Chemeketa requires internatinal students t purchase this plicy. This plicy is administered by HTH Wrldwide Insurance Services and is underwritten by AET- NA. If yur spuse and/r family is entering the U.S. with an F-2 visa, they will als need health insurance cverage. 6 Chemeketa requirements statement Please read and sign Internatinal prgrams will be allwed t use the abve infrmatin fr purpses f prcessing the fllwing services: admissins, immigratin, medical and/r ther items prvided by the prgram. The cllege is bligated t disclse infrmatin t the U.S. Immigratin and Custms Enfrcement Agency thrugh the electrnic system called SEVIS (Student and Exchange Visitr Infrmatin System). Academic Requirements The fllwing academic requirements are based n cllege and federal plicy gverning internatinal students n an F-1 visa. They apply t yu nce yu are admitted. If yu d nt meet these requirements, it may result in academic prbatin r dismissal. Failure t cmply with items related t the USCIS ruling gverning F-1 student status culd affect yur status with the U.S. Department f Immigratin. 1. Each student must cmplete a minimum f 12 credit hurs with a 2.0 GPA each term r 18 hurs per week (nn-credit), with the exceptin f ne term a year which is cnsidered a vacatin term. A vacatin term nly ccurs after three (3) full-time cnsecutive terms. Chemeketa recgnizes summer as an fficial vacatin term. Challenge and nline curses may nt apply. 2. Each student must cmplete his/her study in an apprpriate timeline, as printed n the I-20, r request an extensin. 3. Each student must infrm Chemeketa f his/her current address, phne and within ne week f change. 4. Each student must participate in the rientatin and Student Success prcess, which will include a registratin wrkshp, classes, and supplemental instructin advising. All internatinal students will take placement testing upn arrival. All students will take r successfully challenge required Orientatin classes. Health Insurance Requirements Each student is required t btain illness and accident insurance thrugh Chemeketa that meets the requirements listed belw. Prir t registratin the insurance premium must be paid in full N insurance cverage is prvided until full payment is received: $250,000 benefit per accident r illness ($1,000,000 lifetime maximum: Cverage payable at 100% after $15 c-pay.) $50,000 fr medical evacuatin: (If yu becme ill r have an accident and need t be sent hme fr care, this will pay fr yur transprtatin.) $25,000 fr repatriatin f remains: (If yu die because f an accident r illness, this pays fr the return f yur bdy t yur cuntry.)
