3 WHAT WE DO LETTER FROM THE BOARD PRESIDENT Dear Friends, The Summit Community Care Clinic s continued success is a reflection of the good will and investment by our many supporters and partners in Summit County and across the state. Summit County is a truly amazing community that cares about all of its residents. Since 1993, the Summit County community has enabled the Care Clinic to provide comprehensive health care to those in greatest need. I am personally grateful to everyone who has supported the Care Clinic through the years. Midway through our 20th Anniversary year, the Care Clinic is experiencing growth in the number of patients, visits and diversity of services. Our client population ballooned from 298 patients in 1993 to nearly 6,000 patients and 19,430 client visits in Primary care services are up 40% in the last year and oral health visits have increased by 33% for the same period. We are especially pleased to see that our four school based clinic sites have experienced growth of 10% during the past year. Behavioral health, specialty services, and health education have also seen significant growth in service numbers. Among the Care Clinic s most important new services is our health navigation and coaching component. Bilingual health educators help assure that patients understand their problems, get proper medications and treatments, learn how to navigate community resources, including specialty care, and apply new healthy behaviors to their lifestyle. The health educators are directly responsible for helping patients improve specific health outcomes. The Affordable Care Act has brought expansion of Medicaid eligibility to a portion of the Care Clinic s most needy patient population. It is too early to realize the full impact of this new cadre of insured patients, but our experience to date is positive. The Care Clinic is hopeful that the coming year will bring a significant shift in the number of patients who can take advantage of Medicaid coverage. We have been alternating between being uninsured and having Medicaid since we moved here in We go to the Care Clinic for all our medical and dental needs. I was also referred to Mind Springs Health and the Family and Intercultural Resource Center through the Care Clinic.The survey given prior to being seen was instrumental in getting me to the appropriate resources because financial difficulty can be very emotionally trying. Also, the mental health professionals at the Care Clinic were empathic, concerned and encouraging. My medical provider has been intrinsic to helping me overcome emotional issues as well as physical issues as they are probably interrelated. I have suffered from migraines, digestive issues, and depression for many years. Without the Care Clinic, I would not be getting the treatment I need and would lack the emotional and physical well-being I now enjoy. Patient two three

4 Recent Accomplishments 2013 Accomplishments by the Numbers Unduplicated Patient Numbers El Pomar Outstanding Nonprofit Award In September, the Care Clinic was honored by the El Pomar Foundation with the 2013 Award for Excellence in Health and Recreation. The Awards for Excellence annually recognize the state s highest performing nonprofit organizations. El Pomar Chairman and CEO William J. Hybl shared at the awards presentation that Honorees this year are dynamic, creative organizations and leaders who set the pace for nonprofit performance throughout the state. In addition to a trophy, the Care Clinic received a $25,000 cash award. Expanded Prenatal Services In 2013, the Care Clinic began providing prenatal care, a service not previously available. Research shows that accessing timely prenatal care improves health outcomes for both the mother and the baby. Two of the most significant benefits of early prenatal care are improved birth weight and decreased risk of preterm delivery. The Care Clinic was able to raise the percentage of patients receiving prenatal care in the first trimester from 44% in 2012 to 82% by the end of Quality Assurance Program Enhancement The Care Clinic received a major three-year grant from the Health Resources Services Administration in August to enhance the Care Clinic s quality assurance efforts. The grant will allow the Care Clinic to hire a Quality Assurance Manager and a Data Specialist. These new positions will enable the Care Clinic to more effectively manage and utilize patient outcome data to improve the delivery of high quality care. The positions will also be responsible for tracking and reporting on a myriad of state and federal requirements that monitor the safety and quality of Care Clinic operations. Patient Visits per Discipline School Based Health Center The School Based Health Center continues to grow and serve students and families by offering comprehensive health services in two elementary schools and the Middle and High School. In 2013, the School Based Health Center provided 2,751 primary care, oral health and behavioral health visits to 1,432 students, teachers and family members. As a result of the dental screenings provided through the School Based Health Centers, students in Summit School District experienced a 4% reduction in the rate of tooth decay. The dental screening program has benefited greatly from the ongoing support of the Vail Resorts Echo Program. Integration of Health Records In the spring, the Care Clinic became one of the few Community Health Centers in the country that uses a fully integrated patient electronic health record. In the past, the dental program utilized a separate health record for their patients. Now, all patients are tracked using a single electronic health record system. This integration assures that physicians, dentists and behavioral health specialists are all viewing the same patient information so they can provide the highest level of care. four five

5 WHO ARE CARE CLINIC PATIENTS? Impact of Care Clinic Services on Patient Health Outcomes Patients by % of Poverty Level *100% of poverty for a family of four is $23,551 A single individual who earns less than $11,491 is below 100% of the poverty level. The Care Clinic monitors a total of 15 Clinical Quality Measures as required by Federal Health Center guidelines and as part of the facility s overall quality assurance program. Provided below are examples of measures where the Care Clinic has exceeded national benchmarks and a brief description of what the Care Clinic is doing to address the measure. Adult weight assessment and follow-up: The percentage of patients 18 or older who had their Body Mass Index (BMI) calculated and, if over or under weight had a follow-up plan documented. Description: Maintaining proper weight is vital to overall health of adults and children. At the Care Clinic each patient s weight is measured at all primary care appointments as part of basic vital signs. When vitals are entered into the patient s health record, a BMI is automatically computed. Based on the BMI score, the patient receives educational information from their provider, referral to additional resources, or support from a health educator to develop a weight management plan. Patients by Age Where SCCC Patients Live Hypertension: The percentage of hypertensive patients with last blood pressure reading less than 140/90. Description: It is well documented that elevated blood pressure can lead to a myriad of major health issues including heart attack and stroke. Care Clinic patients have their blood pressure checked at every primary care visit. For patients with hypertension, the care provider discusses the situation with the patient and develops a treatment plan that may include interventions such as medication, weight management, diet changes, health coaching, and self-monitoring. Cervical Cancer: The percentage of women years old up to date on cervical cancer screening. Description: It is recommended that all women between the ages of 16 and 64 receive a pap test for cervical cancer every three years unless there are special circumstances. Cervical cancer is very treatable when detected early. All female patients are encouraged to receive timely Pap test by Care Clinic providers. Reminders are sent out to patients about the importance of scheduling an exam and financial support is available to patients who cannot afford the procedure. six seven

7 FINANCIALS - Income and Expenses What Does the Affordable Care Act Mean to the Care Clinic? Approximately 30% of Care Clinic patients will be eligible for Medicaid due to changes in eligibility requirements. Since the Care Clinic provides a medical home to these patients, it is predicted that most of them will remain active patients at the Care Clinic. The challenge will be encouraging this group of patients to enroll in Medicaid. The Care Clinic has implemented several new strategies to assist with effective Medicaid enrollment. About 15% of Care Clinic patients will be eligible for a tax credit for coverage through the Health Insurance Marketplace. Due to the inflated prices of insurance premiums in resort communities, however, the Care Clinic predicts that very few of these patients will choose to purchase health insurance. Some of these patients may choose to remain uninsured and continue to utilize the Care Clinic s sliding fee schedule for their basic health needs. Approximately 40% of Care Clinic patients will remain uninsured, as they will not be eligible for Medicaid or a tax credit through the Marketplace. The Care Clinic will continue to subsidize services for these patients through grant writing and fundraising efforts Payer Mix 2015 Projected Payer Mix Congratulations to the Community Care Clinic. The full range of medical services and high quality integrated health care the Clinic provides are unique for a mountain community. A $100 donation pays for two dental screenings and cleanings for one child. A $600 donation pays for the entire cost the Care Clinic encounters when providing five sessions of behavioral therapy. A $1,200 donation pays for a diabetic education and treatment program for an adult patient for one year.
