Instead, they want to make sure their loved one is cared for in the comfort of his or her own home. But care costs money. When the time comes, will your family have the financial ability to consider all the options an decide what s best for you? with the CA$H-First Advantage The Assured Solutions long-term care insurance policy combines the complete flexibility of: A Cash Benefit Money that can be used to pay for any type of care you choose & Traditional Reimbursement Benefits Where you are reimbursed for actual expenses incurred through a home health care agency, assisted living facility or nursing home.

3 You can elect to receive CA$H-First benefits one month and reimbursement benefits the next to give you greater flexibility in how you spend your policy benefits. So no matter which option you choose, you know its right for you and your family. BENEFITS OF THE CA$H-FIRST Advantage Complete flexibility The cash is yours to spend as you see fit Allows you to pay for anything, anytime, anywhere CA$H-First benefits begin day one, meaning, there s no elimination period to satisfy. Additionally, while you are receiving benefits, no premiums are due. You simply receive the full monthly cash amount which can help pay for anything at all No bills to collect and mail for reimbursement. No need to worry whether expenses will qualify for coverage. CA$H-First benefits are paid as an alternative to other policy benefits. CASH ALWAYS WORKS Cash for family travel expenses Cash for missed work Cash for the latest LTC technology Cash for housekeeping services Cash for any other out-of-pocket costs CASH TO BUY THE TIME TO MAKE THE RIGHT ONGOING CARE DECISION Reimbursement Benefits Home Health Care Assisted Living Nursing Home You also have the option to receive reimbursement for actual expenses you incur. They may include services to help keep you at home such as assistance with the activities of daily living, help with grocery shopping, meal preparation and housekeeping as well as the services of a registered nurse, home health aide or therapist. If you require a higher level of care, your policy provides reimbursement benefits for care in an assisted living facility or nursing home. The Choice is YOURS Eligibility to Receive Benefits You are eligible to receive benefits when a licensed health care practitioner certifies: IFYou are chronically ill, which means you need hands-on or stand-by assistance to perform at least two of the six activities of daily living: bathing eating dressing toileting continence transferring OR You need continuous supervision due to a severe cognitive impairment AND You are expected to need care for at least 90 days

4 Build Your Benefit Package 1CHOOSE Your Maximum Daily Benefit Your policy will reimburse you for home health care, assisted living and nursing home care expenses up to your Maximum Daily Benefit amount. (Your agent can help you determine the right amount for you based on current daily cost of LTC in your area.) Your choices are: $50/day to $500/day [in $10 increments] 2Your CA$H-First Benefit AMOUNT Your CA$H-First monthly benefits are equal to 10 times the your Maximum Daily Benefit. When you are receiving the CA$H-First Benefit, the full monthly benefit will be paid, regardless of any actual expenses you may incur. Maximum Daily Benefit $ x 10 = CA$H-FIRST Monthly Benefit $ Example: If you select $200 Maximum Daily Benefit your CA$H-FIRST benefit would be $2,000/month. 3CHOOSE Your BENEFIT PERIOD The benefit period you select is the minimum time period your policy will pay benefits. Any days that you receive less than your full maximum daily benefit amount, such as when you receive CA$H-First monthly benefits, will lengthen the time your policy will pay benefits. Your choices are: 730 days 1095 days 1460 days 1825 days 2190 days 2555 days 2920 days LIFETIME [2 years] [3 years] [4 years] [5 years] [6 years] [7 years] [8 years] 4Calculate Your Maximum Lifetime Benefit Multiply your Maximum Daily Benefit by your Benefit Period to determine your initial Maximum Lifetime Benefit. Maximum Daily Benefit Benefit Period Maximum Lifetime Benefit days $ $ x = Example: If you select $200 Maximum Daily Benefit and your Benefit Period is 1825 days, the Maximum Lifetime Benefit would be $365,000. [$200 X 1825 days = $365,000] 5THEN, Choose When YOUR Benefits WILL Begin CA$H-First Benefits CA$H-First benefits begin on day one with no elimination period to satisfy. 0 Days Reimbursement Benefits Reimbursement benefits for home care, assisted living and nursing home begin after you satisfy your elimination period. This is the number of days you receive care under your policy before benefits are payable. Your choices are: 0 Days 30 Days 60 Days 90 Days 180 Days 365 Days

5 Your Inflation Protection Options The cost of long-term care services may continue to increase over time. Adding an inflation protection option guarantees your policy s Maximum Lifetime Benefit amounts will increase, ensuring your coverage is adequate in the years to come when you are most likely to need care. Compound inflation Protection: Compound lifetime On each policy anniversary, through the life of your policy, all current daily benefit amounts and your current Maximum Lifetime Benefit will increase by the percentage you choose with no annual increase in premium. 5% 4.5% 4% 3.5% 3% 2.5% Five Percent Compound 20-Year Your current Maximum Daily Benefit and Maximum Lifetime Benefit amounts will increase by five percent on each policy anniversary date for 20 years. Simple Inflation Protection: Five Percent Simple Lifetime On each policy anniversary date, your Maximum Daily Benefit will increase by five percent of its original value. In addition, the Maximum Lifetime Benefit will increase by five percent of its original value or its current value, whichever is less. No Inflation Protection: Guaranteed Purchase Option If you do not elect an inflation protection option when you apply and are not receiving benefits under the policy, you will have the option to increase your original Daily Maximum Benefit amounts by 10% every two years. Your Maximum Lifetime Benefit will increase by the lesser of 10% of it s original amount or 10% of it s current amount. Each time you exercise the Guaranteed Purchase Option, additional premium for the increased amounts will be based on your age at the time. This option will remain in effect until: your 80th birthday, your 20th policy anniversary or you decline the increase offer two consecutive times. (Not available with Spouse Shared Benefit of Limited Payment Options.) 6 Built-in Policy Benefits 6 Save on Your Premium United of Omaha offers a variety of premium allowances to help you save money. You may qualify for one or more of the following: Spouse/Partner 35% savings if you and your spouse or domestic partner both purchase long-term care insurance policies from United of Omaha Married 15% savings if you are married, but your spouse or domestic partner does not purchase a long-term care insurance policy Two-Person Household 10% savings if both you and another adult living in your household (not your spouse or domestic partner) purchase long-term care insurance policies from United of Omaha Preferred 15% savings for being in good health Association Group 5% savings if you re a member of a qualifying association group Medicare Supplement 5% savings if you own a Medicare Supplement insurance policy from Mutual of Omaha insurance company or an affiliate company Care Coordinator Services Your policy offers the services of a care coordinator a licensed health care professional who can assess your needs, develop an individualized plan of care and services. There is no elimination period, which means you have immediate access to a care coordinator. The use of a care coordinator is optional except when utilizing Stay-At-Home Benefits or Alternate Care. Stay-at-Home Benefits Your policy will pay benefits for the following Stay-At-Home services: Caregiver Training Durable Medical equipment Home modifications Medical alert system Alternate care (See Outline of Coverage for details) Adult Day Care Your home care benefits also are available to pay for care received in an adult day care center. Respite Care Your policy will pay up to the Maximum Daily Benefit amount for up to 31 days each calendar year for the temporary services of another person or facility to provide care for you. No elimination period is required. Hospice Care Your nursing home benefits also can be used to cover the expenses incurred for hospice care in any setting when you are not expected to live beyond six months. No elimination period is required. Bed Reservation Your policy will pay up to 31 days per year to reserve a bed for you in your nursing home or assisted living facility while you are temporarily away. International Travel Full benefits are provided for care received in the United States, Canada or the United Kingdom. Anywhere else in the world, your policy pays a cash benefit equal to your Maximum Daily Benefit, regardless of the amount of actual charges incurred, for one full year. Waiver of premium No premiums are due while you are receiving CA$H-First benefits or reimbursement benefits for home health care, assisted living or nursing home care. Tax-Qualified Coverage Your long-term care insurance policy is intended to be tax-qualified and the benefits you receive tax-free. Five-Year PREMIUM Rate Guarantee

7 Optional Benefits Spouse Shared Care Benefit If you have exhausted your Maximum Lifetime Benefit but still need care, you can access benefits under your spouse s policy provided you leave at least one year of benefits for your spouse. If either spouse dies while both identical polices are in force, the surviving spouse receives the deceased spouse s remaining Maximum Lifetime Benefit amount without having to pay the deceased spouse s premium. There is no effect on the surviving spouse s premium. Waiver of Elimination Period for home care The elimination period you select will be waived for home care and adult day care, which means benefits for home health care will begin with your first day of services. Monthly benefit option This optional benefit converts your Home Care Daily Benefit into a single monthly Benefit. Spouse Security Benefit To provide extra security and income for your uninsured spouse while you receive long-term care services, your policy will pay an additional 60 percent of the reimbursement benefit you receive each month. This money can be used as needed and will not reduce your maximum lifetime benefit. Ca$h-First Advantage Increase Option You may choose to increase your Monthly CA$H- First Benefit from 10 times to 15 times your Maximum Daily Benefit. For example, if your CA$H- First benefit is $2,000 (10 times $200) it will increase to $3,000 (15 times $200). By electing this option, your maximum daily benefits for Home Health Care will also increase. The amount of the increase is 50%. For example: If your Maximum Daily Benefit is $200, it will increase to $300 for home health care and remain $200 for nursing home and assisted living care. Spouse Survivorship & Spouse Waiver of Premium If one spouse dies after both policies have been in force for 10 years, no further premiums will be due. The surviving spouse s policy will be fully paid up. While either you or your spouse receive benefits under your policy, the premiums on both policies will be waived. Limited Payment OptionS * You may pay premiums over the life of your policy or for a limited period of time, after which your policy will be fully paid up with no further premiums due. 10 Years 20 Years Paid-up at age 65 Return of Premium A refund of all premiums paid, less claims paid, will be made upon your death. Non-Forfeiture Shortened Benefit Period Your coverage will continue on a reduced basis in the event you stop paying premiums. Restoration of benefits If you no longer need long-term care services for 180 consecutive days, all benefit amounts paid will be restored to the Maximum Lifetime Benefit of your policy. 7
