4 CASE SCENARIO #1 Your patient, Ms. Sweet, is a 56 year old patient that has type 2 diabetes and was admitted with CHF exacerbation early this morning. Her am labs showed a glucose level of 268. Ms. Sweet s physician places her on the Hyperglycemia Management Protocol default settings with aspart (Novolog) insulin coverage (AC, HS & 0300). Breakfast: 0700 FSBS is Ms. Sweet s correctional coverage would be:. Ms. Sweet ate: 100% cornflakes (16g) 100% reduced fat milk (12g) 2. Ms. Sweet s total carb intake is. Nutritional coverage would be. Lunch: 1130 FSBS is Ms. Sweets correctional coverage would be:. Ms. Sweet ate:100% of the beef tips 50% of the noodles (30g) 50% of the broccoli 25% of the carrot cake (30g) 100% of the dinner roll (30g) 4. Total carb intake is. Nutritional coverage would be:. Afternoon: Ms. Sweet calls you in concerned because she does not feel right. I am kind of shaky. You decide to check a FSBS and it is What would you do? Describe in detail. Supper: 1630 FSBS is Ms. Sweet s correctional coverage would be:. Ms. Sweet ate: 100% turkey 100% dressing (15g) 100% cranberries (15g) 100% green beans 100% LC Vanilla pudding (24g) 7. Total carb intake is.nutritional coverage would be:

5 CASE SCENARIO #1 ANSWERS The Hyperglycemia Management Protocol Default Settings are: x=25 & y=10.

6 1. Ms. Sweet s FSBS was 250. Her correctional coverage can be calculated using the following formula =150, =6 Ms. Sweet s correctional coverage would be 6 units aspart insulin subq. 2. Total carb intake is 28 grams of carbs. Use the following formula to calculate Ms. Sweet s nutritional coverage = 2.8 or 3 units aspart insulin subq. 3. Ms. Sweet s FSBS was 195. Her last two FSBS readings have been high. Advance Ms. Sweet a level on the HMP to level 3, x=20, y= =95, 95 20=4.75 or 5 units aspart insulin subq for correctional coverage grams of carbs. Nutritional coverage can be calculated using the following formula =6.56 or 7 units aspart insulin subq for nutritional coverage. 5. Follow the directions on the Hypoglycemia Protocol, rule of 15/15. Give Ms. Sweet 15 grams of carbs and recheck her FSBS in 15 minutes, repeat until FSBS above 70mg/dL. Notify care provider of FSBS and move Ms. Sweet back a level on the HMP. *Don t forget to document carbs and call to physician. *Don t forget to change x and y in the EMR. 6

7 6. Ms. Sweet was moved back to level 2 on the HMP, x=25, y= = -18, = -0.72, so you will subtract 1 unit from the nutritional dose. 7. Ms. Sweet s carb count is 54 grams of carbs = 5.4 or 5 units. 5-1(from correctional formula) = 4 units aspart insulin for nutritional coverage 7

8 CASE SCENARIO #2 Your patient was started on TPN this afternoon. Your physician has ordered to initiate the HMP for FSBS>140mg/dL. Your patient s 1600 FSBS is 180. How much insulin would you give? Correctional: Nutritional: 8

9 CASE SCENARIO 2 ANSWERS The Hyperglycemia Management Protocol Default Settings are: x=25 & y=10. Use this formula to calculate correctional dose =80, 80 25=3.2 or 3 units regular insulin for correctional coverage. Use this formula to calculate nutritional dose. 20 X 95= 1900, = 19, 19 10=1.9, 1.9 X 4= 7.6 or 8 units of regular insulin subq for nutritional coverage. Since you are not waiting on the patient to eat, add the correctional & nutritional values together and give the patient 11 units of regular insulin. Remember this dose is to cover the upcoming 4 hours. 9

10 CASE SCENARIO #3 You arrive to work and begin your 3-11 shift. Your patient has been receiving pulmocare continuous tube feedings at 70mL/hr for several days and is on the Hyperglycemia Management Protocol. They are currently on default settings. Their FSBS is 310 at What should you do? 1. Your patient is still on pulmocare continuous tube feeding at 70mL/hr. Their FSBS is 301 at What should you do? 2. 10

11 CASE SCENARIO #3 ANSWERS 1. Move the patient to the next level of HMP, level 3, X=20, y=8 Now, use the formula below to calculate correctional coverage =210, =10.5 or 11 units of regular insulin Use the formula below to calculate nutritional coverage. 0.1 X 70=7, 7 8= 0.875, 0.875X4= 3.5 or 4 units of regular insulin * g carbs/ml is found on page 2 of HMP or calculated from can Now add 11+4 and give patient 15 units regular insulin. *Remember you are giving this dose to cover the upcoming 4 hours. 11

Notify physician of FSBS over 300, since twice consecutively and advance patient a level on HMP to level 4, x=15, y= =201, =13.4 or 13 units regular insulin for correctional coverage. 0.1 X 70=7, 7 5=1.4, 1.4 X 4= 5.6 or 6 units regular insulin for nutritional coverage. Now add 13+6 and give patient 19 units regular insulin. *Remember you are giving this dose to cover the upcoming 4 hours. 12
