4 some Great reasons to have Private health insurance It pays to join earlier in life Did you know the cost of your hospital cover depends on the age you first take it out? That s because after you turn 31, the Federal Government s Lifetime Health Cover (LHC) loading means your premium could increase by 2% a year, to a maximum of 70%. This applies for 10 continuous years. Want to avoid the loading? Just take out and maintain hospital cover from 30 June, following your 31st birthday. For couples/families, any loadings are shared across both adults on the membership. Bupa cover could be less than the cost of the Surcharge Did you know the Medicare Levy Surcharge (MLS) kicks in if you don t have private hospital cover and earn over $84,000 (for singles), or $168,000 (for couples/families)?* Your surcharge may even be more than the cost of some Bupa hospital covers. The Australian Government Rebate on private health insurance could help make your private health cover more affordable. 7 BENEFITS AVAILABLE ON ultimate corporate health cover Freedom to choose your provider We pay the same dollar benefit at all recognised providers. If you choose to be treated at a Members First provider you will also have the added certainty of receiving up to 100% of the cost back for selected services on your Ultimate cover. Accident benefit If you are involved in an accident after joining and require urgent hospital treatment, the accident benefit provides up to $2,000 per person, $4,000 per policy which can be used to boost extras cover in the event that limits have been reached. Fixed fee reimbursement Ultimate Corporate Health Cover members will be reimbursed if charged a fixed daily fee by any of our contracted fixed fee hospitals.^ Gap free physio and general dental for kids* We ll cover the cost of your kid s physio and general dental until they turn 25. 12 Hospital cover What s covered Hospital cover helps pay for services that are recognised by Medicare when you are admitted to hospital. When a service is included in your hospital or packaged cover we will help pay for things including: your room, theatre fees, surgically implanted prostheses and fees charged by your doctor/s when they treat you in hospital. What s not covered There are situations where you are likely not to be covered under any of our Hospital Covers. The following are not covered by us at all (which means that we do not pay any benefits for hospital costs, prostheses or medical fees): procedures not recognised by Medicare procedures not approved by the Medical Services Advisory Committee experimental treatment respite care cosmetic surgery that is not clinically required. Surgical podiatry will be paid at minimum benefits. If during a surgical podiatry procedure a prosthesis is implanted, the cost of this will be covered up to the approved benefits prescribed in the Government s Prostheses List. Minimum Benefits For services in a private hospital which are recognised by Medicare, we will pay minimum shared room benefits and you will have your choice of doctor. These benefits would not be adequate to cover all hospital costs and are likely to result in large out of pocket expenses. For services in a public hospital which are recognised by Medicare, we will pay minimum shared room benefits and you will have your choice of doctor. If these benefits are less than the public hospital charges, you will have out of pocket expenses to pay. Staying in a Public Hospital? As a private patient in a public hospital you are entitled to choose your doctor, if they are available. For services recognised by Medicare (that is not excluded or restricted on your cover) if you elect to be treated as a private patient in a public hospital, we will pay shared room minimum benefits and benefits for prostheses up to the benefit prescribed in the Government's Prostheses List. If you choose to stay in a private room for an overnight stay, Bupa will pay a fixed benefit in addition to the shared room minimum benefit. It is important to note that in public hospitals, private rooms are generally allocated to people who medically need them. If the hospital charges are greater than the Bupa benefit, you will be required to pay the balance as an out of pocket expense. The hospital should let you know what these expenses will be before you elect to be a private patient. You should always contact us before you go to hospital. 10

13 Hospital cover Waiting periods A waiting period is the time between when you joined us and when you are covered for a service or treatment. If you receive a service or treatment during this time, you are not eligible to receive a benefit payment from us, regardless of when you submit the claim. The following waiting periods apply to hospital cover: pre existing conditions, ailments or illnesses and pregnancy related services (including childbirth) 1 all other treatments covered by your product two months. There is no waiting period for treatment you require as a result of an accident sustained after joining us. There are also no waiting periods for newborn babies provided you are on a family hospital cover and the baby is added to your membership no later than two months after their birth. Excess An excess is the amount you agree to pay upfront before a benefit is paid for overnight or same day hospital admissions. The total excess is payable once per person per calendar year, up to a maximum of twice on the membership. Each person on the membership will never pay the excess more than once per calendar year. If the total excess is not reached in a single hospital admission then the remaining balance of the excess is payable on any subsequent admission that person may have in the same calendar year. Bupa Medical Gap Scheme This is a direct billing arrangement between Bupa and your doctor/s that either eliminates or reduces out of pocket expenses for in hospital doctors fees (the gap ). See page 24 for more information. We have laid out our extras covers side by side to help you to compare what works for you. Extras are only available when combined with a level of corporate hospital cover. CORPORATE ADVANTAGE Comprehensive With Corporate Advantage you can relax knowing that you re covered to our highest level of extras cover. The real advantage here is access to a wide range of services that Medicare doesn t cover with at least 90% back at Members First providers. CORPORATE CLASSIC If you want access to a complete range of extra services and generous benefits and at least 80% back at Members First Providers then this option may be for you. BASIC CORPORATE ESSENTIALS With Corporate Essentials, you ll enjoy access to a range of key services with at least 70% back at Members First providers. 14

Annual maximums and service limits There are limits to the amount you can claim back and/or the number of times you can claim on most services. Are all extras services covered? There are instances when extras benefits are not payable including: for different services within the same service type from the same provider on the same day. For example, if you went to see an acupuncturist and then received a massage from the same provider on the same day, you cannot claim for both services when a provider is not recognised by us for benefit purposes when a third party provides a benefit (e.g. when you hold a state ambulance subscription). Our Medical Gap Scheme is a direct billing arrangement between Bupa and your doctor/s. In most instances this scheme eliminates your out of pocket expenses for in hospital doctors fees (the gap ).
