1 Lake-Sumter State College Course Syllabus Course / Prefix Number HIM 1800C Course Title: Professional Practice Experience I CRN: 2029 Credit: 2 Term: Spring 2016 Course Catalog Description: Instructor: Lake Hawk This course enables the student to practice health information functions in the laboratory environment. Topics addressed include: imaging records; health record content and format; health information numbering, filing and tracking systems; assembly and analysis; incomplete record control and health information retention and retrieval functions. Students will use medical records and an integrated electronic Health Information Management Virtual Lab to practice these procedures. Additionally, the student will spend time observing the medical record functions at either a dental or medical clinic. Prerequisites: Textbook and Other Course Materials: Technology and Online Computer Access Requirements: Prerequisite or Co-requisite: HIM 03 (if prerequisite, C or higher is required) None This course is a fully Online course. The Lake-Sumter State College website, Blackboard, Blackboard Collaborate Web conferencing utility, Microsoft Word, and a high-speed internet connection is required to attend the course. Visit and select Browser Check to ensure that your system is ready for use. It is the students responsibility to have an alternative plan if their main computer system fails. Computers are available in LSSC s Libraries and Learning Centers on each campus. You may need to arrange your schedule to be able to use on-campus computers to complete your assignments. By the end of the first week of the term, the instructor will assume students are well acquainted with the Blackboard environment and have become proficient at the following: Navigating course content in Blackboard; Attaching documents; Creating discussion postings; Submitting assignments; Browsing the Internet; Creating and formatting documents in Word 20 or Word 2013 By the second week of the term, all students should have resolved all technical issues with the LSSC Helpdesk and should be ready to fulfill the requirements of the course.

2 Course Objectives: (what the course will do) Student Learning Outcomes (SLOs) Assessed in this Course: (what the students take with them beyond this course) This course enables the student to experience practical application of the entry-level HIM strategies and procedures they are learning in other HIM courses. The student will demonstrate the ability to: maintain the completeness of the patient record as defined by organizational policy and external regulations and standards; Use technology, including hardware and software, to ensure data collection, storage, analysis, and reporting of information; use specialized software in the completion of HIM processes such as record tracking use appropriate electronic or imaging technology for data/record storage The learning outcome numbers in parentheses are from the 2011 AHIMA Curriculum Competencies for associate degrees in Health Information. Students will not be dropped from the class for lack of participation but are expected to withdraw by the deadline listed below or receive a grade of F. 4 Miscellaneous: Grade disputes: Please address any grade disputes immediately after you receive a grade. It is recommended that this is done within 2 weeks of receiving assignment feedback. The end of the semester is not an appropriate time to discuss missed assignments or poor grades earned earlier in the semester. Students are expected to stay current on course readings and course content. Students are encouraged to openly participate in course discussion and ask questions to promote critical thinking and stay engaged in the course content. Extra credit: No extra credit is available in this course, including revising an assignment or doing extra work. The opportunity to earn a favorable grade in the course comes in doing the assigned work well, on time, and in accordance with due dates. Please refer to the assignment explanations so you know what is expected from each assignment. Assigning Grades: See Assignment Details for instructions and requirements for each assignment. Points will be removed for failure to meet requirements and follow instructions. Assignment Feedback: Grade explanations will be posted in Blackboard 1-2 weeks after due date for students who lose points on assignments (does not apply to Quizzes). Late assignments: Late assignments will be assessed a point per day penalty. A documented emergency is the only situation where late points will be waived. A student must receive permission from the instructor to submit and assignment later than a week after the due date. Documented emergencies: Late assignment submission will be excused for unexpected emergencies when a student can provide proper documentation. These include severe illness of the student/student's dependent(s), deaths in the immediate family, other crisis and severe weather conditions. Students must contact the instructor and provide documentation as soon as possible. is the preferred method of contact, but it is the student's responsibility to contact the instructor via phone or in person if they are unable to utilize . Judgment of whether a situation is an emergency, whether documentation is sufficient and whether the student contacted the instructor in a timely fashion is entirely at the discretion of the instructor.

5 COURSE CALENDAR (Assignment details) Medicare Learning Network Courses Online Training from CMS: STUDENT MUST ALSO COMPLETE THE FOLLOWING ONLINE MODULES AND TURN A CERTIFICATE OF COMPLETION IN WITH THE FINAL JOURNAL. Certificates will be returned for inclusion in the students resume. Go to: Then, scroll down and select web-based Training. Look to the left side below the search/browse function. There are modules listed on the left side.. Complete the following Modules: World of Medicare Your Office in the World of Medicare. (If you have done these in a previous course please just go to the website and save another copy of your certificate to attach to the submit assignment tool in Blackboard. Also, you may be asked to do one or both of these in a subsequent course so you will be able to submit the certificate of completion with the date from this course at that time) EHR Simulations Students Forms Design will review the rule Master Patient Index Simulations Deficiency Analysis Students will complete multiple EHR simulations including Vista, Athens Cerner, and McKesson EHR exercises. Students will review the rules of good form design and then revise an outdated form. See Blackboard under Course Content. Virtual lab assignment described in Blackboard under Course Content Virtual lab assignment described in Blackboard under Course Content Interview or observation in a medical office due See instructor if you need help finding a medical office that will allow you to either interview the practice manager or observe the non-clinical aspects of the work. The student will interview a medical office manager and/or arrange to spend 4-6 hours observing the non-clinical functions in a physician practice or clinic. The student needs to ask about and document typical patient workflow from scheduling and registration, through check-in, the clinical encounter, check-out, coding/billing, posting payments, reports and finalizing payment with notes on the title of the person doing each step, what forms and software they use, whether or not they scan anything into the system, and, if they have an EHR whether or not it is interfaced with, integrated with or standalone from the practice management system. The student also has to find out who determines if the record is complete and how do they do this. Finally, the student needs to find out what policies & procedures are applicable to release of information. Scanning into an EHR If you have an opportunity to do this during your site visit to a medical practice or you have performed scanning of medical records previously, you just need to document how the process worked, what EHR system was used and what that office was scanning (paper records from prior to their conversion as patients were scheduled or only paper copies received from another provider or the patient) and then submit that description through the "submit assignment tool" on Blackboard. If scanning was not observed during your interview/observation, you must arrange to meet with the instructor no later than April, 2016 to see a demonstration and then scan a record into our academic account in Practice Fusion.
