New York Statewide School Health Services Center October 2007 Immunizations These two immunizations can be given no more than four days before the child s first birthday. o What are the MMR and Varicella vaccines? This kind of exemption from immunizations should contain an expiration date. o What is a medical exemption? Immunizations are not required for students over this age. o What is age 18? A documented case of shingles is acceptable proof of this disease. o What is Varicella? A two dose series of this vaccine is only acceptable for students receiving the doses between ages 11 to 15 years old. o What is Hepatitis B? Who Can Do? Only a person with this licensure may assess and triage. o What is an RN? This person can terminate a drip method Gastrostomy feeding if flushing is not involved. o Who is an appropriately trained unlicensed person? The one who provides food for students on a special diet (i.e. for celiac disease). o The school district. This person may administer epinephrine using a standing order. o Who is a registered nurse? This person may apply vagal nerve stimulation in school settings with an understanding of proper technique and potential side effects. o What is an unlicensed person?

2 Medications This student can identify his or her medication, the purpose of the medication, and refuse the medication. o What is a self directed student? The titles of 5 of the 8 professionals that can prescribe medications for students. o Who are: Physicians, Dentists, Podiatrists, Nurse Practitioners, Physicians Assistants, Special Assistants, Optometrists, and Midwives? Medications are given in a timely fashion when given within this time period of the prescribed time. o What is one hour? Requests for these do not need to be honored by the school nurse but can be. o What are non FDA sanctioned requests, herbal remedies, dietary supplements and natural products? The six routes of medication administration that can be done in school. o What are: oral, subcutaneous, intramuscular, intravenous, inhalant and rectal routes? Commissioner s Regulations Required for new entrants and in Grades K, 1, 2, 3, 5, 7, and 10 o What is distance acuity vision screening? This is valid for 12 months from the first day of the school year in which it is required. o What is a mandated Health Appraisal? Required for male and female students in Grades 5 9 o What is Scoliosis Screening? A valid reason to allow a non homeless student to attend school without proof of immunizations and without a health appraisal. o What is a religious exemption? (written specifically for each requirement) Adequate personal health guidance must be provided to each student receiving them when making these available at school What are condoms?

3 SN Stuff Must be a registered professional nurse o Who is a School Nurse? The ages that pediatric pads are recommended for use with AEDs. o What are ages 1 8? Daily Double! A parent designee is no longer able to do this on a field trip? o What is administer another person s child s medication? What is allowed now? 0 You will need a HPN number to do this in the near future o What is the Immunization Survey? A health appraisal is required every three years for this school group o What is the Committee on Special Education? 0 The SCAT card is a valuable tool in assessing this health condition o What is a Concussion? Secondary This document is valid for 12 months through the last day of the month in which it was done o What is a sports physical? You need this completed for a 7 th or 8 th Grade student to play a high school sport o What is Selective Classification? The three things that must be presented to obtain a work permit o What are evidence of age, parent signature and certificate of physical fitness? This should be prepared and filed within 24 hours of a related event o What is an Accident Report? 0 This kind of permit is issued to a minor from age 14 to 18 to work as a bootblack o What is a street trade working permit?
