3 1. Preface 1.1 Software description The software modul Abutment Designer facilitates the individual design of abutments. Available are two design alternatives. Custom Abutment offers the possibility to design the abutment and an anatomically reduced coping in one step from the fully anatomic outline. Occlusally screw-fixed offers the possibility to create an abutment that can be veneered or an occlusally screw-fixed bridge with individually designed screw canal. The software comes with an extensive abutment library. Amongst others, it contains the abutments of NT Trading. Titanium bases incl. screws, scan abutments and lab analogs are available from NT Trading. The scan abutments are recorded with the scanner Tizian TM Scan 102. The ideal adaption of the abutment is done by superimposing (incl. Best-Fit matching) scan abutment and library abutment. The hybrid technology offers a safe and long-lasting fit of the abutment. 1.2 About this manual Abutmentdesigner is an accessory to Tizian TM Creativ RT modellation software (software versions or higher). Therefore, this manual only deals with the functions and handling of the Abutment Designer. It is an addition to the Tizian TM Creativ RT instruction manual (software version 3.932), which describes the handling of the basis software in detail. This manual cannot and shall not replace the Tizian TM Creativ RT instruction manual

4 2. Creating a case in the workflow software To activate Abutment Designer, select implant type under reconstruction types (in the tooth scale) in the workflow (software version or higher): There are two alternatives: Custom Abutment Okklusal verschraubt offers the possibility to design the abutment and an atomically reduced coping from the fully anatomic contour in one step. offers the possibility to create an abutment with an individually designed screw canal. Note: A 5-axes procedure is necessary to obtain safe and failure-free milling of a screw canal. To activate the scan of a gingiva mask, select yes under separate gingiva scan. After the jaw situation has been created in the workflow completely and has been scanned subsequently, it's time to start the actual modellation. Please see the separate instruction manuals of workflow and scan software for detailed information

5 3. Modellation procedure when selecting "occlusally screwfixed" 3.1 Opening the CAD-software and choosing an abutment from the library The workflow icon opens the modellation software Tizian TM Creativ RT (one left-click with the mouse). The situation created in the workflow and scanned is automatically opened by the modellation software. Furthermore, the following dialog field will be opened: The first step is to upload the desired implant type from the data base. To do so, click the orange triangle and select the desired implant type from the drop-down menu. In the example, it's the S-series of NT Trading

6 If the selected series is available in different sizes, an additional drop-down menu is opened and the desired size must be selected. After the selection, a picture of the selected abutment appears in the dialog field. Superimposed on this picture is a red dot. Loading the library abutment is started by clicking onto the position of the red dot in the scan. Pressing the button Best-Fit matching will initiate the correct alignment, i.e. adapting the library abutment to the scanned abutment. Pressing next will finish this workstep

7 3.2 Definition of the emergence profile Next, the dialog field define emergence profile is opened. Set up and function of the buttons are along the lines of those for the determination of the prep margin. They are described in detail in the instruction manual of Tizian TM Creativ RT modellation software. The emergence profile is roughly provided by setting several points along the profile. To do so, move the cursor (cross) into the desired position on the gingiva scan and left-click on the mouse to confirm the position. After setting several points, press start to initiate the calculation. Clicking the tab Correct/Draw will activate this tab. The course of the emergence profile can be individualized by clicking the green points - 7 -

8 The pink line in the white field constitutes the enlarged section plane around the cursor. The blue arrow displays the exact position of the cursor. The field facilitates finding the course. Pressing next will finish this workstep. 3.3 Tooth placement The buttons for tooth placement are described in detail in the instruction manual Tizian TM Creativ RT. Please see these instructions for more information on the handling. Pressing next will finish this workstep

9 3.4 Designing the abutment bottom The course of the abutment bottom follows the course of the emergence profile. Modellation is displayed on screen: So-called control points line the margin of the abutment bottom (marked with red circles in the picture). These points facilitate the modification of the course of the abutment bottom. Magnified diagram of a control point: Each control point has three functions: Arrows pink ball pink dot move the point into the direction they point to free movement of the point into all directions releases the control point from the gingiva To change the position of a control point, move the cursor onto the desired function and change the position by keeping the left mouse button pressed and moving the point as desired. Pressing Ctrl and left-clicking the margin of the abutment bottom creates additional control points. The below picture shows abutment bottom and screw canal of an example case. All other views were hidden for a better view

10 Based upon this picture, the functions of the dialog field Guide Generate abutment bottoms are described below. The dialog field is separated into two tabs: 1. Bottoms, 2. Advanced. Switch between the two tabs by clicking them. The active tab is white, while the inactive tab is displayed in grey. The section Shape offers the possibility to change the shape of the abutment bottom. The buttons are described below. The diagram shows the handling of the control points. Undo will cancel all entries. Redo will repeat the last entry. Back opens the previous workstep. Next finishes this workstep. Initially, the buttons are deactivated, i.e. displayed in grey. Left-clicking on the desired button will activate it, i.e. display it in orange. Click to increase the dents in the profile of the abutment bottom, without regenerating it. Keep button pressed for a continuous movement. Click to increase the bulge of the profile, without re-generating it. Keep button pressed for a continuous movement. Click and pull the slider to regenerate the profiles in the desired shape. Example: dented abutment profile bulged abutment profile

11 By checking the box Freeform, this function is activated. The picture will change and a short description of the function Freeform will appear. The abutment bottom will be displayed yellow in the modellation, i.e. it is activated for individual design. Freeform offers the following possibilities: 1. Press Ctrl and left-click onto the abutment bottom margin to create new control points. 2. Press Ctrl keep the left mouse button pressed with the cursor in the yellow area to bulge it. 3. Press Shift and left-click onto the yellow area to dent it. Example: Abutment bottom dented on the left side and bulged on the right side

12 Checking the boxes Freeform and Visualize activates an additional function and at the same time, it activates the sliders below. A color scale is displayed underneath the picture, which illustrates the distances between abutment bottom and gingiva. These colors are likewise displayed on the yellow abutment bottom of the modellation. Beside the option to visualize the distance to the gingiva by color, there is also the possibility to check the box Limit to limit this distance accordingly. Check Above Limit, i.e. activate it to limit the distance to the ginigva. Deactivate this function and accordingly the limit by removing the check. Check Below Limit, i.e. activate it to determine the maximum intersection of the gingiva. Deactivate this function and accordingly the determination of the intersection by removing the check. Left-mouse click onto the typ Advanced will activate this tab. The tab has three sections: 1. Profile borders 2. Move all margins 3. Unstick all Pressing Back will open the previous workstep. Pressing Next will finish the workstep

13 Profile borders facilitates the individualization of the abutment bottom margin. Height Radius Changes the height on the border of the emergence profile. A higher value will create a less harsh border which facilitates milling, especially when working with ceramic materials. Creates an additional radius (horizontal) in the contact area interface/abutment. 1. Example "Standard parameters" Height marked with a red circle: Radius marked with a red circle: 2. 14 The function Move all margins lets the user move the abutment bottom margin into the desired direction. In Out Up Down All control points are moved toward the abutment center. Keep left mouse button pressed for continuous movement. All control points are moved away from the abutment center. Keep left mouse button pressed for continuous movement. All control points are moved up. All control points are moved down. All control points are unstuck from the gingiva. There the following possibilities: 1. Keeping the left mouse button pressed on a point will change its height. 2. Pressing Ctrl and left-clicking on a control point can create a predefined height, i.e. the height of all control points is changed simultaneously. 3. Pressing Shift and left-clicking on a control point can create a predefined (horizontal) radius. This also affects all control points. All control points are stuck to the gingiva

15 3.5 Further design steps The following modellation is carried out conventionally. The procedure is described in detail in the instruction manual of Tizian TM Creativ RT modellation software (software version 3.932). Therefore, we will not describe the different worksteps "Freeforming - Adjusting - Shrinking and subsequent freeforming" here. 3.6 Designing the screw canal If occlusally screw-fixed was selected in the menu point Implant type in the workflow, the next step is the screw canal design. The modellation is displayed on screen as seen below: The function field is separated into different sectors: 1. a tooth scale with several buttons in its middle 2. the functions Thickness and Height C opens a text field which briefly describes the different functions. Back opens the previous workstep. Next finishes the workstep. There are several options for the design of the screw canal

16 3.6.1 Modifying the screw canal with help of the buttons Thickness Height modifies thickness of the screw canal (the inner diameter remains unchanged) modifies the entire height of the screw canal Modifying the screw canal with help of the control points Move the control points with the mouse to modify the shape of the screw canal. Add control points by pressing Ctrl and leftclicking onto the margin. Remove additional control points by left- and right-clicking onto the point simultaneously. All control points can be unstuck from and restuck to the occlusal surface by clicking onto the circle below each control point. green = unstuck black = stuck Clicking a control point will activate a central control point in the middle of the screw canal. This central control point will change the height of those control points that are unstuck. The height of the control points stuck to the occlusal surface will remain unchanged

17 Once the control points are unstuck from the occlusal surface, press Ctrl + Shift and click onto the central control point to create a plane screw hole surface. Examples: all points are stuck all points are unstuck only the top two points are stuck Modifying the screw canal with help of the tooth scale Once a tooth in the tooth scale is selected (clicked), the user will receive three suggestions for design: 1. Fix screw canal to anatomy (white tooth) 2. Adjust screw canal to reduced crown (green tooth) 3. Cancel screw canal i.e. do not create a screw canal (yellow tooth)

18 3.6.4 Buttons in the middle of the tooth scale shows or hides the the screw canals snaps the screw canal to the anatomy or to the framework i.e. the reduced crown With On the control points are stuck, with Off they are unstuck from the occlusal surface End result Finish the workstep Designing the screw canal by clicking next. Save the construction in the last step. 3.7 Saving The bridge construction is automatically calculated into one complete structure. On screen, the finished construction is displayed in green

19 The dialog field Merging will open: To finish the modellation press Finish. Remove existing merged parts erases the finished, millable construction. The separate segments remain. Optimize for selective laser melting optimizes the millable construction, i.e. all transitions are smoothed. To do so makes sense when using highly difficult techniques such as laser melting. After clicking Finish, the following screen will appear: Press this button to shut down the software and to return to the workflow. The following dialog field will appear before finally closing the software: Confirm with yes (Ja) to save the current scene to the project directory, i.e. the scene can be loaded again when opening the software next time

20 4. Modellation procedure when selecting the type "Custom Abutment" 4.1 First modellation steps The following worksteps are identical to those of the type "occlusally screw-fixed". Therefore, we will forego describing them again. - Opening the CAD-software and selecting the abutment from the library (see 3.1) - Defining the emergence profile (see 3.2) - Tooth placement (see 3.3) - Design of the abutment bottom (see 3.4) - Further modellation steps (Freeform Adjust to anteriors) (see 3.5) 4.2 Determining the secondary insertion axis This step determines the insertion axis for the supraconstruction, i.e. the secondary insertion axis. The following modellation is displayed on screen: blue arrow = insertion of the abutment green arrow = insertion of the supraconstruction Grab the green arrow to change the insertion axis for the supraconstruction (right diagram). Determine the insertion axis conventionally. The button Set current view as insertion axis influences the insertion axis of the abutment as well as of the supraconstruction

21 4.3 Abutment design The dialog field for the abutment design is separated into three tabs. 1. Top, 2. Bottoms, 3. Advanced. Left-clicking on the tabs lets the user switch between them. The active tab is white, while the inactive ones are grey. The tab Top facilitates design of the abutment top. It is separated into three sections: 1. Minimum angle 2. Distance to chewing surface 3. Visualize Undo cancels the latest entry. Redo will repeat the latest entry. Press back to return to the previous workstep. Press next to finish the workstep. The button minimum angle facilitates the determination of the angle of inclination. The dotted line constitutes 0. Examples

22 There are so-called control points along the lines. The abutment top can be created individually with help of these control points. Please see 3.4 (ff.) for the correct handling of the control points. Distance determines the distance between anatomic shape and abutment Auto-adapt to occlusal area adapts the abutment shape to the anatomic shape Apply Reduce the distance is applied to the control points; simultaneously pressing Ctrl + Shift when pressing apply cancels all changes and returns the shape to standard reduces the abutment size; simultaneously pressing Ctrl + Shift when pressing apply cancels all changes and returns the shape to standard Checking one of the boxes will display the color scale, which illustrates the according distance. Distance to anatomy (abutment colored) Distance to abutment (anatomy colored)

23 The tab Bottoms contains all applications for the abutment bottom design. It is separated into the sections Shape and Freeform. Please see 3.4 for a detailed description of its handling. Undo cancels the latest entry. Redo will repeat the latest entry. Press back to return to the previous workstep. Press next to finish the workstep. The Abutment bottom is displayed in grey. If the button Freeform is aktivated, it will be displayed in yellow. The tab Advanced contains all applications for the design of the abutment borders. It is separated into the sectors Profile borders, Milling parameters and Move all margins. For the handling of Profile borders and Move all margins, please see 3.4. Undo cancels the latest entry. Redo will repeat the latest entry. Press back to return to the previous workstep. Press next to finish the workstep

25 4.5 Modellation of the supraconstruction In the following, the supraconstruction is created: - design of the crown bottoms (cement gap, border design, undercuts) - shrinking the construction - freeforming the reduced construction Use the conventional modelling technique. Please also see the instruction manual Tizian TM Creativ RT (software version 3.932)

26 4.6 Saving The bridge construction is automatically calculated into one complete structure. On screen, the finished construction is displayed in green. Subsequent modification of the construction Finished and/or saved cases can be modified subsequently. To do so, select the according case from the workflow (load button) and is then opened with the CAD-button. The modelling software will open and the following dialog field appears: Yes/Ja No the saved scene (modelled construction) is loaded the scan is loaded to restart the modellation

28 After confirming with Yes/Ja the modellation is reloaded: In general, there are two possibilities for subsequent modification: 1. Subsequent modification of the complete supraconstruction by freeforming Right-click into the background and select Freeform connected construction. This will open the dialog field Freeform, where the user can proceed to change the data set as normal. Please also see the instruction manual Tizian TM Creativ RT (software version 3.932). Please note: The abutment bottom of the occlusally screw-fixed abutment as well as the "customized abutment" cannot be modified for safety reasons. 2. Subsequent modification of separate parts of the construction To do so, delete parts already saved. Right-click into the background and select Delete reconstructions. The following drop-down menu will open: Under Parts to delete, the user will find a list or parts that can be erased. Select the list, which contains the construction parts that are to be modified. The parts are all listed separately in the dialog field

29 After the parts were deleted, right-click into the background to activate the functions for the desired modifications. Proceed as normal. Important: Manually save all modifications after you are finished. To do so, click into the background and select Save restorations. Always confirm with Finish, otherwise the modifications will be cancelled
