2 At Sky Lakes, we look at cancer not as something for an individual to face, but rather as a problem that medical professionals, families and friends, working in concert and with patients, can solve. A central part of the Sky Lakes Medical Center s Oncology Program is the Sky Lakes Cancer Treatment Center, which is accredited by the Commission on Cancer, putting it in the top 1 percent of cancer treatment centers in the nation. It also is internationally accredited by Det Norske Veritas (DNV). Paul Stewart Chief Executive Officer The program, however, is much more than that. People throughout the Sky Lakes family in Pathology, Lab Services, Surgery, Nursing, Rehabilitation Services, Dietary, Infusion Services, Spiritual Care, Pharmacy, Resource Management, Quality Improvement, the Cancer Registry, support groups for patients and families, and more as well as primary care physicians, surgeons and other healthcare professionals in the community coordinate their work to help patients and their families. Receiving a diagnosis of cancer is a life-changing event. Treatment may include radiation therapy, using our state-of-the-art linear accelerator and related technology. And it may include medication therapy or surgery in the medical center. Regardless, our Oncology Program ensures there are resources for support, follow-up services, financial counseling, education and more to help the people we serve along their journey. We are fortunate to live in a small community, but when it comes to diagnosing and treating cancer in Klamath Falls, there s no such thing as small-town medicine.

The Sky Lakes Oncology Committee is the designated multidisciplinary team responsible for the administrative oversight, development, and review of cancer-care services at Sky Lakes Medical Center. As a cohesive team, we continually strive to make our oncology program better for those we serve, says radiation oncologist Bradley Kramer, M.D., who served as committee chair for the past two years. The Committee provides leadership to plan, initiate, stimulate, and assess the institution s cancer-related activities. Bradley A. Kramer, MD Committee Chair The committee s activities and recommendations directly impact various programs and patient care services. Our team includes board-certified physicians from surgery, medical oncology, radiation oncology, diagnostic radiology, and pathology, along with a cancer liaison physician, clinical research manager, a specialist in pain control and palliative care, and representatives from hospital administration, nursing, social services, cancer registry, and quality assurance, says Dr. Kramer. We also have a representative from the American Cancer Society. During its quarterly meetings, the committee addresses topics including oncology leadership and quality service, education, community outreach, and relationships with other medical providers. Under the leadership of the committee, the Sky Lakes Oncology Program enjoys national accreditation from the American College of Surgeon s Commission on Cancer as a Community Cancer Program. Oncology care is complex and continuously evolving. Our multidisciplinary team has the knowledge, experience, equipment and collaborative spirit to provide the very best care to our patients afflicted with cancer.

4 It is with great pride that I present the 2015 edition of Sky Lakes Oncology Program s annual report. This report is a summary of the program s activities in calendar year 2014, and includes other information I hope you find useful. Sky Lakes Cancer Treatment Center is an important part of our local Oncology Program. While I am new to the Cancer Treatment Center family, I ve been a Sky Lakes Medical Center nurse in various management capacities for more than 20 years. Susan Morton Director, Sky Lakes Cancer Treatment Center All of us associated with the Oncology Program recognize that what we do affects not only patients but also their families. That s why we continually strive toward goals that exceed the national benchmarks. The Sky Lakes Oncology Program is accredited by the American College of Surgeon s (ACOS) Commission on Cancer (CoC), which demonstrates our commitment to providing patients with access to the services they need. I am proud of the high-quality care our program delivers, and I am excited by the future as we challenge ourselves to do more in behalf of our patients. Thank you for giving us the opportunity to serve your needs. Susan Morton, RN Director, Sky Lakes Cancer Treatment Center

Cancer Treatment Center Sky Lakes Cancer Treatment Center was founded in 1990 to provide Klamath Basin residents with the cancer care that once required travel far from home. It has expanded twice once in 1994 and again in 2003 to better serve the community. In addition to providing high-quality cancer care, the Cancer Treatment Center team also provide patients and those who love them cancer education, screening and early detection information, and an assortment of support services in an environment filled with optimism, hope and understanding. The Cancer Treatment Center family includes three physicians, a nurse practitioner, six registered nurses, five administrative personnel, and 21 support staff. Cancer Treatment Center caregivers routinely treat blood disorders, and provide chemotherapy, and radiation therapies. More than 3,700 people were treated in the decade ending in Richard Johnson, D.O. A board-eligible hematologist/oncologist with extensive training in the psychosocial aspect of medicine, Dr. Johnson joined Sky Lakes Cancer Treatment Center in mid-2015 after completing a fellowship at the University of Missouri, Kansas City. Nasser Abu-Erreish, M.D Dr. Abu-Errish joined the Sky Lakes Cancer Treatment Center team in January 2010 following three years of internal medicine and hospitalist experience, and a medical oncology fellowship at Eastern Tennessee State University. Dr. Abu-Erreish is board certified in internal medicine with a subspecialty of medical oncology.

4 Commission on Cancer The Sky Lakes Oncology Program is accredited by the American College of Surgeons Commission on Cancer, which recognizes the program for its voluntary commitment to providing high-quality patient-centered care. The Commission on Cancer is a consortium of professional organizations dedicated to improving survival and quality of life of patients with cancer through standards setting, prevention, research, education, and the monitoring of comprehensive care. The Commission challenges cancer programs to enhance the care they provide by addressing patient-centered needs and measuring the quality of care they deliver against national standards. The accreditation ensures there are continuous quality improvements in the care of our cancer patients: Receive comprehensive, patient-centered care through a multidisciplinary team approach; Have access to information on clinical trials and new treatment options; Get ongoing monitoring of care and follow-up Are provided with pycho-social support and survivorship care. Accreditations The services delivered by the skilled professionals in the Sky Lakes Diagnostic Imaging Department are an important part of the local Oncology Program. By providing accurate and up-to-date scans, the department helps patients receive more efficient treatment. The department s Mammography and CT services are certified by the American College of Radiology. This means the Sky Lakes facilities regularly go through a rigorous review process to be certain our equipment meet strict criteria for quality control, image quality, and safety protocols. Further, accreditation means our technologists are well-qualified, through education and certification, and meet or exceed national standards. The medical center s new MRI unit was installed in early 2015 and the accreditation process is in process. Sky Lakes Diagnostic Imaging also provides PET-CT services using a mobile unit that visits the hospital every week.

5 Cancer Registry Among the behind-the-scenes departments vital to the Sky Lakes Oncology Program is the Cancer Registry, which helps patients by adding to the global body of knowledge researchers use to find more effective ways to treat cancer. Sky Lakes Cancer Registry staff collect cancer-related data from patients who are either diagnosed or receive their first treatment here. The Oregon State Cancer Registry collects that data along with that of other hospitals in the state. Oregon is unique in that state law requires the Oregon State Cancer Registry to inform patients they are included in the cancer database. This gives Oregon patients the opportunity to participate in special studies that can benefit them and future generations. The staff in the Sky Lakes office are Certified Tumor Registrars following training in specialized cancer education, medical terminology, anatomy and physiology, characteristics of cancer and treatments, statistics, and knowledge of the regulatory rules set by national cancer organizations. They are members of the Oregon Cancer Registrars Association.

Community Outreach Sky Lakes Medical Center and Sky Lakes Cancer Treatment Center actively promotes awareness, prevention, and early detection of cancer. At the annual Sky Lakes Living Well Community Health Fair and at the Oregon Tech Wellness Fair, Cancer Treatment Center staff provided educational materials regarding skin cancer and used a sun scope demonstration to help people detect sun-damaged spots on their faces. They distributed nearly 1,300 samples of sunscreen lotion in the two events. A May event provided cancer awareness information to more than 500 people in rural Klamath County. To support low-income and uninsured women battle against breast cancer, Sky Lakes provides no-charge mammograms to qualifying women who would otherwise not have access to these services. In the two decades since it began, the program has provided mammograms or ultrasound breast exams to some 1,800 women, although the Affordable Care Act has considerably reduced the number of qualifying women. Funding began with a $7,575 grant from the Susan G. Komen Foundation. The Sky Lakes Cancer Advisory Committee has continued the program with contributions of more than $150,000. The funds are also used to purchase special gowns for all women receiving mammograms at the medical center or at Sky Lakes Outpatient Imaging. The Sky Lakes Early Detection multimedia campaign includes an assortment of newspaper and magazine advertising as well as news and feature stories. The campaign reaches more than 16,000 local households.
