PROMOTION DESCRIPTION AND ENTRY PERIOD: Through the Donate Life Promotion, people who complete designated activities using the Allegheny Health Network Rafflecopter application on the website or Allegheny Health Network Facebook page between April 9, 2015, at 12:01am and May 1, 2015, at 12:01am Eastern Standard Time are eligible to enter the promotion for a chance to win one (1) grand prize pack including one (1) signed Neil Walker Pirates jersey and four (4) tickets to a Pittsburgh Pirate Game of Allegheny Health Network s choosing or to win (1) of the weekly prizes that includes (4) tickets to a Pittsburgh Pirates game of Allegheny Health Network s choosing. A total of three (3) weekly prize packs and one (1) grand prize will be given away for this promotion. All entries must be submitted using the Allegheny Health Network Rafflecopter application between April 9, 2015, at 12:01am and May 1, 2015, at 12:01am Eastern Standard Time. All rules of Rafflecopter, Facebook, Twitter, and Instagram apply. The three (3) weekly prize winning entries will be selected at random at 09:00 am Eastern Standard Time on Wednesdays during the contest period. One (1) winning entry will be selected on each of the following Wednesdays, April 15, 2015, April 22, 2015 and April 29, The one (1) grand prize winning entry will be elected at random at 09:00 am Eastern Standard Time on Friday, May 1, Winners will be notified via the entry method they used in the Rafflecopter application. 2 ELIGIBILITY: There is a limit of one Rafflecopter account entry per person. The promotion is open to anyone age 18 and older. Employees, officers, agents and directors of Allegheny Health Network and each of their parent, subsidiary companies, affiliates, divisions, franchisees, representatives, consultants, subcontractors, suppliers, distributors, legal counsel, advertising, public relations, promotional, fulfillment and marketing agencies involved in the administration of this promotion, and their immediate family members (defined for these purposes as including spouse, parents, children, and siblings and each of their respective spouses) are not eligible to participate. All entries must be submitted and visible on Rafflecopter by 12:01am on May 1, No entries will be accepted after 12:01am. on May 1, Each entry means that the entrant acknowledges that he or she has read, understands, and agrees to these Official Rules. Limit: Each participant may obtain only one (1) Rafflecopter account entry into the Sweepstakes. Multiple entrants are not permitted to share an , Twitter, Instagram or Facebook account. Any attempt by any entrant to obtain more than the stated number of entries by using multiple/different , Twitter, Instagram or Facebook accounts, identities, registrations and logins, or any other methods will void that entrant's entries and that entrant may be disqualified from the entire promotion. Use of any automated system to participate is prohibited and will result in disqualification. In the event of a dispute as to any registration, the authorized account holder of the account used to register will be deemed to be the entrant or player. The "authorized account holder" is the natural person assigned an address by an Internet access provider, online service provider or other organization responsible for assigning addresses for the domain associated with the submitted address. The potential winner may be required to show proof of being the authorized account holder of the Twitter account associated with the winning entry. Any obscene or inappropriate photos that are submitted will be immediately removed and the account that posted them will be disqualified.

3 WINNER SELECTION AND NOTIFICATION: One (1) grand prize winner and three (3) weekly prize winners will be selected by Allegheny Health Network at random from all entrants. The grand prize winner will be selected at 09:00am Eastern Standard Time on Friday, May 1, One (1) weekly prize winner will be selected at 09:00am Eastern Standard Time on Wednesday, April 15, 2015, Wednesday, April 22, 2015 and Wednesday April 29, The selection of the winning entries will be conducted by Allegheny Health Network. AN ENTRANT IS NOT A WINNER OF ANY PRIZE, EVEN IF THE PROMOTION SHOULD SO INDICATE, UNLESS AND UNTIL ENTRANT'S ELIGIBILITY AND COMPLIANCE WITH THE OFFICIAL RULES HAVE BEEN VERIFIED AND ENTRANT HAS BEEN NOTIFIED THAT VERIFICATION IS COMPLETE. The four (4) winning entries will be notified by Allegheny Health Network through contact information the entrant provided in the Rafflecopter application during the promotion following the random selection of the winners, and the winners must respond to confirm their real name and claim their prize within five (5) business days or an alternative winner will be selected. They will be instructed that they can claim their prize with their photo ID. If winners live outside of the Pittsburgh area, the prize will be mailed to them. Allegheny Health Network is not responsible for, and shall not be liable for, (1) late, lost or misdirected online entries or (2) Allegheny Health Network's unsuccessful efforts to reach potential winners. If any of the selected potential winners (1) cannot be contacted or do not respond to direct messages or tweets after the winners are selected or (2) are ineligible because of failure to abide by these official rules, or any other reason, the prize may be forfeited and alternate winner(s) may be selected by Allegheny Health Network. PRIZE/ODDS: Winner will receive one (1) prize pack. Approximate retail value of the weekly prize is $88. Approximate retail value of the grand prize is $150. Odds of winning depend on the number of eligible entries received by the deadline. GENERAL PRIZE CONDITIONS: No cash or other prize substitution is permitted except at Allegheny Health Network 's discretion. The prizes are nontransferable. Any and all prize-related expenses, including without limitation any and all federal, state, and/or local taxes, shall be the sole responsibility of the winners. Potential winners will be required to execute a document that sets forth a Publicity and Liability Release (the "Prize Claim and Release Form"). If a potential winner fails or refuses to show photo ID and/or sign and return the Prize Claim Release Form document when they claim their prize, the potential winner may be disqualified and an alternate potential winner may be selected. GENERAL RELEASE: By entering, each participant agrees (a) to be bound by these Official Rules and the decisions of Allegheny Health Network, which shall be

4 final, and (b) that Allegheny Health Network and each of their respective agents, subcontractors, representatives, affiliates, subsidiaries, officers, directors, employees, legal advisors and advertising, promotion and fulfillment agencies are not responsible for and shall not be liable for: (1) late, lost, misdirected, or incomplete entries; (2) any condition caused by events beyond the control of Allegheny Health Network that may cause the promotion to be disrupted or corrupted; (3) any injuries, losses, or damages of any kind arising in connection with or as a result of the prize, or acceptance, possession, or use or misuse of the prize, or from participation in the promotion, including without limitation personal injury, death, property damage, and claims based on publicity rights, defamation, or invasion of privacy; (4) any typographical errors in any materials associated with the promotion; or (5) technical, network, telephone, computer hardware or software malfunctions of any kind, or inaccurate transmission of, or failure to receive any entry information on account of technical problems or failures of any wireless or Internet service providers, traffic congestion on the Internet or at any web site or any combination thereof, which may limit a person s ability to participate in a sweepstakes or receive communication from Allegheny Health Network. Allegheny Health Network reserves the right, in its sole discretion, to modify, suspend, or cancel the promotion at any time if it is not capable of running as planned, including if a problem corrupts the administration, security, or proper play of the promotion. If the promotion is canceled for any reason, Allegheny Health Network will award the applicable prize from among all eligible entries received as of the time of cancellation in a random drawing. Allegheny Health Network reserves the right at its sole discretion to disqualify any individual it finds to be attempting to defraud or in any way tamper with the promotion. By submitting an entry for the promotion, each participant (a) agrees to a complete release of Facebook, Twitter, Instagram, Rafflecopter, Allegheny Health Network and each of their respective agents, representatives, affiliates, subsidiaries, officers, employees, directors, legal advisors or agents with respect to any claim, suit, loss, injury or damages (directly or indirectly) arising from or in connection with the promotion or the prize awarded through the promotion and (b) acknowledges and agrees that the promotion is in no way sponsored, endorsed or administered by, or associated with, Facebook, Twitter or Instagram. GOVERNING LAW/JURISDICTION: All issues and questions concerning the construction, validity, interpretation and enforceability of these official rules or the rights and obligations of entrants or sponsor in connection with the promotion shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania.
