5 SECTION I General Information/ Introduction

7 n I. Introduction: Alcohol and other drug addictions are chronic illnesses that affect nearly 1 million Illinoisans. Many of us know someone who suffers from substance abuse addictions, a disease that can take over a person s life and cause devastating consequences for addicts and their families.* According to the 2009 US Department of Health and Human, National Survey on Drug Use and Health, an estimated 22.5 million persons aged 12 and older were classified with substance abuse or dependence in the past year. The majorities of these 22.5 million persons needing treatment do not seek or receive treatment for their drug use or dependence, even though it has been shown that treatment reduces drug use by 40 to 60 percent. This percentage is comparable to success rates for other chronic diseases. ** This directory provides the contact information of licensed substance abuse treatment programs in Chicago. How to Use the Directory: This directory contains a list of publicly-funded licensed substance abuse treatment providers in the Chicagoland area. The directory was developed with the goal of including all programs that provide alcohol and other drug treatment and recovery programs. It is organized alphabetically by provider, including the program name, address and telephone number, the type of service provided, and the provider s payment structure. TTY numbers and websites are included, if available. Almost all treatment providers licensed for Level I, II, and III treatment can conduct an assessment to determine the appropriate treatment level. n II. Addiction: According to the American Society of Addiction Medicine, addiction is a disease process characterized by the continued use of a specific psychoactive substance despite physical, psychological, or social harm. The Diagnostic and Statistical Manual of Mental Disordersfourth edition (DSM-IV) recognizes substance addiction and dependence as, the individual s failure to abstain from using the substance despite having evidence of the difficulty it is causing. n III. An Appropriate Referral: Physicians, nurses, psychologists, case managers, and social workers are examples of professionals who are in the best position to make referrals to a substance abuse treatment program. If it is believed that an individual has an alcohol or other substance abuse disorder, the attending professional is expected to make a referral to a licensed substance abuse provider for a substance abuse assessment. The treatment professional, in turn, will assess the client s level of addiction using empirically valid tools based on the American Society of Addiction Medicine s criteria. A level of care will be determined based on this assessment. n IV. Referral Process: Referrals can be made via the telephone directly to the service provider. As the referring agency/professional, specific information about the client will be requested. Any additional information needed about the client will be obtained from the client during the initial screening and assessment at the treatment site. *Illinois Department of Human, Division of Alcoholism and Substance Abuse **National Institute on Drug Abuse 7

8 A. Waiting List Substance abuse treatment programs may have a waiting list for prospective clients. However, the federal government and State of Illinois have each identified priority populations for substance abuse treatment. When an individual meets the priority population criteria interim services should be offered while waiting for placement into a treatment program. Priority populations are identified on Page 10 of this directory. The intake staff of the substance abuse treatment program will explain their waiting list policy and the treatment eligibility requirements. Waiting list policies and wait-times may vary for each service provider. The waiting list eligibility requirements may include the prospective client calling the service provider periodically, participation in pre-treatment groups, and/or attending a 12-step recovery meeting. The sooner a prospective client is placed on a waiting list, the sooner he or she will get into treatment. B. Follow-Up To follow-up with a referral a service provider must have a written consent from the prospective client. If there is a need to confer with intake staff, a release of information signed by the prospective client with appropriate referral papers will be sent. Additionally, if written progress reports are needed, it is suggested that the service provider be informed as soon as possible. n V. Common Terms: This section briefly defines treatment modalities on the continuum of care, target populations, and payment structure indicated in this directory. American Society of Addiction Medicine (ASAM) Treatment Levels: Level 0.5. Early Intervention: Pre-treatment services for individuals whose problems or risk factors appear to be related to substance abuse but who do not meet any diagnostic criteria for substance abuse related disorders. Level 1. Outpatient: The purpose of outpatient services is to help individuals achieve permanent changes in their alcohol and drug-using behavior. These services are non-residential, can be provided in a variety of settings, and are appropriate for both adults and adolescents. The services include faceto-face individual, group, and family counseling, and may include medication maintenance, such as methadone for heroin users. These services are performed on a regular schedule that averages less than nine hours per week. Level 2. Intensive Outpatient: The purpose of intensive outpatient services is to provide essential educational and treatment components while allowing patients to apply their newly acquired skills within their own environments and without an inpatient residential stay. Generally, treatment services are delivered during the day, before or after work or school, in the evening, or on weekends. This service is for both adolescents and adults. The frequency and intensity of treatment at this level depends on patient need, but consists of a planned regimen of scheduled sessions for a minimum of nine hours a week. Level 3.5 Residential Rehabilitation: These services provide individuals with a safe and stable living environment to address their specific functional deficits and develop their recovery skills. Provider personnel provide care 24 hours a day/seven days a week and treatment services feature a planned regimen of services for a minimum of 25 hours per week. These services are designed for both adults and adolescents. 8

9 3.2D Social Setting Detoxification/3.7D Medically Managed Detoxification: The purpose of detoxification services is to provide immediate and shortterm clinical support to individuals who are withdrawing from alcohol or other drugs. Detoxification occurs in a clinically managed residential facility and is delivered by trained provider personnel that provide 24-hour supervision, observation, and support. This service provides structured care and support, 24 hours per day, for patients with severe intoxication/withdrawal signs and symptoms. Level 3.1 Residential Extended Care (Recovery Home/Halfway House): The purpose of residential care programs is to provide clinical services to adults and adolescents in a 24-hour structured and supervised recovery setting. The primary goals of these services are to support a patient s recovery in a sober environment and to facilitate a productive return to his/her family, employment, and community. The facilities that provide residential extended care are halfway houses (HH), recovery homes (RH), and sanctuaries. Other Relevant Terms/ Methadone: is a synthetic opiate, used in the context of a structured treatment program, including counseling for individuals addicted to heroin or other opiates. For the most part, methadone is provided in an outpatient setting. Aftercare: Aftercare services are important adjuncts to Levels 1 through 3. These services include continued counseling, recovery support services such as vocational training, parenting classes, housing assistance, and 12-step support groups. Oxford Houses: These houses are independent, democratically run, self-supporting and drug free environments. Entry on a limited basis may be accessible to individuals following a successful completion of a treatment episode from a provider licensed by the Illinois Department of Human, Division of Alcoholism and Substance Abuse (IDHS/DASA) and others in recovery. Access to Recovery (ATR): ATR is a federally funded program operated by IDHS/DASA for individuals who are 18 years of age or older, have a history of substance use or abuse, and reside in Cook County, and who may be involved in the criminal justice system The program allows the client to choose from a variety of traditional and non-traditional faith- and community-based providers that assist with the client s recovery. ATR funded services include: transportation, employment coaching, vocational training, continuing care, recovery coaching, recovery skills, pastoral counseling, spiritual support, and peer coaching. Specialty Service List: Bilingual : immediately accessible in languages other than English. Women s : specific to the needs of women and their families. Mentally Ill Substance Abuser (MISA): Programs that offer services to individuals with co-occurring substance abuse and mental health diagnoses. Driving Under the Influence (DUI) Evaluation and Risk Education: In Illinois, anyone arrested for driving under the influence of alcohol and/or other drugs must undergo an alcohol and drug evaluation before sentencing can occur, or the Office of the Secretary of State can grant restricted or full driving privileges. An alcohol and drug evaluation must also be completed before a judicial driving permit (J.D.P.) can be granted. This directory includes agencies that are licensed to conduct DUI evaluations and provide risk education. 9

10 Admissions, Special/Priority s: IDHS/DASA has identified populations for priority admission to substance abuse treatment services. These populations are given priority status because of their impact on families and society. Priority is currently given to the following populations in rank order: 1. Pregnant injecting drug users 2. Pregnant and post-partum women 3. Pregnant, Post-partum women and women with children 4. Department of Children & Family (DCFS) referred persons 5. Temporary Assistance for Needy Families (TANF) eligible persons 6. Department of Corrections (DOC) releasees, and Treatment for Special Clients (TASC) referrals : Fees that are based on verification of income with higher fees paid by persons with higher incomes. For some programs, the bottom of the sliding fee scale is zero, while others expect that the client pay some nominal amount no matter how low the income level. Third Party Payers: Treatment is paid for by private insurance companies. Medicare, Medicaid or grant funded treatment slots, at little or no cost to the consumer. Medicaid: Programs certified by IDHS/DASA to receive Medicaid reimbursement for some substance abuse services. The Chicago Department of Public Health also funds (under the Ryan White Program, Part A) community-based agencies in the Chicago Metropolitan Area to provide treatment to individuals who are HIV positive. You can contact the Department directly for a list of providers of these treatment services or a referral. Contact information for the Department is located at the end of this section. Source: IDHS/DASA s: Adult: An individual 18 years or older. Most adult programs will take emancipated 16 to 17 year old persons as adults. Adolescent: An individual age 12 to 18 years. Some programs may define this category more narrowly, and some may include immature 18 or 19-year-olds. s: Client Fee: The fee set by the substance abuse treatment provider for services. The client, his/her family, or a private insurance company is responsible for paying this fee. 10
