10 In March 2006, an explosion at National Institution of Higher Learning in Chemistry (ENSCMu) in France killed a Professor, injured a 19 year old student and caused damages amounting to US$130 M! Consequence of Consequence of safety lapses Fatalities & injuries!

11 More stringent enforcement by regulators

12 Accidents in Universities - Explosion APh Ph.D. research student, t performing an experiment inside id a fumehood, was injured by flying glass shards, which were generated from an explosion that occurred in a reaction flask (see photo below). Although the fumehood sash was partially down (about half way), the researcher received injuries mostly to the right side of his face (see photo below) and to his left hand and arm. No injuries were associated with the eyes since the researcher was wearing safety glasses with side shields.

13 Accidents in Universities Eye Injury Although he was wearing eye protection and gloves., the accident occurred later when, having removed his glasses, he returned to pack the acid and other equipment away. It can only take a moment's inattention or one wrong step for you to lose one of the most precious assets you have your vision.

14 Accidents in Universities iti Eye Injury An investigator was blinded in one eye when a cryotube exploded while being thawed. The probable cause was the rapid expansion of liquid nitrogen that had entered the tube through a small crack during storage.

28 New Approach to Safety Management Reduce Risks at Source. Emphasize the importance of good OSH management systems, especially the need for comprehensive risk assessments. Assign liability to those who are in control o of workplace risks. Promote Industry Ownership of Standards and Outcomes. Shift industry mindset from following the letter of the law to taking responsibility of standards and outcomes. Higher Penalties for Poor Safety Management.
