2 9:00-10:00 am Colorado Health Alliance Panel Discussion Joe Sammen Executive Director for Colorado Coalition for Medically Underserved Joe has a passion for working to solve complex public health problems through systemic changes and has a long history of community organizing both across the nation and around the world. His work at CCMU maximizes his great talents as a relationship-builder, researcher, and communicator, to engage and mobilize Coloradans, empowering them to improve the health of their community. Joe works to build authentic relationships with diverse organizations, communities, health leaders and community residents across the state. As a leading expert on collaboration, health equity and health care systems change, Joe advises and coaches state and local leaders and works closely with communities around the state of Colorado providing technical assistance in facilitation, research, strategic visioning and networking. Joe serves on multiple boards and committees throughout the state, including Aurora Health Access Board of Directors, the West Mountain Regional Health Alliance s Board of Directors, Mile High Health Alliance s Leadership Team, the Colorado Health Access Fund s Advisory Committee, and the Better Care, Better Cost, Better Colorado Steering Committee. Joe earned bachelor s degrees in both biology and computer science at the University of Denver and also holds a master s in public health from the University of Washington. 3 10:30-12:00 pm MORNING KEYNOTE: Life Care Centers of America presents: Kathy Fraser CMSA National President: Case Management: If You Aren t Climbing Upward, You Are Tumbling Down! Kathy is a Registered Nurse, a Certified Case Manager, Certified Rehabilitation Registered Nurse and Board Certified in Case Management with the American Nurses Credentialing Center. She holds a Masters Degree in Nursing and also a Masters Degree in Health Care Administration. Kathy has been a nurse for 37 years beginning with Hospital nursing which included Emergency Room, Labor & Delivery and as a Director of Nursing. She entered Case Management 23 years ago, creating the 1 st Case Management Department, for a large National Hospital System. Kathy moved to Worker s compensation over 20 years ago and for over 15 years she has worked as a Regional Team Manager of Zurich Financial Services Managed Care Case Management Unit. She was recently selected by Seak, Inc. and the National Workers Compensation and Occupational Medicine Conference as one of the 50 Most Influential People in Workers Compensation and Occupational Medicine. Kathy is the current Case Management Society of America s National Board of Director s President through June She is also on the Role Delineation and Expert Panel Committees of the ANCC Council for Case Management, Board of Directors of NTOCC, on the steering committee of the National COPD Foundation and a member of the National Certification Roundtable Committee. 1. Describe how emerging trends will affect case management and the essential role case managers will play in promoting improvements in health care delivery. 2. Discuss collaborative strategies and share best-practice information for improving professional practice with case management colleagues and experts from across the care continuum. 3. Analyze current trends in quality improvement, patient safety, clinical advances, health care policy, and professional leadership that contribute to effective models of case management across the health care continuum. 12:00-1:00 pm Vendors/Lunch/ Poster Session 1:00-2:00 pm Managed Care Case Management Colorado Access. Beth Neuhalfen, BS, CHC Manager of RCCO Care Management and System Transformation Beth Neuhalfen is the RCCO Manager of Care Coordination and System Transformation with Colorado Access. Beth has a B.S. in exercise physiology with an emphasis in cardiac rehab from Colorado State University and continuing education classes at University of Colorado, along with her license from ACE as a health coach. She has served various populations in cardiology, pulmonary rehabilitation, corporate health coaching, chiropractic-rehabilitation, care-management, pediatrics, and as the project manager for the PCMH, medical neighborhood, Advancing Care Together behavioral health initiative, and integrated medicine. Beth has also done speaking engagements in many states on caremanagement, medical neighborhood specialist/hospital, PCMH, and behavioral health. Beth was a
4 reviewer for the Safety Net Medical Home Initiative (Continuous and Team-Based Healing Relationships). Recently, she was interviewed by a Policy Analyst at the National Academy for State Health Policy (NASHP) on the medical neighborhood. She is particularly committed to developing and evaluating ways to integrate physicians, stakeholders, and patients to unite as a team in health care. 1. Participants will be able to understand the population we serve within the RCCO. 2. Participants will be able to view the structure of care management and how we are helping to not segment health care. 3. Participants will understand how we stratify the population and how we address access to the medical home, specialty care and communities. 4. Participants will see how we collaborate with hospitals, primary care, and communities during a transition. 5. Participants will see how we are fostering the engaged patient and our outcomes to date. 2:00-2:30 pm Vendor Raffles/Break 2:30 3:30pm AFTERNOON KEYNOTE: Case Management At the Summit. What s Next? Nancy E Skinner, RN Nancy has for the past 30 years served as a national case management educator. In her current role as principal consultant for Riverside HealthCare Consulting, she advances programs that promote excellence in care coordination and other transitional care strategies. In 2002, she was named the Case Management Society of America (CMSA) National Case Manager of the Year and in 2008, she received CMSA s Lifetime Achievement Award. She currently serves as a Board Member for the Hospital Quality Foundation and as a reviewer for the Patient Centered Outcome Research Institute. Additionally, she assisted in the development of Post-Discharge Accreditation Standards for Cardiovascular Disease in partnership with the Society of Cardiovascular Patient Care. Nancy is an Advisory Task Force Member of the National Transition of Care Coalition and serves as primary faculty for the University of Southern Indiana Case Management Certificate Course. Objectives: 1. Understand the changing role of the care coordinator as the mandates and regulations associated with the Affordable Care Act are implemented 2. Recognize competencies that advance desired continuing care outcomes while advancing the provision of quality and cost-effective care through the healthcare continuum. 3. Develop a care map that is respectful of and responsive to individual patient preferences, needs and values.
