2 ASSOCIATE IN SONOGRAPHY (AAS DMIS) Mission Statement The primary goal of the Associate Degree in Sonography Program is to educate students in didactic, laboratory and clinical experiences in preparation for a health career as a Diagnostic Medical Sonographer. The graduate will demonstrate competency to meet certification requirements, deliver compassionate patient care, and function as an integral member of the health care team with competence and confidence. Program policies and procedures have been designed to meet or exceed those established by the Joint Review Committee on Education in Diagnostic Medical Sonography (JRC-DMS). Program Goals 1. Educate competent and compassionate sonographers capable of functioning as an entry level Sonographer within 24 months. 2. Provide a comprehensive competency-based curriculum. 3. Prepare the student to think and act independently. 4. Prepare the student for entry into the ever-changing sonography work environment. 5. Prepare the student to achieve a satisfactory result in the American Registry of Diagnostic Medical Sonographers (ARDMS) board exams for Obstetrics/Gynecology, Abdomen/Superficial Structures, Physics Instrumentation (SPI) & Vascular Technology (RVT). 6. Provide comprehensive student education to aid in student retention, course, and program completion. 7. Provide an opportunistic education for students in three major modalities of sonography. Program Description/Introduction Diagnostic Medical Sonography (Ultrasound) is a clinical discipline involving the use of equipment that produces high-frequency sound waves that create images for medical interpretation and diagnosis by a Radiologist. The Sonography Degree Program is designed to prepare students for entry-level positions in Sonography. All general education courses required for the Associate in Applied Science Degree must be completed prior to entry into the Sonography Degree Program. Admission requirements are outlined in this packet. To be considered for Fall 2015 admission, completed applications must be received by May 15, All prerequisite course work must be completed by the end of the spring semester prior to program start in the Fall. Academic Success in the AAS Sonography Degree Program All students enrolled in the Sonography program must maintain a minimum of C or better in all DMIS courses in order to continue in the program. Receiving a D in any DMIS course will terminate the student s continuation in the program. Students requesting readmission for the following year are evaluated based upon the current merit evaluation and ranked with all possible incoming students. An F in any course will negate the possibility of readmission to the sonography program at College of DuPage. Upon successful completion of the program, the graduate is awarded an Associate in Applied Science degree from College of DuPage and becomes eligible to take the American Registry of Diagnostic Medical Sonographers examinations in Physics (SPI), Obstetrics/Gynecology, Abdomen/Superficial Structures, and Vascular Technology. Students who successfully pass the Physics (SPI) examination as well as a corresponding specialty examination (Obstetrics/Gynecology, or Abdomen/Superficial Structures) will receive the national credential of Registered Diagnostic Medical Sonographer (RDMS). Completion of the SPI examination and the Vascular Technology examination will earn the credential Registered Vascular Technologist (RVT). Last Updated 4/13/2015 2

3 Statement of Health Prior to starting clinicals, all students in the AAS Sonography Degree Program will need to complete health requirements, a drug screen, background check, and complete the medical document manager requirement. You will be informed by the Program Coordinator when you will need to complete your health requirements. Please do not complete these requirements until you are instructed to do so. The Health Requirements packet is available on the Health Sciences Admissions and the Diagnostic Medical Imaging Sonography webpages. Students with chronic medical conditions and physical limitations are required to submit documentation of current health status relating to their specific condition. The statement should include the physician s assessment as to the student s ability to meet the program objectives and the current medications the student is required to take for health maintenance. Statement on Medical Care and Health Insurance Students are responsible for their own medical care and health insurance while in the program. Students must possess health care insurance in order to train at the clinical affiliate. The student is responsible for any costs that may be incurred related to personal injuries he/she may acquire while performing activities at the clinical affiliate. The affiliating clinical facility and/or College of DuPage cannot be held liable for such injuries. Students enrolled in at least 6 credit hours of classes may purchase student health insurance through University Health Plans. Students are required to provide evidence of current health insurance coverage prior to starting the program. Statement on Medical Malpractice Insurance Malpractice insurance is required and is included in the clinical education costs on an annual basis. That is, the cost of the insurance is included with the tuition and fees for appropriate courses. Students must be officially registered and listed on the instructor s class roster to be covered by the malpractice insurance. Statement on Drug Testing/Criminal Background Check A drug test and Criminal Background Check are required prior to starting a clinical rotation in the Sonography program. Several clinical affiliates also require finger printing. The costs of these requirements are the responsibility of the student. You will be informed by the Program Coordinator when you will need to complete these requirements. Please do not complete them until instructed to do so. Criminal Background Check Information All health care workers and students in a health care program are required to undergo a criminal background check in order to train in a clinical setting. A student with a positive background check containing disqualifying conditions as defined in accordance with the Health Care Worker Background Check Act (225 ILCS 46) and 77 Ill Adm. Code 955 Section will not be allowed to enter the clinical portion of the program, thus preventing the student from obtaining mandated certification and/or licensure in Diagnostic Medical Sonography. PLEASE NOTE: If you have been found guilty of a felony you will be unable to obtain a clinical site, or become registered to practice in this profession. Transportation Students are required to provide their own reliable transportation to the college and clinical sites. Public transportation is not an option since many clinical sites are more than an hour away from the college. Clinical placements are assigned by the Clinical Coordinator to meet the needs of all students and the program. Students cannot select clinical sites. Many clinical sites have parking fees, which are the responsibility of the student. Clinical sites can be over 100 miles from COD. Last Updated 4/13/2015 3

4 Standard Skills in Sonography To participate in the clinical education portion of the Sonography program, the applicant must possess additional non-academic skills. These technical standards are consistent with the duties of the entry-level sonographer in a professional position. The applicant must have the ability to: Lift and carry 30 pounds of weight. Lift and transfer patients to and from wheelchairs, stretchers, beds, and imaging tables. Move, adjust, and manipulate equipment to perform imaging studies. Position patients for exams. Respond appropriately to sounds such as the patient s voice and movements at a normal conversational volume. Review and evaluate recorded real-time images to determine the quality of the image, as well as demonstrate visual accuracy in differentiating among subtle shades of grays used in Diagnostic Medical Sonography. Respond appropriately to equipment signals such as sound and light. Manipulate the necessary hospital equipment for patient care and patient imaging, such as dials, switches, push buttons, and keyboards. Stand and perform exams for a prolonged period without breaks in a typical shift of eight to twelve hours. Fluent communication in the English language to function effectively in the program and at clinical sites. Position a transducer device in a back and forth manner while maintaining balance. Communicate orally and in writing with patients, physicians, and other personnel; clearly and effectively. Be able to follow verbal and written directions. Statement on Clinical Education The purpose of clinical education is to allow the student to apply the theoretical principles of the didactic portion of the program. Students may be required to rotate through multiple clinical education centers. Students will not replace members of the affiliated clinical staff; they will have the status of students. Students are not paid for clinical hours. Please be advised that the AAS Sonography Degree Program follows a rigorous curriculum requiring serious student memorization, practical application of theory, coursework, and transportation to/from class and clinicals. It is a full-time program. Taking personal time off (i.e. vacations) outside scheduled breaks is not permitted and will affect program continuation or ability to complete the program as scheduled. 6 College of DuPage Health Science Programs POLICY FOR PROFESSIONAL CONDUCT IN CLASSROOMS, AND AT LABORATORY/CLINICAL SITES Revised Fall 2013 Socialization of students to appropriate professional conduct is a significant component of the Health Science Programs of College of DuPage (the Programs ). Students in the Programs are expected and required to be reliable and competent, exercise sound judgment, act with a high degree of personal integrity, represent themselves and College of DuPage (the College ) in a respectful manner, and observe all the rules and regulations of the clinical sites to which they are assigned. Students also have a responsibility to protect the welfare and safety of the patients/clients for whom services are being provided. Students placed at clinical sites must realize the privilege of this experience, its responsibilities, and the reflection on the College of DuPage and its numerous health programs. Any violation will be handled immediately so as to not affect C.O.D. s use of the clinical site for future students. In accordance with these expectations, when participating in laboratory exercises and /or assigned to clinical sites, students in the Programs must adhere to the following Policy for Professional Conduct. This Policy is in addition to the Student Code of Conduct outlined in College catalogs and/or policy (Board Policy 20-35). The policy of Professional Conduct includes, but is not limited to, 1. Providing competent and reliable services to patients/clients using sound judgment and discretion. Students are expected to: a. be prepared for the learning environment and actively participate in appropriate ways to ensure learning of key components b. be on time c. refrain from excessive absenteeism d. remain in assigned work areas, leaving only with permission e. not leave patients unattended and/or release patients without prior approval from a qualified staff member (if applicable) 2. Demonstrating respect and courtesy to patients/clients and their families 3. Demonstrating respect and courtesy to peers/classmates, instructors, supervisors and all other members of the health care provider team in classrooms and at the site 4. Cooperating with faculty, staff and peers without insubordination 5. Providing care and/or services on a non-discriminatory basis 6. Performing only procedures and/or services authorized by supervisor(s) in accordance with accepted professional standards 7. Observing the rules and regulations of classrooms, and all clinical facilities including (but not limited to): a. proper use of equipment and other property b. not removing property of clinical facilities c. not distributing, possessing, and/or being under influence of illegal drugs or controlled substances, d. not possessing and/or being under the influence of alcohol e. complying with all current health requirements, drug testing and criminal background checking requirements f. not possessing or using any type of weapon g. comply with cell phone or other electronic device (ipod, ipad, electronic tablets, etc.) usage consistent with the policies of the classroom/clinical site or lab 8. Maintaining the confidentiality of patient/client information in accordance with recognized professional and institutional rules; without unauthorized release and/or misuse of patient/client information or institutional data 9. Maintaining proper dress, appearance, hygiene and decorum in accordance with the standards set by the program, clinical site, and professions 10. Complying with the ethical and professional standards set by the profession and the Programs including (but not limited to): a. never taking prescription blanks or writing prescriptions b. never falsifying documents regarding student performance or the care of patients The penalty for violating the Policy for Professional Conduct will be discipline up to and including suspension or dismissal from the Programs. Due to the sequential design of the curriculum in most Health Science programs, any period of suspension or dismissal may result in a delay in program completion. If the disciplinary action is suspension or dismissal from one of the Programs a form will be completed by the program coordinator and submitted to the appropriate Associate Dean. The student may appeal that disciplinary action to the Health Sciences Professional Conduct Review Board within 10 business days of the discipline imposed. This appeal must be in writing and submitted to the appropriate Associate Dean in the program area (HSC 1220). The student will not be permitted to participate in the clinical/lab portion of the Programs during the appeal process. However, the student may continue with on campus courses until a final decision is rendered from the appeal process. The appeal will result in a hearing before the Health Science Professional Conduct Review Board, comprised of faculty from other COD health science programs within 5 business days of the date the appeal is filed with the Associate Dean. Both the student and faculty involved in the disciplinary action may make an oral presentation to the Board in addition to written comments. There is no further appeal; the decision of the Board is final. If the decision to dismiss or suspend a student from a Program is upheld, no refund of tuition or fees or equipment or books will be provided. When a student is dismissed from a health program for professional misconduct, they are not eligible for readmission, or admission to any other health program at the College of DuPage. All health programs require completion of current health requirements and drug testing as a condition of program participation. Drug testing (performed at the student s expense) will precede clinical experience in all cases, and will be required prior to admission in some programs. Positive results on a drug screen or misrepresentation regarding drug use will lead to immediate dismissal from, or non-admittance to the particular program(s). When a student is dismissed from or not admitted to a program for a positive drug screen, he/she may reapply to the same program or to any other College of DuPage health programs after one year has elapsed. Criminal background checks are performed at the student s expense, and are required for many programs prior to admission. 7 Eligibility for Admission To be considered for admission into the Sonography Program, an applicant must: Complete the College of DuPage General Admissions Application online or in person at the College of DuPage Admissions and Outreach office, SSC There is a $20 general admissions fee due upon submission. If you have previously attended COD, you can omit this step. This application is for general admissions to College of DuPage only; it is not an application to the AAS Sonography degree program. Attend a mandatory Diagnostic Medical Imaging Sonography Advising Session. View upcoming session dates and times and please make sure to RSVP for your session to secure your spot using the directions provided. It is required that an applicant attend an advising session within a 12 month period prior to the application deadline. Schedule an appointment with a Health Program Advisor by calling The Health Program Advisor will help you develop an Academic Plan to ensure you complete all prerequisites. To ensure that all of your eligible credits are evaluated towards the admission requirements turn in all official Transcripts from any schools that you have attended. Please follow the steps listed below: Submit your official transcript(s). Contact your previous institution(s) to order an official copy of your transcript to be sent to the College of DuPage, Office of Student Records, 425 Fawell Blvd, SRC 2150, Glen Ellyn, IL Verify receipt of your transcript(s). Login to your myaccess account, click on myaccess for Students, then select My Profile. The receipt status of your transcript will be listed under Transcript Institutions. NOTE: If you have an international transcript from high school or college, it must be evaluated by EDUCATIONAL CREDENTIAL EVALUATORS, Inc. Forms may be obtained from All pre-requisite courses must be completed by the end of Spring 2015 semester, preceding Fall 2015 admission. Please meet with a Health Science Program Advisor by calling so that your academic plan reveals that you take the following 3 courses; DMIS 1110, DMIS 1105, and MATH 1120 last, after your other pre-requisite courses are completed. * Please see page 9 for listing of required minimum core courses. Complete the online Health Sciences Application and select Diagnostic Medical Imaging Sonography Degree when asked which Health Sciences program are you applying to? and Fall 2015 as your anticipated entry term. NOTE: You can only submit the application online and must pay a $50 non-refundable application fee by credit card. If you do not have a credit card, you can purchase a prepaid credit card from your local retailer. Be sure you have selected the correct entry term and program. Once your term is selected and application submitted, you may not change this without submitting a new application and paying another $50 non-refundable application fee. If you previously applied to the Diagnostic Medical Imaging Sonography Degree program, you will need to complete this step again and pay another $50 non-refundable fee to be considered for this admissions cycle. Take the PSB-Health Occupation Aptitude Exam (PSB-HOAE) in the College of DuPage Testing office prior to the application deadline. Visit the Testing Office webpage for information, test dates, and registration information or for sample questions. We look at the following 4 sections (out of the 8): Natural Sciences, Arithmetic, Verbal, and Reading. A score of 25% or higher must be obtained on the Verbal portion of the exam to be considered for admission. See below for scoring criteria. When applying to health science programs at COD, the PSB-HOAE exam can be taken up to a total of 2 times with at least a 30 day interval in-between the two exams. Any repeat exams are not accepted for period of at least 3 calendar years. 8 PSB-HOAE Score: 8 Total Units (4 sections are scored for the Sonography Program) Natural Sciences, Arithmetic, Verbal, and Reading Score > 75% = 2 points on merit evaluation Score 50-74% = 1 point on merit evaluation Score < 50% = 0 points on merit evaluation In summary, you should aim to get >75% in all 4 sections mentioned above. The percentages are located on the left hand column of your results form. A score of 25% or higher must be obtained on the Verbal portion of the exam to be considered for admission. Only scores from the most current exam will be used for the merit score. Provide documentation of experience working with patients in a healthcare setting (if you have relevant experience) or provide proof of experience working directly with others. Letter provided must be on company letterhead stating that a minimum of 20 hours were completed, within the last 3 years. (The Service Learning Placement project in DMIS 1110 does not count for this requirement). Submit to the Admissions and Outreach office, SSC Complete the College of DuPage Residency Verification form by providing 2 proofs of residency to the Office of Student Registration Services, Student Services Center (SSC) Room 2221 by the program application deadline date. Note: If 2 proofs of residency have not been submitted to Registration by the program application deadline date the applicant will not be considered for admission. Submit three (3) Recommendation Forms (see pages 12-17), to the Admissions and Outreach office, SSC 2207, from individuals who can attest to the applicant s work and/or academic ability. Forms completed by friends or relatives of the applicant are unacceptable. The enclosed forms must be used according to the instructions on the forms. 1. One form must be completed by one of the applicant s college-level instructors. 2. One form must be completed by an employer. 3. The remaining form may be completed by an additional college-level instructor, or an employer of the applicant. After the application deadline, you may be contacted for an interview. It is the applicant s responsibility to ensure that all material listed above has been completed and received by the Admissions & Outreach office, SSC 2207, no later than May 15 th preceding Fall 2015 admission. Admissions Process The AAS Sonography Degree Program can admit only a limited number of students. The number of students selected is determined by the current availability of clinical sites and may vary from year to year. An Admissions Committee, consisting of clinical site instructors and college faculty will oversee the selection process using a Merit Evaluation based upon admission requirements. Students are admitted to the program according to Merit Evaluation scores. Admission to the program is not a guarantee of completion of the program nor does it guarantee employment upon graduation. Priority for Admission: a. In-district applicants b. 9 c. Out-of-District applicants Letters of decision (accept, decline, alternate) are mailed to each applicant approximately 4-6 weeks following the application deadline. Accepted applicants must confirm their desire to accept in writing (on a provided form) within a two-week period. If no response is received, the next qualified student on the alternate list, according to Merit Evaluation rank order, will be offered acceptance into the program. GENERAL EDUCATION CORE The Sonography degree includes a minimum general education core curriculum that must be completed prior to the application deadline. The required cumulative college GPA is 2.75 on a 4.0 scale, with some courses requiring a 3.0 on a 4.0 scale. If A&P and Biomedical Terminology have not been completed in the last 5 years, they will need to be repeated prior to the application deadline.
