7 Two Tracks Providers who currently meet OOD s Community Rehabilitation Program Standards, OAC Providers who are currently DODD certified but are not accredited and will need to apply for a waiver of OOD s accreditation rule

8 Providers who meet the current Community Rehabilitation Program Standards, OAC , may download a Provider Agreement form from OOD s website, the Provider Agreement form is located in the lower right corner

9 Providers complete the Provider Agreement and a W-9 (You can obtain a W-9 form from the IRS website, under forms.) and submit it to crpvendor@ood.ohio.gov for review using Subject: Employment First. OOD reserves thirty (30) days to review, approve, and post Provider Agreements to OOD s website.

10 If there are questions or issues with the Provider Agreement or W-9, OOD will contact the Provider for additional information and/or clarification.

11 Providers who are not accredited may apply for a Waiver. Providers who are requesting a waiver should request a copy of the form from crpvendor@ood.ohio.gov, please use the Subject: OOD Waiver Providers should also download and complete a Provider Agreement and W-9.

12 Providers should complete and submit the Waiver, Provider Agreement, and W-9 at the same time to using Subject: Employment First RSC will review the information and make a decision and notify the Provider within 30 days. First set of waivers should be submitted by October 15, 2013.

13 Providers who are granted a Waiver will also have some additional requirements Providers granted an accreditation waiver will only be allowed to service Consumers involved in Employment First. If the Provider wants to offer services to other Consumers they will need to become accredited according to OAC

14 OOD will conduct monthly random case reviews to ensure quality of services If potential concerns are identified, OOD will work with the Provider on an action plan to bring services up to standards Providers that are unable to improve service delivery will be removed from the Provider pool

15 OOD Fee Schedule VR has set rates for typical services. You may find the current rates as part of the VR Provider Manual located on OOD s website:

16 OOD Fee Schedule The Provider Manual includes the rates, definitions, and requirements of services on the VR Fee Schedule. Providers should review the information. If providers have specific questions they should send them to OOD will have specific Provider trainings that will include review of the Fee Schedule and billing processes. Training will be mandatory for new VR Providers.

20 If Providers have other community based/integrated services that they would like to offer Consumers they can include it in the Addendum section of the Provider Agreement and OOD will review them.

21 Counselor prepares authorization based on Individualized Plan for Employment (IPE) Provider should not provide any services without a written authorization Provider must submit report and invoice to OOD within ninety (90) days of last date of service or last date of the authorization cycle, whichever occurs first OOD makes every attempt to process payments within thirty (30) days
