1 Speakers David Chi, MD Dr. David Chi completed his Otolaryngology Residency at the University of Virginia and his Pediatric Otolaryngology fellowship at Children s Hospital of Pittsburgh of UPMC. Dr. Chi is Associate Professor of Otolaryngology at the University of Pittsburgh School of Medicine and the Clinical Director of the Hearing Center and Cochlear Implant Program at the Children s Hospital of Pittsburgh of UPMC. Umamaheswar Duvvuri, MD, PhD, FACS Dr. Umamaheswar Duvvuri, assistant professor of otolaryngology at thedepartment of Otolaryngology University of Pittsburgh School of Medicine, specializes in head and neck oncology. Dr. Duvvuri completed his undergraduate education in Bioengineering at the University of Pennsylvania, and remained there to obtain his MD and PhD degrees. He completed his residency training in otolaryngology at the University of Pittsburgh Medical Center, then trained in advanced head and neck and endocrine oncologic surgery at the MD Anderson Cancer Center in Houston. Dr. Duvvuri has special interests in minimally invasive endoscopic and robotic surgery of the head and neck; tumors of the thyroid and parathyroid glands; and molecular oncology. He also has an active research program investigating the role of calcium-activated chloride channels in head and neck cancer. Jeffrey Fichera, PhD, MS, PA-C Jeff Fichera is a PA with the Ear, Nose, and Throat Surgical Associates in Winter Park, Fla., where he has practiced since He received his PA degree from Alderson Broaddus College in West Virginia. Upon graduation he practiced rural medicine for several years prior to returning back to Florida in pursuit of Otolaryngology as a specialty. He is an adjunct instructor at the University of Central Florida since 2005 where he is an instructor of head and neck anatomy and physiology. His PhD is in Epidemiology. Fichera practices with a large 10 physician ENT group where he practices general otolaryngology and head and neck surgery with a special interest in Trans Oral Robotic Surgery (TORS) as well as otitis media in children. In 2012 he was invited by the AAPA to represent AAPA as a panel member with the AAO-HNSF developing the 2013 AAO-HNS Guidelines for Tympanostomy Tubes in Children accepted for publication and presented at the 2013 annual AAO-HNS meeting. Marie Gilbert, PA-C, DFAAPA Marie Gilbert has been a PA for 33 years, all but two spent in Otolaryngology. She has worked in private practice as a PA and as an office administrator. She has been a preceptor and lecturer for PAs as well as NPs.

2 Gilbert has been a SPAO member since its inception, and has served in every SPAO board position. She is currently SPAO s and AAPA s Liaison to AAO-HNSF, and serves on AAO s General Otolaryngology Education Committee and Physician Resource Committee. Ray Hinerman, DDS, MD Dr. Ray Hinerman is practicing ENT in Wheeling, W.V. after having previously worked at West Virginia University and in private practice in Abingdon, Va. Dr. Hinerman received both his DDS and MD degrees from WVU, and also completed his residency at WVU. Ashutosh Kacker, MD Weill Medical College of Cornell University Department of Otorhinolaryngology Medical Director, ENT for the PA-C Annual conference 2011, 2012, 2013 Dr. Ashutosh Kacker completed all of his schooling in India, and all of his post-graduate training in New York. He is an Associate Professor in the Weill Cornell Medical School system, training physicians and PAs, seeing patients within the Department of Otolaryngology. He conducts research here, is well-published, and is on staff at three other hospitals in New York. Dr. Kacker has received many teaching awards, participated in humanitarian missions around the world, and serves on several committees of the AAO-HNSF. It was in his service to the General Education Committee that he started his association with this meeting, and has been a tireless and staunch supporter of PAs in Otolaryngology. J. Seth McAfee, MD Seth McAfee is the current neuotology fellow at the University of Pittsburgh Medical Center, under the direction of Barry Hirsch. He attended medical school at Wright State University in Dayton, Ohio, and completed his otolaryngology residency at the Eastern Virginia Medical School in Norfolk, Va. Andrew McCall, MD Dr. Andrew McCall is an Assistant Professor in the Department of Otolaryngology at the University of Pittsburgh Medical Center. Dr. McCall received his medical degree and residency training in Otolaryngology Head and Neck Surgery at the University of California, Los Angeles. He then completed a two-year Fellowship in Otology/Neurotology at the Massachusetts Eye and Ear Infirmary, Harvard Medical School. Dr. McCall s clinical expertise is the diagnosis and management of otologic and neurotologic conditions. His scientific interest is focused on investigating multisensory processing within central vestibular pathways with the goal of translating these scientific findings into clinical therapeutic strategies.

3 Jose Mercado, MMS, PA-C José C. Mercado is a PA with Dr. Scott Goldberg and South Florida ENT Associates in Miami, Fla., since After serving in the U.S. Army as a medical specialist and flight medic for eight years, he went on to complete the PA Program at Miami-Dade College s Medical Center (MDC) and earned a Master of Medical Science degree from Nova Southeastern University (NSU). In addition to his training in family medicine, he completed an internship in Adult ENT/Head & Neck Surgery at the Veterans Affairs Medical Center in Miami, as well as a Preceptorship in Pediatric ENT at Miami Children s Hospital. Currently, he serves as an Adjunct Associate Professor and Clinical Preceptor in Miami-Dade College s Physician Assistant Program, and at Barry University School of Graduate Medical Sciences, Physician Assistant Program. He is a guest Lecturer at Nova Southeastern University s Physician Assistant Program. He is a Fellow of SPAO-HNS and a Past-President. He is on the Speaker s Panel for Daiichi Sankyo Pharmaceuticals and Alcon Laboratories. His special interests include sleep medicine and PA education. His hobbies include competitive saber fencing and traveling. Debra Munsell, DHSc, PA-C, DFAAPA Dr. Munsell is an Associate Professor and the Inaugural Program Director of the Louisiana State University Health Sciences Center - New Orleans Physician Assistant Program. She has over 30 years experience as a clinically practicing PA. She practiced as a member of the Department of Head and Neck Surgical Oncology at the University of Texas M.D. Anderson Cancer Center for many years, as well as clinically practicing in the Department of Otorhinolaryngology at the University of Texas Medical Branch-Galveston. She received her doctoral degree in Health Care Science in Health Policy in She is a founding member of the Society of Physician Assistants in Otorhinolaryngology-Head and Neck Surgery, Past President and Board Member. She has served the AAPA and the PAEA in several roles and continues to serve in local and state PA organizations. Grant Needham, PA-C Grant Needham graduated from the University of Utah PA program in 2000 and has been in ENT/HNS for the past 13 years. He started his career in Salt Lake City, spent a year in at Mayo Clinic in Scottsdale, Ariz., and has now been in Saint George, Utah, for the past 10 years. He works for Southern Utah Ear, Nose and Throat and Facial Plastics in St. George, about 90 miles north of Las Vegas. He s also worked part time in a pediatric urgent care facility for the past 10 years. The ENT office includes general ENT, Otology, Head and Neck cancer, and facial plastics. He works with three ENT surgeons. Needham s job description includes time in the clinic, primary first assisting, and hospital rounds. He s currently serving as treasurer for SPAO and has been a member since Needham has been married to Christina, his wife, for 25 years and has four children (Three boys and one girl) ranging from 25 to 20.

4 Ryan J. Soose, MD Dr. Ryan Soose is the Director of the UPMC Division of Sleep Surgery and Assistant Professor of Otolaryngology at the University of Pittsburgh School of Medicine. He received his undergraduate degree from the University of Notre Dame and his medical degree from the University of Pittsburgh School of Medicine. He completed a residency in otolaryngology head and neck surgery at UPMC, and fellowship training in sleep medicine at the UPMC Sleep Medicine Center. Board-certified in both otolaryngology and sleep medicine, Dr. Soose specializes in the medical and surgical treatment of snoring and obstructive sleep apnea. He is an appointed member of the Sleep Disorders Committee of the American Academy of Otolaryngology Head & Neck Surgery (AAO-HNS), and is an active member of the American Academy of Sleep Medicine (AASM) and the International Surgical Sleep Society (ISSS). Dr. Soose has a special interest in druginduced sleep endoscopy in the evaluation and management of patients with sleep apnea. Miguel (Mike) Valdez, PA-C Miguel V. Valdez MPAS, PA-C, received his PA training in the United States Air Force with a baccalaureate degree through the University of Oklahoma graduating in Following his baccalaureate degree, Valdez completed an Otolaryngology Head and Neck Surgery Residency for Physician Assistants in 1983 through a combined program USAF/University California-Davis and then completed a Masters of Physician Assistant Studies through the University of Nebraska in Valdez has held teaching positions with Wright State University School of Medicine, Dayton, Ohio, Lake Erie College of Osteopathic Medicine, Erie, PA, and most recently Greater Lawrence Family Health Center, Family Practice Residency Program, Lawrence, MA. After retiring from the USAF, he now resides in MA, working at Andover ENT Center, North Andover, MA. Eric W. Wang, MD Dr. Eric W. Wang is Assistant Professor of Otolaryngology at the University of Pittsburgh School of Medicine. Dr. Wang completed his undergraduate studies in biochemistry at Rice University and his medical degree from Baylor College of Medicine in Houston. He completed his internship and residency at Barnes-Jewish Hospital and Washington University School of Medicine in Saint Louis. During his residency, Dr. Wang completed a twoyear NIH sponsored research fellowship studying chronic infections of the head and neck. Dr. Wang then completed a one-year fellowship in Rhinology and Endoscopic Skull Base Surgery at the Department of Otolaryngology- Head & Neck Surgery, Medical University of South Carolina. Dr. Wang is a diplomate of the American Board of Otolaryngology. He is also a member of the American Academy of Otolaryngology Head & Neck Surgery (AAO-HNS), the American Rhinological Society, and the

5 North American Skull Base Society. Dr. Wang has published several peerreviewed journal articles, presented multiple national podium lectures, and participated as course faculty in several endoscopic courses. He serves on the Centralized Otolaryngology Research Effort Study Section and the Skull Base Surgery Committee of the AAO-HNS. He has received several honors and awards including an Honor Award at the 2011 Annual Meeting of the American Academy of Otolaryngology Head & Neck Surgery, and is listed amongst the Best Doctors in America. Dr. Wang specializes in rhinology and endoscopic anterior skull base surgery. His areas of focus include revision endoscopic sinus surgery, orbital decompression, repair of CSF leaks, and management of sinonasal and anterior cranial fossa tumors. His current research interests include outcomes and quality of life assessment in patients undergoing endoscopic skull base resections.
