2 A Message to Our Communities Robert A. Chrencik Donna L. Jacobs The University of Maryland Medical System (UMMS) delivers a special blend of clinical skill, innovation and caring in each area we serve. We share the research and specialized care for which our teaching hospital is known with our member hospitals, health centers, doctors and care teams, all of which are focused on the people we serve. With 12 hospitals in the Baltimore area and Central Maryland, we provide care for people in seven counties. This is a unique time in health care. Now, more than ever, the focus is on prevention and health education. In Maryland, we are very aware of this new focus. Maryland s new system to repay hospitals based on population health is a key change in how we deliver health care in our state. ABOUT THE UNIVERSITY OF MARYLAND MEDICAL SYSTEM The University of Maryland Medical System (UMMS) currently includes 12 health systems and hospitals across Maryland including the University of Maryland Baltimore Washington Medical Center; University of Maryland Charles Regional Medical Center; University of Maryland Medical Center; University of Maryland Medical Center Midtown Campus; University of Maryland Rehabilitation & Orthopaedic Institute; University of Maryland Shore Regional Health; University of Maryland St. Joseph Medical Center; and University of Maryland Upper Chesapeake Health System, as well as the freestanding University of Maryland Shore Emergency Center at Queenstown and the University of Maryland Shore Nursing & Rehabilitation Center in Chestertown. UMMS also operates Mt. Washington Pediatric Hospital as a joint venture with Johns Hopkins and also operates University of Maryland Medical System Health Plans for Medicare and Medicaid beneficiaries. The health of people in the areas we serve has always been a top priority. Our hospitals have worked for years with many local groups and people to address their area s most urgent health needs. This report features a few of the many ways UMMS hospitals made a difference in fiscal year Through our work, we have given more than $382 million in time and contributions to programs and services that benefit areas and people across Maryland. We were able to do this with a limited amount of funds and resources. We work hard every day to improve the health of the people we serve in Baltimore and across the state. 3 Vascular Screenings Save Lives University of Maryland Baltimore Washington Medical Center s (UM BWMC) Vascular Center offers a screening program to educate people on and detect major vascular problems. Those problems include abdominal aortic aneurysms, peripheral artery disease and carotid artery disease. UNIVERSITY OF MARYLAND BALTIMORE WASHINGTON MEDICAL CENTER We offer free vascular screenings to adults 50 years of age and older. This age group (especially men) is at higher risk and should be screened every five years. Other risk factors include high blood pressure, high cholesterol, smoking, diabetes, family history of vascular disease and prior stroke. Finding out if you have a vascular issue is done using the latest ultrasound and Doppler technology. Results are reviewed by a doctor or nurse right away. People leave with their results to share with their doctor. UM BWMC partners with local groups such as senior centers, places of worship and civic groups to host these events. In the past year, UM BWMC held five vascular screenings. A total of 269 people were screened, with 10 having results needing follow-up and treatment. UM BWMC offers other programs to address vascular disease risk factors. Examples include blood pressure screenings, how to reach a healthy weight by eating well and exercising, stop-smoking and diabetes classes. In these ways, UM BWMC saves lives as it prevents vascular disease and catches and treats it early. 4 Successful Blood Drive Community Partnerships Save Many Lives As another example of a fruitful community partnership, University of Maryland Charles Regional Medical Center (UM CRMC) has been teaming up with La Plata United Methodist Church and the La Plata American Legion to conduct regular blood drives for more than 10 years. UNIVERSITY OF MARYLAND CHARLES REGIONAL MEDICAL CENTER The partnership began during a hospital renovation when the church and the American Legion Post hosted the blood drive offsite, and they have continued to host and help staff the blood drives ever since. Hospital auxiliary volunteers also provide staffing support for the blood drive. In partnership with the La Plata United Methodist Church and American Legion Post #82, La Plata, the drives are volunteer-driven and self-sustaining, with a strong track record of successful collection. The hospital provides leadership, coordination, volunteers and publicizes the blood drive, in its magazine, website and in the newspaper. For fiscal year 2015, UM CRMC collected 453 units of blood, achieving a 78.4% participation rate compared to its target goal the highest participation rate among hospitals in the University of Maryland Medical System (UMMS). Those 453 units can save as many as 1,359 lives. Based on meeting collection benchmarks, the Red Cross offers a discount to the hospital system for purchasing blood products. Charles Regional Medical Center enjoys a very high participation rate for the drives, which speaks to the strong partnership the hospital has with the community. The increased collection of units allowed the system to meet its goal of 30% average participation saving $17 on every unit of blood purchased from the Red Cross. This cost saving allows all UMMS hospitals to ensure they can maintain an adequate lifesaving blood supply. PAGE 2 UNIVERSITY OF MARYLAND MEDICAL SYSTEM CHIR-FY2015-UMMS.indd 2

5 Wellness for Women Program Provides Breast Cancer Education and Screenings for At-Risk Women KEY PROGRAM FACTS: The Wellness for Women (WFW) program, offered through the University of Maryland Shore Regional Health Comprehensive Breast Center, provided 207 free annual mammogram screenings in fiscal year 2015 a 24% increase from previous years. WFW took part in 62 community outreach events, and provided speakers to nine professional organizations a total of 3,324 contacts. WFW provides interpretation and translation services to Latina women. UNIVERSITY OF MARYLAND SHORE REGIONAL HEALTH A program offered by the University of Maryland Shore Regional Health s Comprehensive Breast Center (CBC) assists uninsured and underinsured women with age- and risk-specific mammography screenings and clinical breast exams. Called Wellness for Women (WFW), the program serves younger women (20 to 39) identified as at-risk, women over 64, and Latina women of all ages. During fiscal year 2015, WFW provided 207 women in Caroline, Dorchester, Kent, Queen Anne s and Talbot counties with free annual mammograms. Educating women about breast cancer is a vital part of the program. WFW team members took part in 62 outreach events in fiscal year 2015, including health fairs, multicultural programs, college events and social services programs. Mattie Fountain, CBC community outreach worker, works primarily with African-American women. African-American women are often more reluctant to receive preventive care, says Fountain, adding that fear is the main reason. We build relationships in African-American communities by doing presentations on breast cancer awareness in churches and at community events. WFW partners with all five county health departments to ensure all age groups are covered and to assist women in determining their eligibility for services. WFW also receives referrals from health departments, gynecologists and former patients and senior centers, and partners with other breast cancer groups, such as the Survivors Offering Support program and the American Cancer Society s Road to Recovery program. 6 St. Clare Medical Outreach Program The St. Clare Medical Outreach Program, located in Lutherville and owned, operated and financed entirely by University of Maryland St. Joseph Medical Center (UM SJMC), has stayed true to its roots in Baltimore, where it began as a traveling medical outreach van for needy uninsured patients, then evolved into a primary care clinic within the Esperanza Center in Fells Point before relocating to Baltimore County. UNIVERSITY OF MARYLAND ST. JOSEPH MEDICAL CENTER This year, the UM SJMC s St. Clare Medical Outreach Program s patient access and visits really grew with the addition of medical director Joanna Saba, MD. Her presence has doubled the hours a doctor is available and has worked with nurse manager Mary Jo Huber, RN, to offer more services. The program s mission is to provide high-quality, comprehensive primary care, serving culturally diverse people who are the working uninsured with no other access to care. About 99% of patients are Hispanic. Good health is very important to our hardworking patients. We want to help them continue to be productive members of the community, said Dr. Saba. Patients come from Baltimore City and across Baltimore County. The program is located on a major bus route. The St. Clare staff, many of whom are native speakers of Spanish, include a nurse manager, registered nurses, certified nurse practitioners, physician assistant, phlebotomist, pharmacy liaison who arranges medications for patients, health care coach who provides nutritional and lifestyle counseling, office staff and volunteers. We provide culturally appropriate care and understand the stresses and financial challenges of this immigrant population, said Dr. Saba. St. Clare utilizes medical students from the University of Maryland School of Medicine, who are enrolled in a Spanish-language medical program for the chance to use their Spanish-speaking skills in a clinical setting. A key service St. Clare provides is to refer patients to specialty care as needed, ranging from surgeries to inpatient care. Thanks to the generous medical specialty community at UM SJMC who donate their time to uninsured St. Clare patients, we are expanding referrals for a wide variety of care, said Dr. Saba. 7 Acting Out to Stop Diabetes Actions speak louder than words! At the University of Maryland Medical Center (UMMC), we do more than just say: Make lifestyle changes so you don t get diabetes. We actually show members of our community how to stop diabetes by teaching them how to make healthy food choices and increase their physical activity. UNIVERSITY OF MARYLAND MEDICAL CENTER UNIVERSITY & MIDTOWN CAMPUSES Group classes are now held at UMMC s Midtown Campus through the National Diabetes Prevention Program. Our Diabetes Prevention Program is recognized and supported by the Centers for Disease Control and Prevention. The year-long program starts with four months of weekly classes for people who are screened and deemed as high-risk for diabetes. The classes cover many topics and vary from learning how to read food labels and understanding fat content to how to add 20 minutes of activity to the day and coping with stress. The group setting works well since the participants all have something in common, and they bond throughout the experience. They talk about their struggles to keep diabetes at bay. Patients have open dialogue about food choices, and they are learning to take charge of their own health. They are given food journals and activity trackers so they can write down what they eat and what they do. The journals are reviewed, and then instructors provide tips on how to do even better! Today, one in three American adults is at risk of developing Type 2 diabetes. Diabetes is a serious condition that can cause heart attack, stroke, blindness, kidney failure and loss of toes, feet or legs. Losing weight and starting moderate physical activity can help reduce the risk of developing diabetes. The participants are incentivized to continue the program. At 16 weeks, they become chefs as they get together for a cooking class, where they learn how to make a healthy whole-wheat pasta dish. The goal is that people decrease their weight by 7% at the end of the year. One woman was close to her goal by losing 22 pounds. She is quite vocal about how the classes taught her many things about keeping diabetes away! She also felt a great sense of accomplishment as did many others. 8 Adapted Sports Program The University of Maryland Rehabilitation & Orthopaedic Institute s (UM Rehab) Adapted Sports Program promotes involvement in adapted recreational and competitive sports for people with physical disabilities in order to improve independence, self-confidence, health and overall well-being. UNIVERSITY OF MARYLAND REHABILITATION & ORTHOPAEDIC INSTITUTE People in UM Rehab s Adapted Sports Program boost their physical strength, range of motion, flexibility and endurance as well as emotional well-being and overall quality of life. Just ask 28 year-old Randy Johnson. In 2013, Randy suffered a traumatic spine injury in a horrific car accident. He spent more than a month in therapy at the UM Rehab. A seasoned athlete of many sports, all Randy could think about was not competing again, or doing much of anything else for that matter. After a visit from recreational therapist Mike Henley, a trauma survivor and inspiration for many others, Randy began to embrace his new life. After leaving UM Rehab, Randy spent the next two years in a variety of settings, having to rely on others for his daily care. He lived with several family members, and ended up living in a nursing home for an entire year. In September 2014, Randy attended the annual Adapted Sports Festival at UM Rehab, where he was introduced to wheelchair (quad) rugby, and the Maryland Mayhem quad rugby team (UM Rehab s home team). He had found his new sport. Randy began working out in his chair and managed to find rides to rugby practice whenever he could. Through hard work and desire, he eventually earned a starting position on the Maryland Mayhem team. Wheelchair Rugby has given Randy a new lease on life. Through UM Rehab s Adapted Sports Program, and his desire to compete, Randy has achieved tremendous strength and independence. In fact, he recently moved into his own apartment, where his Mayhem teammates surprised him with a house-warming party. Now Randy is living independently and making the most of every day; his next goal is to make the USA Wheelchair (Quad) Rugby team. 9 Community Outreach: Health Screenings, Flu Shots and More Disease prevention, health promotion and wellness efforts can have a big impact on people at risk for health problems. These efforts include health screenings, education and flu vaccine clinics. UM Upper Chesapeake Health s (UM UCH) Community Outreach Department offers many such programs free of charge or for a small fee to improve the health of people in northeast Maryland. UNIVERSITY OF MARYLAND UPPER CHESAPEAKE HEALTH Health Screenings UM UCH provided 5,294 medical screenings in fiscal year 2015 to area residents, with a focus on seniors and the underserved. UM UCH also offered educational programs and clinics within its medical mobile van at locations throughout the county and at health fairs. These programs help people with limited or no access to health care learn about their risk factors. Regular screenings can detect health problems early, when they can be more easily treated. They may also prevent heath issues from becoming more serious problems. Health screenings include: Blood pressure Body fat analysis Cardiac risk assessment Cholesterol Colorectal cancer Diabetic foot and eye Head and neck cancer Hearing Hemoglobin A1c (HbA1c) Osteoporosis Senior fall risk Skin cancer Sleep disorder Stroke risk assessment Vision Flu Vaccine Clinics Influenza is a serious illness that can lead to time in the hospital and sometimes even death. Flu seasons vary from year to year, and even healthy people can get sick from the flu and can spread flu germs to others. In fiscal year 2015, the UM UCH provided 1,345 free or low-cost flu vaccines to area residents who otherwise would not have been vaccinated. Our clinics took place throughout the county to reach people who are often isolated and may have limited access to health care. This includes the homeless, uninsured, senior citizens and those people without a primary care provider. PAGE COMMUNITY HEALTH IMPROVEMENT REPORT CHIR-FY2015-UMMS.indd 7

10 Healthy Living Academy and Jump Start Help Prevent and Treat Childhood Obesity Mt. Washington Pediatric Hospital s (MWPH) Healthy Living Academy (HLA) is a wellness program for kids in kindergarten through the third grade at the Mount Washington School. MT. WASHINGTON PEDIATRIC HOSPITAL HLA uses a health program called the OrganWise Guys, which uses items shaped like organs of the body to teach that lifestyle choices can have good and bad effects on the body. The project s goals are 1) to increase healthy behaviors in kids and decrease the risk of obesity and related illnesses, and 2) to increase MWPH s exposure and role in helping kids lead healthier lives. A total of 267 students went to HLA sessions. These classes were taught by the psychologist, a psychology post-doctoral student, physical therapy assistant and dietitian who work within MWPH s Weigh Smart program. Jump Start In 2015, MWPH s groundbreaking Weigh Smart program added a new program designed to get very obese kids and their families in for treatment as soon as possible. The Weigh Smart Jump Start program runs for four weeks and offers both education and group exercise sessions for kids and their families. Jump Start s goal is to get families in quickly for an assessment and then offer other services tailored to each family s needs. In 2009, MWPH added Weigh Smart Jr., a program targeting kids from ages 2 through 7. The typical Weigh Smart Jr. patient is 5 years old and weighs about 100 pounds, says Michelle Demeule-Hayes, Weigh Smart Program Director. We need to treat children earlier than ever before. The City Health Department released data on a random sample of 2,143 students assessed for body-mass index (BMI). The major findings: 26% of kids were obese, as were 31% of pre-kindergarten and kindergarten students. In fiscal year 2015, MWPH clinical staff evaluated 134 new patients, and saw 133 patients for follow-up visits. PAGE 8 UNIVERSITY OF MARYLAND MEDICAL SYSTEM CHIR-FY2015-UMMS.indd 8
