed his Orthopaedic Surgery training at the Carolinas Medical Center under the tutelage of world wide leaders in Orthopaedic Surgery. While there he was selected by the Department to serve as Administrative Chief Resident in his final year. During his time there he also completed a year- long "Fellowship in Orthopaedic Surgery Basic Science and Clinical Research". Following his Orthopedic training, he moved to Cedars-Sinai Medical Center in Beverly Hills, Los Angeles where under the direction of Rick Delamarter, MD he received world class training in adult spine surgery including scoliosis surgery, complex revision surgery, minimally invasive surgery, dynamic stabilization techniques and both fusion and non-fusion alternatives. He has also received extensive training in motion-preserving spine surgery such as disc replacements for the lumbar and cervical spine and has participated in a number of clinical device trials. He has authored numerous articles and presented his research at numerous national and international meetings. Dr. Ashraf's clinical interests include cervical and lumbar spinal surgery, the evaluation and surgical management of degenerative disorders of the cervical, thoracic and lumbar spine (such as herniated discs and spinal stenosis), minimally invasive spine surgery, microsurgery, spinal trauma and spinal tumors. Dr. Ashraf is a nationally recognized leader in spinal surgery and has advanced expertise in motion sparing procedures of the cervical and lumbar spine including artificial disc replacement. He has authored several articles and contributed to numerous textbooks in the field of spinal surgery. Elizabeth Baker, MD (Physical Medicine and Rehabilitation) Dr. Baker is board certified in Physical Medicine and Rehabilitation as well as Electrodiagnostic Medicine. She is also boarded in the subspecialty of Pain Management. Dr. Baker received her medical degree from Columbia University, College of Physicians and Surgeons in New York. She completed an internship in Internal Medicine at Columbia-Presbyterian Medical Center as well as her residency in Physical Medicine and Rehabilitation. Dr. Baker treats patients in our Teteboro Concentra Clinic. Michael Baruch, MD (Plastic Surgeon, Hand Surgeon) Dr. Baruch is board certified as both a general surgeon and plastic surgeon. He is a graduate of Albert Einstein College of Medicine and did his residency in general surgery at Montefiore Hospital and Medical Center in New York City and Episcopal Hospital in Philadelphia. Dr. Baruch completed his plastic surgery training at Boston University Hospital. Dr. Baruch covers the full range of plastic surgery, cosmetic and reconstructive surgery. He specializes in hand injuries, including finger and hand fractures, as well as hand surgery. Dr. Baruch also specializes in skin cancer. He is the president of the Hudson County Medical Society. Peter Chan, MD (Hand Surgeon) Dr. Chan completed his undergraduate studies at Duke University and went on to graduate Summa Cum Laude and valedictorian from Georgetown University School of Medicine in Dr Chan completed his post-graduate training in Orthopaedic Surgery at the Hospital of the University of Pennsylvania, and in 1999, completed a fellowship in Hand Surgery and Microsurgery under the direction of David P. 2 Robert Doidge, DO (Orthopedic Surgeon) Dr. Doidge received his doctor of osteopathy from the Philadelphia College of Osteopathic Medicine. He continued with this orthopedic residency at Oakland General Hospital, Madison Heights, MI and completed an Arthroscopic Reconstruction Knee & Shoulder/Sports Medicine fellowship at Michigan State University, East Lansing, MI. Dr. Doidge treats general orthopaedic cases, but specialized in injuries related to the knee and shoulder. He is currently the Chief of Orthopedics with the Meadowlands Hospital Medical Center, Secaucus, NJ. Demetrios Econopouly, DPM (Podiatric Surgeon) Dr. Demetrios S. Econopouly received his BA degree from Columbia University and his Doctor of Podiatric Medicine from Barry University. After a two-year podiatric surgical residency at Meadowlands Hospital Medical Center in Secaucus, New Jersey and an Orthopedic Trauma Fellowship at Stadisches Klinikum with rotation at Children's Hospital in Karlsruhe, Germany, Dr, Econopouly established his practices in both New York and New Jersey. Suk Ghong, DPM (Podiatrist) Dr. Ghong attended Temple University School of Medicine where he received his degree in Podiatric Medicine. Patricio Grob, DO (Orthopedic Spine Surgeon, Trauma) Dr. Grob received his doctor of osteopathy as well as completing his orthopedic residency from the Philadelphia College of Osteopathic Medicine. Following his residency, Dr. Grob completed an orthopedic trauma fellowship from the University of Tennessee-Campbell Clinic, an acute spine trauma fellowship from Northwest University, and his final fellowship was with John Border Memorial European Trauma. Dr. Grob currently treats out of the Clifton, Newark, and Elizabeth Concentra locations. Scott Jarmain, MD (Physical Medicine and Rehabilitation) Dr. Jarmain is Board Certified in Pain Management and Physical Medicine and Rehabilitation, obtaining his MD from the American University of the Caribbean School of Medicine in the British West Indies. He completed his residency at Temple University/Moss Rehab Hospital in Philadelphia, PA and his fellowship in Musculoskeletal/Pain, Physical Medicine and Rehabilitation at Johns Hopkins University Hospital in Baltimore MD. Stephen Kocaj, MD (Orthopedic Surgeon) Dr. Kocaj is a Board Certified orthopedic surgeon, obtaining his BA degree from Middlebury College in Vermont followed by his MD from UMDNJ-New Jersey Medical School in Newark. 3 Jonathan Lester, MD (Physical Medicine and Rehabilitation) Dr. Lester received his undergraduate degree and medical degree from University of Florida, Gainsville, FL. Dr. Lester completed his fellowships with the American Board of Pain Medicine, American Board of Electrodiagnostic Medicine, and with the American Board of Physical Medicine and Rehabilitation. Dr. Lester is the Medical Director for the Post-Acute Injury Management Program with Concentra. He is currently active with the Medical Society of New Jersey Committee on Legislation as well as the Essex County Medical Society, Committee on Pain Management. Rafael Levin, MD (Spine Surgeon) Dr. Levin received his medical degree from Johns Hopkins University School of Medicine, Baltimore, MD. He continued with his medical training by completing his orthopaedic surgery residency with the Boston Medical Center and further went on to complete his fellowship training with spine surgery through the NYU-Hospital for Joint Diseases. Todd Lipschultz, MD (Orthopedic Surgeon) Dr. Lipschultz graduated with an MD from The University of PA School of Medicine and completed both his internship and residency at Mt. Randy Marrinan, MD (Physical Medicine and Rehabilitation) Dr. Marrinan completed his medical degree at Albert Einstein College of Medicine in New York. Armando Martinez, MD (Orthopedic Surgeon, Hand) Dr. Armando Martinez is an orthopedic surgeon who has been Concentra s full time orthopedic surgeon since Dr. Martinez treats general orthopedic injuries but also specializes in the treatment of hand injuries. Dr. Martinez is affiliated with the Hudson Crossing Surgery Center and is currently treating patients out of the Newark, Jersey City, Teterboro, and Secaucus Concentra locations. Kamaldeep Momi, MD (Orthopedic Surgeon, Spine) Dr. Momi is a board-certified orthopedic surgeon whose focus is exclusively on the neck and spine. After earning his bachelor's degree at Pennsylvania State University, Dr. Momi completed his medical degree at Jefferson Medical College in Philadelphia, and his internship at Beth Israel Medical Center, in New York City, followed by a residency in orthopedics at UMDNJ-New Jersey Medical School. Dr. Parolie completed a sports medicine fellowship at Cleveland Clinic Foundation. Richard Schenk, MD (Orthopedic Surgeon, Trauma) Dr. Richard Schenk received his undergraduate degree from Cornell University and continued on to earn his medical degree from New Jersey Medical School University of Medicine & Dentistry NJ. Dr. Schenk completed his orthopedic residency through the University of Medicine & Dentistry NJ and its affiliated hospitals. Seth Scholl, DO (Physical Medicine and Rehabilitation) Dr. Scholl is a specialist in physical medicine and rehabilitation and deals solely with the nonsurgical treatment of neck and back pain. Dr. Scholl was born and raised in New York, where he studied at Binghamton University, graduating cum laude, with a Bachelor of Science in biology. He was then accepted into the New York College of Osteopathic Medicine and began his medical training. After medical school, Dr. Scholl was one of four physicians chosen to spend an extra year teaching as a fellow through the anatomy department. He then completed his internship at Long Beach Medical Center in Long Beach, New York. During his internship, he wrote a computerbased medical book that was designed to diagnose and treat medical conditions using the Palm Pilot. He then moved to Philadelphia to begin his three-year residency at Temple University/Moss Rehab Hospitals in physical medicine and rehabilitation. At CoastalSpine, Dr. Scholl specializes in helping many patients find relief of back and neck pain symptoms through nonsurgical treatments. Dr. Scholl is the director of the electrodiagnostic laboratory at CoastalSpine. He is proficient in performing EMGs that are done to help diagnose a pinched nerve in the spine. He is also well-trained in trigger point injections, which can relieve muscle-related pain. Dr. Scholl maintains his affiliation with Temple University and has been granted the title of associate professor through the rehabilitation department. Dr. Scholl is active in teaching and in the mentoring of physical medicine residents. He is a member of the American Association of Physical Medicine and Rehabilitation and the American Association of Physiatrists. Over the last few years, he has contributed to the publication of multiple articles. During his free time, Dr. Scholl enjoys sprint and Olympic distance triathlons, competitive running and hiking. Steven P. Shikiar, MD (General Surgeon) Dr. Shikiar is a Board Certified General Surgeon whose practice specializes in offering minimally invasive solutions to common surgical problems. Dr. Shikiar graduated medical school at the Albert Einstein College of Medicine and completed his General Surgery residency training at New York Medical College, Westchester County Medical Center. With the use of Laparoscopy, Dr. Shikiar treats such conditions as gallstones, hernias, severe gastro-esophageal reflux, gastric and colonic disorders. Mark Solomon, DPM (Podiatrist) Dr. Solomon attended the New York College of Podiatric Medicine where he received his degree in Podiatry. Dr. Solomon was the Chief Resident at Atlantic Health Systems in Morristown, NJ during his residency in Reconstructive Foot and Ankle Surgery. Anthony Spinnickie, MD (Orthopedic Surgeon) Dr. Anthony Spinnickie received his medical degree from Robert Wood Johnson School of Medicine-UMDNJ in Piscataway, NJ. He completed his orthopedic surgery residency at Stanford University Medical Center before

5 going on to complete a fellowship in Orthopedic Trauma Surgery with Stanford University Medical Center. Dr. Spinnickie treats patients out of the Elizabeth Concentra Clinic. Nora Taha, M.D. (Physiatrist) Dr. Taha is Board Certified in Physical Medicine and Rehabilitation and in Musculoskeletal diagnostic ultrasound. She specializes in musculoskeletal disorders and interventional pain management. Her background in physical medicine and rehabilitation affords her to better diagnose various acute and chronic conditions that contribute to pain. During her fellowship, Dr. Taha gained proficiency in fluoroscopic and ultrasound guided procedures which aim to reduce pain and help restore maximal function and quality of life. With the combination of these two medical disciplines, Dr. Taha is able to develop a specific treatment plan tailored to the needs of her patients. Dr. Taha graduated from Rutgers University with a Bachelor s of Science degree and she obtained her medical degree at St. Matthew s University in the Cayman Islands. Tovey, MD (Orthopedic Surgeon) Dr. Tovey is a Board Certified Orthopedic Surgeon with Somerset Orthopedic Associates in Bridgewater, New Jersey. Welch, DPM (Podiatrist) Dr. Matthew Welch is a New Jersey podiatrist specializing in the treatment of all foot and ankle problems.
