We want the UK to have a healthcare system of the highest possible quality safe, effective, personcentred, timely, efficient and equitable. 2 The challenge you all know: Population is older and frailer, many live with multiple, long-term conditions Life expectancy 12 years longer than in 1948 People over 60 yrs = nearly 25% of UK population and 50% have chronic illness Pressure on hospital capacity is growing Emergency admissions up 12% since 2007/08 in England Health budgets held down due to financial crisis In the 10 years before 2010 average annual increase for NHS 4%, now 1% 3 The Flow Cost Quality Programme 2 NHS hospitals : South Warwickshire NHS Foundation Trust Sheffield Teaching Hospitals NHS Trust Approach had been used before Larger scale implementation Experimental and ambitious 4 Why did we focus on flow? Poor systems deliver poor results for patients, NHS staff and taxpayers - Complex systems (organised by function not patient) - Invisible problems (no one person responsible or sees the whole pathway) - Poor quality at many steps in care pathways Flows in a Healthcare System - how, when, where & who = impact quality 5 6 1

3 Case Study: Patient access to appropriate clinicians The problem: Lack of availability of right staff at right times to meet demand Frail elderly waiting in A&E <4 hrs hours before transfer to Medical Assessment Unit (MAU) 2/3 admissions to MAU after 6pm only junior medical staff available Overnight stay to see consultant next day Further delay - first seen by consultant physician before referral to geriatric medicine consultant Case Study: Patients getting early access to senior clinicians The solution Changed consultant hours to match admission patterns Geriatric medicine consultants available at the front door Faster turnaround process for diagnostic tests Dedicated MAU for frail older people established with a multi-disciplinary team Pooled junior doctors to meet patient demand and eliminate repeated assessments Key learning points from the programme Improved flow at Sheffield associated with decline in geriatric deaths Impact on mortality The programme indicated that poor flow increases the likelihood of harm to patients System-level measures at both hospitals showed an apparent correlation between poor flow and mortality As patient flow improved the measures showed a reduction in mortality Improving patient flow helps to reduce avoidable harm and deaths
4 Impact on quality By reducing lengths of stay, bed occupancy and re-admissions the programme helped to improve patient and carer experience Improved flow at Sheffield associated with increase in percentage of patients discharged from frailty unit on day of admission or following day Despite an 11.5% growth in emergency admissions over 12 months, South Warwickshire maintained A&E performance and achieved high levels of patient satisfaction as a result of improved flow In Sheffield, improved flow led to a reduction in bed occupancy and a 37% increase in the number of patients discharged on the day of admission or the following day at a time when demand remained the same Embedding and sustaining change Two years on both Sheffield and South Warwickshire hospitals have sustained momentum and impact Sheffield s discharge to assess model is in operation on two wards and is in the process of being rolled out across the whole city In January 2014 South Warwickshire had hit A&E targets for 7 consecutive months, had reduced mortality, was operating within budget and implemented a discharge to assess model Important lessons: Focus on patients needs - patient stories are a great driver for change Diagnosis, rigorous data collection & sharing between teams enabled staff to understand why change was necessary Visual management - making issues, data, patient stories, learning & progress visible widens involvement Flow solutions involve cross-department redesign Testing ideas on a small-scale - gives confidence and builds the evidence Improvement strategies must fit the local situation - an adaptable, participative approach is crucial Achieving impact takes time change at system level has taken 2 to 3 years Read all about it: Improving Patient Flow How a focus on patient flow improved the quality of the care and patient experience at a time of financial pressure Widening the impact We are working with policymakers across the UK to make sure they know about what has been achieved NHS Scotland is testing the Flow Cost Quality model as part of its Whole Systems Patient Flow improvement programme The Patient Flow programme in NHS Wales, led by 1000 Lives Improvement builds on the Flow Cost Quality programme the Health Foundation is funding the evaluation of this national programme Download the learning report for FREE from: In England we are working with the Urgent and Emergency Care Review team and the Seven Day Working programme team to see how Patient Flow approaches can add to this work
5 Where next? Patient flow is not just a concern for A&E departments every health care service and department needs to take an interest in flow All service planners and managers should review if capacity and staffing levels match the variations in patient demand Changes in existing structures, work processes and culture at every level are needed in order to improve patient flow across the health and social care system Sheffield and South Warwickshire have shown what is possible!! 25 5
