1 College University Transfer BSN Program Students (Victoria Campus only) Offsite Placement Guidelines NURS 491 (6 weeks) or NURS 475/NURS 491(12 weeks) for 2016 Acceptance for an Offsite Placement is provisional, pending the successful completion of all BSN program course work prior to the commencement of the Offsite Placement and pending acceptance of the request by the placement site itself. The School of Nursing reserves the right to revise and/or cancel the placement opportunity at any time. Offsite Placement Application Portfolios are to be submitted in their entirety directly to Marius Miklea, Administrative Assistant, Practice fax Application Deadline: 4pm, May 1, 2015 Late or incomplete application packages will not accepted Individual Offsite Request for Placement To obtain an Offsite placement(s), students will prepare and submit an Offsite portfolio indicating the location where they would like to have their final six or twelve weeks of Consolidated Practice Experience (CPE) placement. The location will be outside the usual placement areas used by the University of Victoria, BSN program (i.e. Island Health, South Island). Please note: In all regions in Canada and in Island Health Central/North Island, local student requests are given priority over Offsite student requests. Each offsite request is for NURS 475/491 OR N491 only. Applicants for an Offsite Placement must submit for committee approval: Portfolio of information detailing the request (see below), and including: a. Cover letter b. Resume c. Background details about the agency d. Budget e. Two faculty references All previous college and university practice appraisals will be reviewed as part of the evaluation process. Each applicant is considered on a case by case basis to balance students interests and career goals with individual student practice performance and the University of Victoria graduation requirements. Final approval and selection is made by a subcommittee of the UVic Undergraduate Education Committee (Offsite Placement Review Committee). Please Note: Applicants currently on practice probation or any form of University probationary status will not be considered eligible for an Offsite Placement Preceptor Criteria All offsite placements are offered in a preceptorship model of practice education. Preceptors must be Registered Nurses with active practicing registration within the jurisdiction in which they are employed and the preceptor must be in a practice setting relevant to the student s identified learning needs. A preceptor may not be a family member or close friend of the student. Preceptors are not reimbursed by the University, or the School of Nursing, for their time and efforts. Expectations

2 Applicants must: maintain strongly positive practice evaluations prior to NURS 475 and/or 491 a minimum cumulative GPA of 6.0 (B+) on the UVic scale of 9.0 students are responsible for all costs associated with organizing the Offsite Placement, travel, accommodation, food, and any other associated financial costs (including but not limited to liability, travel and medical/basic and extended health insurance). be prepared to engage in a minimum of one phone call per week with the instructor be prepared for a minimum of three phone calls (over a six week period) including the preceptor, student, and instructor. Please plan for one hour for each of these conversations. attend all on campus workshops/orientations associated with the NURS 475/491 practice experiences prior to departure. Students will negotiate with their instructor any further learning activities for completion of course requirements during the Offsite placement. Such arrangements must be discussed with the CPE Coordinator and/or the course instructor BEFORE departure. Finding a Placement Many agencies have a specific department which reviews all student requests and requires the Practica Coordinator at your learning site to make the initial contact. In order to find out how to proceed, and before initiating contact with possible placement locations, it is important that you make an appointment with the Practica Coordinator. You can check the hospital or agency website to see if there is information on student requests. If you have an opportunity to speak with a preceptor directly, always ensure the unit manager knows you have expressed interest in placing a practicum request. No placement can go ahead unless the agency administration has given explicit approval. Requests for agency placement(s) in the Vancouver & Lower Mainland Health Authorities Placement requests at hospitals, public health, home care and/or primary health care agencies within the Vancouver Coastal, Fraser Health or Provincial Health Services Authorities must be made by the Practica Coordinator. Be specific in your application as to site, unit, and area of focus. If you are exploring a location in the Lower Mainland not under the auspices of a Health Authority, you may contact the agency directly yourself. Tips for Finding a Placement: Be professional and courteous in your search. Use the internet to research ideas and identify possible health care agencies Ask family, friends and nursing colleagues about placement suggestions Write a brief script to use for phone inquiries that includes: student s full name, specific interests in the agency and learning needs, the course level and learning requirements, and the UVic School of Nursing Practica Coordinator contact information. Ask for dates by which you should follow up. Insurance Coverage Effective September 2013, there are two significant changes to practicum insurance coverage for students provided by the University of Victoria. Registered nursing students participating in practicum in BC or Ontario will no longer be provided the university purchased practicum insurance, but will receive coverage through WorkSafeBC or WSIB. Practicum students that are placed outside of these two provinces will continue to be enrolled in the university practicum insurance policy. 3 Applying for an Offsite Placement STEP ONE: Attend the Offsite Placement Information Session (February 25, 2015) STEP TWO: Preparing the Offsite Application Package (Practice Portfolio) A) Cover Letter* (maximum one page) Include: 1. Proposed agency contact information 2. Rationale for how this placement application supports your learning needs as a student 3. Previous nursing and other life experiences that support the request B) Resume* *Submit 2 electronic copies. Please note that one cover letter and resume will be used for a blind review of applications and therefore, should include no identifying information (submit 1 normal resume/cl and 1 anonymized resume/cl). C) Letter to the Offsite Application Review Committee (maximum 2 pages) 1. Agency description: mandate of the agency and activities in which students can participate (one paragraph). 2. Student background: describe any non nursing based experiences, education, and preparation from previous experiences in challenging and new locations to support an offsite request; indicate language proficiency in the location of choice (half page maximum). 3. Student expectations and learning goals: how this experience will foster student learning goals and enhance professional practice, future career or life plans, challenges anticipated, etc. (half page max). 4. Self assessment of nursing competencies: summary of your nursing practice experiences, strengths and areas in need of improvement (half page max). 5. Contacts and placement logistics: organization s mandate and size; name and contact information (telephone, , fax, agency website) of the administrative person and the preceptor. In addition, describe the background experience, current responsibilities and educational preparation of the preceptor indicating the preceptor s strengths to support student learning (one paragraph). 6. Faculty References: Provide two faculty/instructor references (forms attached). Note instructions on forms. D) Budget (see worksheet below) This budget estimate outlining the costs and required resources for the Offsite Placement(s) must include: the total budget for the duration of the Offsite Placement, any other funds you have either applied for or received, and, a brief rationale to support proposed expenditures. E) Reference Forms (see forms below) Two practice reference forms are required. It is the student s responsibility to provide the referees with appropriate information concerning the Offsite Placement application process. 4 F) Application Fee A $100 non refundable administration fee made payable by cheque or money order and addressed to the University of Victoria, School of Nursing. Applications exceeding the above page requirements will not be processed. Incomplete or ill prepared applications will not be reviewed. Attention to spelling, grammar and overall presentation will be considered. STEP THREE: Notification of Decision Decisions will be forwarded by UVic to all candidates for Offsite Placement in late August. Please note that all offers will be conditional pending the successful completion of all required courses, as outlined in these guidelines. STEP FOUR: Final Approval Final approval is determined after a review of September December Term 7 indicating a strong academic standing and progression towards meeting the curriculum competencies in N370/470 consolidated practice. However, if a significant concern arises during any prior placement up to the final appraisal in N475 and the offsite placement might jeopardize the student s success in the BSN program, then the offsite committee reserves the right to cancel the approval and require that the student remain on site for the placement. The Practica Coordinator must be notified of all placement details, some of which are required for the affiliation agreement that must be arranged between the agency and the University and finalized before a placement can start. Except for Vancouver Lower Mainland partner sites, once final approval has been received, students can confirm their placements with the preceptor and manager. For Vancouver Lower Mainland sites, the Practica Coordinator will make the initial contact and inform the student of the details for connecting with the site. In addition to the UVic affiliation agreement being in place, all documentation outlined on the Document Checklist must be submitted to the Practica Administrative Assistant (at UVic) or the Practice Coordinator (at UVic) as appropriate at least four weeks prior to departure. STEP FIVE: Communication and Grading Mechanisms for ongoing communication between instructor, student, and preceptor must be identified, established and on the student s file, prior to departure. It is the responsibility of the student to ensure that the course instructor is in receipt of the student and preceptor s contact information (e.g. telephone number, cell number and address). The course instructor provides ongoing instruction to the student, as well as course information, support and guidance to the preceptor. Students are responsible for initiating weekly contact with the instructor to provide evidence of learning. The instructor determines whether or not the student passes or fails the practice experience, thus communication with the student and the preceptor must be maintained throughout the practice course. Students must notify the instructor with any changes to contact information, and are responsible for costs associated with communication, such as phone calls. Students must notify the instructor and Practica Coordinator immediately if the experience is terminated prior to the established completion date. STEP SIX: After the Practice Experience Upon completion of the practice experience, and as part of their final practice appraisal, students will provide a brief report to their instructor outlining the successes and challenges of their experience to assist with future requests. 10 UVic School of Nursing Offsite Practice Placement Opportunity RELOCATION CHECKLIST NO LATER THAN FOUR WEEKS PRIOR to your scheduled relocation you MUST provide the following completed list to the Practica Administrative Assistant. Workshop Requirements Participate in required pre departure workshops provided by the School of Nursing and/or UVic. Communication and Logistics Review and take with you the Offsite Placement guidelines, including any communications from the Offsite Placement Review Committee. Contact instructor (if/when known) with student and preceptor contact information, practicum schedule, and plan for initial check in and 3 way teleconference. Note: A Learning Plan to address course ends in view (consolidation, or transition) will be required prior to entering practice. Tuition/Financial Considerations Consult with a Financial Aid Officer (if taking scholarship or loan). Make arrangements for Power of Attorney if loan needs to be picked up while away. Pay all outstanding UVic fines (e.g., parking, library, etc). Safety and Health Requirements Carry UVic and placement emergency contact information. If required, secure prescription drugs for the duration of the offsite placement (and a physician letter if needed). Consider travel and additional medical; basic & extended health insurance for the offsite experience. Provide evidence of appropriate medical vaccinations as required. Site Requirements Complete any pre requisite requirements of the placement i.e. online orientation to the Health Authority, photo ID, security pass, etc. prior to or upon arrival.
