1 Medication Therapy Management Program A service for better understanding and managing the drugs you take at no additional cost to you.

2 Better information for better outcomes at no additional cost to you Medication safety can be confusing. From when you take your medication to what you eat with it simple behaviors affect outcomes. And missteps can be dangerous. The Medication Therapy Management Program is a service offered to you at no additional cost on behalf of your health plan. This program is designed to help you better understand your medicines increasing safety and lowering costs when possible. The Medication Therapy Management Program is managed by Prescription Solutions by OptumRx and has two parts. One part gives you direct access to a personal pharmacist. Over the phone, he or she will conduct a Comprehensive Medication Review. The pharmacist will then work to help you better understand your medicines and answer questions you have. The second part of the program includes providing you with ongoing medication information through the mail. 2

3 The program starts with a simple phone call To receive your Comprehensive Medication Review call our program provider, Prescription Solutions by OptumRx, at (TTY 711), 8 a.m. 8 p.m., ET, Monday Friday. A friendly customer service agent will guide you through the phone call, gathering information on your current medicines and overall health. Most calls take about 15 minutes. If you have easy access to current health information, gather it before you call for more personalized results. Items could include: A list of your current medicines, including prescription and over the counter medications Any recent lab results, if available Most recent blood pressure reading, if monitored Most recent blood sugar reading, if monitored 3

4 Next, you speak with a pharmacist After your first phone call with a Prescription Solutions by OptumRx customer service agent, you will be scheduled for a call with a Prescription Solutions by OptumRx pharmacist. This call usually takes another 15 minutes. All calls are confidential and available at no cost to you. The Prescription Solutions by OptumRx pharmacist will conduct a Comprehensive Medication Review with you. During this review, he or she may help you: Better understand your doctor s orders Understand your medicine and their uses Reduce side effects Save money With your permission The pharmacist may share gathered information with your doctor when it s in your best interest. For example, the pharmacist may fax a letter to your doctor if you are taking a medicine that does not work well with other medicines you take. The pharmacist may also notify your doctor when you are not taking a medicine that may be helpful in managing your health condition. 4

5 For your files After the phone conversation with the pharmacist, a written summary will be mailed to your home. It will include your medication history and also will include personal recommendations from the Prescription Solutions by OptumRx pharmacist about how to improve your medication use. Helpful follow up and ongoing education Once in a while you will receive helpful information in the mail from our pharmacy team. This information can include a review of the medicines you have filled recently and suggestions from our pharmacists about how to make the most of your medicines and benefits. This information can be helpful when meeting with your doctor or pharmacist in the future. 6 Frequently Asked Questions How many calls will I have with the pharmacist? You may have more than one call with the pharmacist during the year. The number of calls depends on the medicines you take and whether there are any issues with your medicines. Can my caregiver speak with the pharmacist on my behalf? Yes. A family member, friend or other caregiver may speak with the pharmacist on your behalf with your permission. Each time you want your caregiver to speak with the pharmacist on your behalf, the pharmacist will first need to speak with you to verify your consent. You may also fill out an Authorization of Representation (AOR) form from Prescription Solutions by OptumRx. When you complete, sign and mail it back to Prescription Solutions by OptumRx, your caregiver may speak with the pharmacist on your behalf without needing consent each time. If I don t want to be in this program, will it affect my Medicare drug plan benefits? No. Your Medicare prescription drug plan benefits will not change whether you do or don t choose to participate in the Medication Therapy Management Program. Is the medication review with the pharmacist available in other languages? Yes. The medication review with the pharmacist is available in other languages. When you call, ask the pharmacist to connect a translator to the conversation. The translator will help you and the pharmacist communicate. 6 What does the program not provide? The pharmacist will not prescribe new medication or make changes to your medication therapy. Only your doctor can make these changes. The program does not provide financial help such as assistance with copays.

7 How to cancel or opt out of the Medication Therapy Management Program You can cancel or opt out of the Medication Therapy Management Program at any time. Simply follow these easy steps: 1. Call Prescription Solutions by OptumRx at , 24 hours a day, 7 days a week. When asked, please enter the 7 digit security code provided at the bottom of your letter. 2. You have three options to opt out of the program. Please select one of the following options: If you do not want to take part in the Comprehensive Medication Review with a pharmacist, press or say one after the chime If you do not want to receive ongoing education through the mail, press or say two after the chime If you do not want to take part in the medication review with a pharmacist and do not want to receive any mailings, press or say three after the chime 3. When you are done, the phone system will confirm your selection. Once confirmed, you can end the call. TTY users: Please call the TTY customer service number , 24 hours a day, 7 days a week. A customer service agent will request the following information: Name Date of birth Member identification number 7 digit security code provided at the bottom of your letter Please note: You may continue to receive mailings for up to 30 days after your call as we process your request. You do not need to call again to opt out. 7
