2 Enhancement of Nursing Educational Capacity: Three Pilot Projects in Wyoming Wyoming Center for Nursing and Health Care Partnerships (WCNHCP) Summary of Pilot Project Outcomes July 2011 The Wyoming Center for Nursing and Health Care Partnerships (WCNHCP) developed a Request for Proposal (RFP) that called for projects focused on enhancing nursing educational capacity for the State of Wyoming through industry partnerships (See separate file for RFP). Specifically, eligible projects were required to have a minimum partnership consisting of an educational institution and an employer of nurses. The RFP was distributed via to the WCNHCP Advisory Board, Wyoming Nurses Association (WNA), Nurse Educators of Wyoming (NEW), Wyoming Hospital Association (WHA), 2010 Educational Summit participant list and an internal nursing administrator distribution list. It was also posted to the WCNHCP website. Four proposals were received and three proposals received funding. The outcomes of the pilot projects were presented at the 2011 Nursing Education Summit in Casper Wyoming. I. Education to Nursing Practice Graduation is Just the Beginning This project focused on increasing educational capacity through successful preparation of experienced nursing professionals to take on clinical instructional roles. (See Appendix A for power point presentation and final evaluation). A. Project Personnel: 1. Project Director: Kathy Wells, Central Wyoming College (CWC), Nursing Program 2. Educational Partner: Central Wyoming College 3. Industry Partners: Riverton Memorial Hospital, Lander Regional Hospital. B. Project Goals & Outcomes: 1. Increase numbers of nurse educators a. Process 1. Two CWC faculty assisted 12 clinical staff nurses to develop and present a medical surgical nursing module to nurses in the facilities in which they worked. b. Outcomes: 1. Participation resulted in increased satisfaction ratings of support offered by CWC faculty.

3 2. Nurse participant ratings of the likelihood of considering a Part-time or Fulltime faculty position as excellent or very good increased from 2 of 12 prior to the workshop to 6 of 12 after the workshop. 3. One presenter applied for and was interviewed for a clinical faculty job at CWC 2. Increase numbers of preceptors a. Process 1. Duration of preceptor orientation was lengthened within both Hospitals. 2. Formalized mentor relationships regular meeting times were established for preceptors and mentors to share experiences and feedback. 3. Established objectives to be completed during orientation. b. Outcome: 1. Pre-test: 72% of Preceptors rated their satisfaction as a preceptor as extremely or very satisfied. 2. Post-test: 79% of Preceptors rated their satisfaction as a preceptor as extremely or very satisfied. 3. Program attrition was 50% due to turn-over in nursing staff. II. Associate Degree Curriculum Review for Addition of Simulation The project focused on developing recommendations for incorporating simulation into the Sheridan College AD Nursing Program curriculum and its possible effects on enrollment. First, curriculum review was to be used to identify adjustments needed to incorporate simulation into the didactic classroom instruction and to identify the ideal percentage of clinical practicum that should be devoted to simulation. The next step would be to identify the education needed by current faculty to utilize the simulation. Finally, the project proposed to determine whether simulation resulted in better NCLEX scores and would allow for an increase in enrollment. A. Project Personnel: 1. Project Director: Crissy Hunter RN, BSN Sheridan College 2. Educational Partner: Judith McDowell Sheridan College 3. Industry Partner: Sheridan Memorial Hospital (SMH) B. Project Goals & Outcomes: 1. Sheridan College will develop recommendations for increasing simulation in AD nurse education curriculum.

Review of the current curricula and discussion with nursing faculty, resulted in the addition of two simulation clinical experiences in the fall semester. i. 1 st year students will have one clinical day utilizing simulation to take care of a post-operative patient requiring IM and SubQ medication administration. ii. 2 nd year students will have two clinical days utilizing simulation to enhance clinical confidence and competence in the obstetrical field. b. Faculty are currently discussing spring semester simulated experiences. 2. Develop recommendations for increasing enrollment through SIM use a. Staff from 13 Nursing programs were interviewed. Programs included: i. Glen Oaks Community College- Michigan ii. Miles Community College-Montana iii. University of Montana-College of Technology iv. University of Alaska- Anchorage v. Provo College- Utah vi. Oglala Lakota College- South Dakota vii. Laramie County Community College- Wyoming viii. Western Wyoming Community ix. Gillette College- Wyoming x. Central Community College- Nebraska xi. North Dakota (5 community colleges with state-wide curriculum). xii. Grossmont Community College- California xiii. Glendale Community College- Arizona b. Nursing programs that were interviewed utilize their simulation to enhance current clinical experiences, with a 4 hour maximum. It was felt that this approach would not allow them to increase enrollment. c. Based on the interviews, literature review and the addition of the simulation experience as a clinical rotation, the project director estimates that Sheridan College might be able to increase enrollment to 32 from 28. d. Summer 2012, after using simulation for one year, the Sheridan College enrollment will be re-evaluated. 3. Determine value of simulation for increasing NCLEX scores. a. Reviewed literature indicates that simulation enhances student s self-confidence, clinical judgment, and overall competence. b. While none of the nursing programs interviewed regarding their use of simulation had noted an increase in NCLEX pass rates, all of the programs felt it was too soon to evaluate due to early stages of simulation implementation. c. Sheridan College will evaluate simulation and its correlation to NCLEX scores each year, starting Develop faculty training recommendations in relation to simulation a. Reviewed literature suggests that successful use of simulation requires a commitment to its use by faculty. b. In the summer of 2011, four faculty attended simulation training and after completion, they rated themselves as more confident in the role that simulation could play in nurse training.

5 c. Sheridan College plans to mandate the next simulation training session for all faculty. III. Educational Capacity Assessment for a Regional Simulation Center This project focused on identifying the sustainability of a regional simulation center. Specifically the goals of this study were to determine types of certification and training the proposed simulation center should offer, willingness of health centers to pay for simulation training for their employees and perceived barriers and benefits to simulation training. To do this, the grantee conducted interviews and deployed a survey to regional health centers. The results of the data collection were used to obtain a HRSA grant by SMH and Sheridan College to build the simulation center. (See Appendix C for power point presentation and final evaluation). A. Project Personnel: 1. Project Director: Nancy Hooge, MS, BSN, Sheridan Memorial Hospital 2. Educational Partner: Judith McDowell, Sheridan College 3. Industry Partner: Patty Callantine, RN, Sheridan Memorial Hospital B. Project Goals & Outcomes: 1. Establish business aspects of a Simulation Center, including potential clientele, pricing structure for trainings, and number of students to be served through interviews and a follow up survey. a. Interviews with Health Care Partners in 7 communities identified a need for additional clinical training. Sixteen (16) surveys were returned from the 7 facilities. b. Fourteen (14) of the 16 respondents indicated their facility has not used simulation in the past, but 12 (75%) indicated they would be willing to send employees to Sheridan for training; the remaining 4 indicated they were not sure. c. Currently, 57.1% of respondents do not use SIM because training equipment is not available or within driving distance. d. 100% of facilities pay for employee certification. 2. Prepare a business plan (1, 3, 5 year). The business plan was developed 3. Building of the Wyoming Simulation Center through funds obtained from Health Resources and Services Administration grant. Open house for SIM center was July 21, 2011 in Sheridan, Wyoming.

6 Summary Overall, these three projects support the utility and feasibility of partnerships between nursing schools and the nursing industry for improving nursing education and potentially capacity. The collaboration between nurse educators at Central Wyoming College and clinical nurses at Riverton Memorial Hospital and Lander Regional Hospital demonstrates the potential for increasing the numbers of nurse educators. In the second set of projects the collaboration between Sheridan College nurse educators and Sheridan Memorial Hospital resulted in the building and opening of a simulation center. This center has the potential to provide high quality education to student nurses as well as practicing nurses and other health care professionals. Improved education may lead to increased retention, improved NCLEX scores, and possibly the ability to provide education to more students more efficiently. This is the second round of pilot projects the WCNHCP has funded.
