3 Meet our Speakers Nate Warren, MPH is OCHIN s Patient Engagement Coordinator, Nate strives to be a champion of the patient s needs, perspective and desired health care systems experience, as well as a translator of those needs to the OCHIN organization. This position is a creative resource to OCHIN members in best practices in patient engagement healthcare (research, governance structure, technological tools), and serves as a critical communication conduit for OCHIN, its members, and its partners in healthcare. Alex Harkins is a disabled Viet Nam Navy veteran who since September 2008 receives all medical care from the Portland VA Medical Center. He has been a user of the VA s patient portal My HealtheVet since then. Since August 2012 he volunteered, supporting other veterans using My HealtheVet and providers in using Secure Messaging. He also facilitates veterans signing up for Virtual Electronic Lifetime Record (VLER). Previous to the VA, he had experience in HIPAA s administration simplification transactions on both sides and planned, implemented, and managed installation and integration picture archiving and communications systems (PACS). He is a 1967 U S Naval Academy graduate and has completed post graduate studies in information technology, business, and management. Amber Zupancic-Albin, JD, BSN, RN is a Professional Liability and Claims Manager at Oregon Health and Science University. Up until just recently Amber was an attorney at Harrang Long Gary Rudnick, PC with an emphasis in healthcare compliance and regulatory matters, contracting, third-party liability subrogation claims, ERISA, litigation and general advice issues in health care law. Prior to joining HLGR, Amber owned her own medical legal consulting firm, AZA Consulting, LLC, having spent several years providing litigation support and high-exposure claims management services to large healthcare institutions and law firms. Also a Registered Nurse, Amber received her nursing degree from Johns Hopkins University School of Nursing, spending nearly 7 years serving as an RN in the Portland area and in California before studying Health Law at California Western School of Law in San Diego, receiving her J.D. in 2008.

4 JoLynn McCulloch, JD, RRT focuses on professional negligence defense, insurance defense and representation for professionals in licensing matters. Prior to joining HLGR, JoLynn worked as a critical care and flight team respiratory therapist. She enjoys integrating her medical knowledge with her legal practice to bring effective solutions to legal problems involving personal injury and healthcare issues. JoLynn is admitted to the Oregon State Bar, the U.S. District Court for the District of Oregon, and the Washington State Bar. She attended Washburn School of Law. Doug Wojcieszak is a disclosure training consultant who created Sorry Works! and has trained over 30,000 healthcare, insurance, and legal professionals. Wojcieszak is a sought-after speaker and trainer and the Sorry Works! content (books, booklets, on-line learning, etc) has become the go to references for organizations dealing with adverse medical events. Sorry Works! and Wojcieszak recently partnered with The Sullivan Group to provide on-line disclosure training content for healthcare professionals. Bethany A. Walmsley, CPHQ, CPPS has been the Executive Director for the Oregon Patient Safety Commission since December With nearly 20 years of healthcare experience, Bethany has led numerous patient safety and quality improvement efforts across the healthcare continuum, including integrated delivery systems, ambulatory healthcare facilities, and health plans. Before moving to Portland in 2010, Bethany was the Director of Quality and Patient Safety for Kaiser Permanente in Atlanta, GA. She is certified in both healthcare quality and patient safety, and has a passion for implementing the changes necessary to support a strong culture of patient safety to reduce preventable patient harm. Melissa Parkerton is Director for Early Discussion and Resolution. Prior to assuming this role, she directed the Commission s Quality Improvement Collaboratives, working with hospitals, dialysis facilities, and nursing homes around the state to improve care and safety. She has worked extensively in the areas of clinical improvement and health research, as well a working as a Child and Family Therapist in community mental health. Melissa holds a BA from Dartmouth College and an MA in Applied Behavioral Science from LIOS/Bastyr University.

5 William C. Bud Pierce MD/PhD is Co-chair for the Task Force on Resolution of Adverse Healthcare Incidents. He has been in private Hematology/Oncology practice in Salem Oregon for the past 20 years. He is board certified in internal medicine, medical hematology, and medical oncology. He is an Assistant Clinical Professor of Medicine at OHSU, and is a member of the Knight Cancer Center. Dr. Pierce is a Past President of the Marion Polk County Medical Society, and the Oregon Medical Association. Dr Pierce practices medicine 5 days a week, and sees preventable medical errors too often. Mark Bonanno, JD, MPH is General Counsel and Director of Health Policy at the Oregon Medical Association. Mark has both a law degree and a graduate degree in public health. He has worked in private practice for 20 years representing physicians and other health care businesses with a focus on both business and regulatory legal issues. He is active with the American Health Lawyers Association (AHLA), and in 2014 completed a three-year tenure as Chair of the AHLA Fraud and Abuse Practice Group that is made up of over 2,400 members nationwide who work in health care regulatory enforcement and compliance. Leilani Schweitzer did not choose a career in health care, it chose her. Eight years ago her son died after a series of medical mistakes, now she works at the same hospital where those errors happened. As a Patient Liaison for Stanford University Hospitals Risk Management, Leilani uses her own experience with medical errors to navigate between the often insular, litigious and administrative sides of medical error; and the emotional side of the patient and family experience. Her work at Stanford has given her a unique view of the importance and complex realities of disclosure and transparency in health care.
