The J Shaped Effect of Catfee Consumption on the Risk of Developing Acute Coronary Syndromes: The CARDI02000 Case-Control Study. J Nutr Oct;133(10): abstract The effect of colfee oonsumptoon on cardiovascular dosease has been debated for many years. In this work, we evaluated I he assodatoon between ootfee oonsumption and the nsk of developong acute coronary syndromes, based on a random sample of 848 patients with!heir firsl OOfonary heart disease event and 1078 frequency-matched cantreis with no cardoovascular disease in!heir medica! hostory, from the entire country. The multrvariate analysis raises a J shaped assoetabon between lhe risk of developong acute coronary syndromes and lhe quantity of colfee consumcd per day. In particular, lhe odds ratios lor moderate (<300 mud). heavy ( mud), and very heavy (>600 mud), consumption, rctative to no consumplion, were 0.69 (95% Cl, ), 1.56 (95% Cl, 1.1ü-2.34) and 3.10 (95% Cl, ), rospectivety, alter cantrolling lor the presence of hypertension. hyperchotesterolemia, diabetes mellitus. family history of premature coronary heart disease, physical activity status, smoking habils, BMI, alcohol consumplion, lriglycerides, consumplion of several lood items, depression scale score and cd u cation status. The suggested J-shaped assodation between coffee consumption and lhe nsk of developing acute coronary syndromes may partoally explaon the conflicting resulls from ether studies on the past. Study population. The CARDI02000 is a multicenter case-controt study that explores the association between saveral demographic. nutnhonat, psychological, lifestyle and clinical factors wolh the risk of developing nonfatal acute ooronary syndromes Aocording to the popuiatien distribution provided by the National Statoslocal Services we stratified our sampling into all regoons of Greece. For each regoon, we randomly selected a specific number of pallenis from!he prefectonaf or lhe maj()( pnvate hospitals of each county. The patienls were trom approximatety half of the elinoes in Athens and Thessatonoca (the two major cilles on Greece, covering -55% of the total populaloon}, and from al most all of the clinics of the ether countoes. The number of partoeipants was delermined lhrough power analysis to evaluale a mooimum difference of 7% in relahve nsk between those who do and do nol drink colfee with slalislical power of 0.80 and P < Thus. trom January 2000 lo August 2001, 848 of 956 patienls (89% response rate) who had been randomly presetected trom the hospitals with a first symptom of coronary heart dosease (slable angina was exctuded froon the anatysos) on their file agreed to participate in the study (cases). I) doagnosos of first acute myocardial onfarctoon thal was defincd by two of three features, i.e., electrocardiographic changes, or campalobie clonical symptoms orland specific diagroosllc enzyme elevatoons (49% of the patients had myocard ia I infarctoon}; or 2} doagnosos of unstable angina (i e one 0< more angina episodes at rest wothon the preeedoog 48 h wilh no abnormal enzyme rise) correspondong to class 111 olthe Braunwald classoficallon (51% of thc pallenis had unstabte angona). Patients with a prevoous ho story of cardiovascular disease, mctuding aartic aneurysm, stroke and peripheral vascular dosease. were excludcd trom thos study. After the seteetion of the cardoac patients, we randomly selected 1300 controts who were free of cardiovascutar disease; 1078 of these (83% response rate) agreed to participale. The controts were frequency matched to the pallents accordong to their age distnbul,on (wrthon classes of = 3 y). their gender and the region of Greece (we matched!he controts woth the patoents by region 10 reduce the potenbal confaunding effect of culture drilerences among Greek Cltizens}. Controts were mamly individuals (91 % of the total number of conlrols) who

9 v1s1ted the outpatient departments of the same hosp1tal during the same time period as the coronary patients tor minor surgical operations (e.g.. bone fracture) or tor routine examinations (e.g., medica! examinahans tor the driving license or other official certificates such as validatien of the soc al security handbooks). We used these types of controts to have more accurate medica! information, to eliminale the potenlial actverse effect of several, unknown. confounders and to increase the likelihood!hal cases and controts sharcd lhe samc study base ill). However, in a tew country hospitals in wh1ch the available number of "hospitalized" controts was nol sufficient lor the match ng procedure, we randomly selected a small proportion of people (9% of the lotal number of controls) trom the municipality rolls. An 1nd1cator variabie was used tor the sensitivity analys s that was applied to explore whether the different "sources" of controts m ght influence our l10d1ngs. All controts were people Wilhout any clinical symptoms. s gns or suspicion of cardiovascular disease in!heir medica! h1story, ncludmg stroke, aortic aneurysm and penpheral vascular disease. as evatualed by a card ologist. /nvestigated measuremonts. All participants were asked to descrîbe the1r usual frequency of consumption of colfee over the last year. Based on the d stnbution of colfee consumption, we categorized usual colfee consumpbon as none, up to 300 mud (moderate use). or up to 600 mud (heavy use) and >600 mud (very heavy use). All reported types of colfee (instant colfee. "Greek" type, fittered or "cappuccino")were adjusted lor one cup (150 ml) of colfee and a caffe1neconcentration of 27.5% (1.~). We did nol nclude the consumption of decaffeinated coffee. tea and caffeinecontaining drinks (Coca-Cola) or chocolate. Ceasing to dnnk coffee dunng the last year (in months of abstinence) was recorded and taken into account as a covariate tor the analysis.
