4 INTRODUCTION In 2003 the Commonwealth Department of Health and Ageing (DOHA) funded the Commonwealth Respite and Carelink Centre Southern Region (CRCCSR) to develop the Community Transport Directory for the southern region in Victoria. As there is a continuing need for current information on community transport options, the Carelink team at CRCCSR revised and updated the Transport Directory in 2009 and The southern region includes ten Local Government Areas (LGAs): Port Phillip, Stonnington, Glen Eira, Bayside, Kingston, Casey, Cardinia, Greater Dandenong, Frankston and Mornington Peninsula. There are 1,282,998 residents in the southern region, accounting for approximately one quarter (23.9%) of Victoria s total population (2011 Census data). The outer LGAs, such as the Mornington Peninsula, often face difficulties with transport, given their distance from major hospitals and medical services and poor transport infrastructure. Community transport providers are generally local councils providing fixed destination transport, service providers such as community health services providing transport for clients to their own programs, and volunteer community or church based services. The Carelink team respond to callers to the Commonwealth Respite and Carelink Centre Southern Region who are seeking information about aged, disability and community care services in the southern region. These callers report that transport issues are an increasing challenge for individuals wishing to live independently in the community. Some callers require minimal community support or assistance, but are often restricted in the community as they need transport support in order to manage their daily lives. Additional issues include: A lack of services to transport older people who may be transitioning between rural and regional aged care facilities Lack of options to assist people (who are not on packaged care) to attend medical appointments Many local councils provide local bus runs on a daily basis, but if an individual s needs are outside the run, they are left with no options Often the only option is the council s Home and Community Care (HACC) program which can at times provide personal care workers to attend and drive an individual to locations such as the local grocery store and library. The 2013 Community Transport Directory contains contact details for 24 service providers offering transport within the southern region. Section A contains information on Victorian transport subsidies and Section B contains information on the services providing transport. Information is not provided on services which only provide transport to clients of their own programs and the focus is on services providing transport to individuals rather than community groups. 6 SECTION A: VICTORIAN SUBSIDISED TRANSPORT SERVICES MULTI-PURPOSE TAXI PROGRAM The Victorian Multi Purpose Taxi Program makes it easier for Victorians with a severe and permanent disability to afford taxis by providing members with half price taxi fares. It pays up to $60 per trip and some members have a yearly limit. Members are given a card, which they have to show when they first get into a taxi. The cards cost $16.50 and are valid for six years. Travel can be for any reason, except trips that are covered by insurance or other funding, which are not covered by the Multi Purpose Taxi Program. Victorian members can get the same discount interstate for a limited number of taxi trips. : You can become a member of the Multi Purpose Taxi Program if you: Live in Victoria, and Have a severe and permanent disability, and Have a disability that means you can't use public transport by yourself. To become a member you also need to either: Use a wheelchair all the time, or Hold a department of veterans' affairs pensioner concession card or gold repatriation health card, or Hold certain pensioner concession or health care cards from centrelink, or Be able to show you have financial hardship by providing financial information. To become a member of the Multi Purpose Taxi Program, both you and your doctor have to fill in an application form and send it to the Victorian Taxi Directorate. 7 NON-EMERGENCY PATIENT TRANSPORT Non-emergency patient transport (NEPT) is available for patients who do not require a time critical ambulance response and who have been assessed by a medical practitioner. NEPT is generally for people who have clinical barriers to making their own transport arrangements using privately owned vehicles, public transport services, taxis, community transport or volunteer services. There is a range of public and private organisations involved in the provision or coordination of NEPT services, including registered private transport providers (PTPs), health services and Ambulance Victoria (AV). AV is a major provider of road and air NEPT services in the State. It subcontracts PTPs to deliver all its NEPT services in the metropolitan area and a large proportion of rural and regional Victoria. AV also provides a clinic transport service. Hospitals provide either their own NEPT services, contract PTPs or use AV to deliver NEPT. Most NEPT transfers occur between hospitals and between home and hospital. Many nursing home patients are transported by NEPT to and from specialist health appointments and rehabilitation. The majority of non-emergency transports are provided by road, with a small number undertaken by air services. Non-emergency patient transports need to be authorised by a medical practitioner. Authorisation considers whether there is a demonstrated medical requirement for ambulance transport on the grounds that: The patient requires the use of the specialised equipment contained within the vehicle, or The patient requires the skill levels and qualifications of staff on either an ambulance vehicle or on a non-emergency patient transport vehicle A patient has an illness or a disability that makes it impractical to use any other form of transport, for example, severe immobility or disorientation. : Phone: Web: Date updated: 5 th March Page 7

8 DISCOUNTED CAR REGISTRATION You can apply for a concession on your vehicle registration or six monthly car registration if you, or your spouse or domestic partner is, a Victorian resident and the holder of a: Health Care Card issued by Centrelink Pensioner Concession Card issued by Centrelink Pensioner Concession Card issued by Department of Veterans Affairs Gold Repatriation Card issued by the Department of Veterans Affairs. Date updated: 5 th March PUBLIC TRANSPORT CONCESSIONS Concession fares for Disability Support Pensioners and Carer Payment recipients Disability Support Pensioners and Carer Payment Recipients (DSP/CAR) are eligible for concession fares and free weekend travel. Free Weekend Travel Application Forms are available at staffed Metro Premium stations, the Public Transport Victoria (PTV) Hub at Southern Cross Station (near entrance at Collins and Spencer streets) or online at Free Travel Passes The PTV Pass Office issues the following free travel passes relevant to people with a disability for use on Victoria s public transport network: Access Travel Pass is for people with a significant permanent disability who travel independently on Victoria s public transport network and can demonstrate that due to their disability they cannot use the ticketing system. Scooter Wheelchair Travel Pass is for people with a permanent and severe disability who depend on the use of a scooter or wheelchair for mobility outside of their home. Vision Impaired Travel Pass is for people who are legally blind. War Veteran s Travel Pass is for Victorians who have undertaken war/peacemaking/peacekeeping service overseas and are in receipt of a Department of Veterans Affairs (DVA) gold or white card or documented evidence of ongoing or lump sum compensation from DVA for an injury/illness related to this service and have had confirmed by their treating Doctor that due to this injury/illness related to service the veteran s locomotion is limited significantly. EDA / TPI War Veterans Travel Pass is for Victorian Veterans who have undertaken war/peacekeeping/peacemaking service overseas, are holders of a DVA Gold Repatriation Health Card embossed with EDA (Extreme Disablement Adjustment) or TPI (Totally Permanently Incapacitated). : Phone: Public Transport Victoria on Web: Page 8

9 The Carer Card is issued by the Victorian Department of Human s (DHS) to eligible Victorian residents who are carers. Carer Card holders are eligible for free travel on public transport on a Sunday plus two return off-peak travel vouchers each year. Seniors Seniors using myki receive a number of benefits, including concession fares on public transport during the week and free travel on Saturdays and Sundays in zones 1 and 2 in metropolitan Melbourne and on regional town bus services. To be eligible for a free seniors myki card, registration for the Victorian Seniors Card is required. Speak and listen users phone , then ask for (13 myki) In person: PTV Hub at Southern Cross Station (near entrance at Collins and Spencer streets) Web: Date updated: 5 th March TRAVELLERS AID AUSTRALIA Travellers Aid Access (TAAS) Travellers Aid Access (TAAS) promotes independence, autonomy and inclusion by assisting seniors or travelers with a disability with their personal care needs. They maximise peoples' ability to participate in day-to-day activities, such as employment, education, volunteering, social and community events. No referral or appointment is necessary. Friendly, experienced Client Support Officers can provide: Meal assistance Personal care Communication assistance for people with speech impairments, or who require assistance with filling out forms Fully accessible bathroom facilities with ceiling hoist and adult change table Wheelchair/scooter recharging facilities Accessible public internet computers with JAWS software available. Clients friends, family, and personal support workers are also welcome. 10 Medical Companion Trained Medical Companion (MCS) volunteers meet medical travellers at Flinders Street Station and Southern Cross Station and accompany them by public transport to and from their medical appointment (includes dental / health care) at all hospitals, specialists and medical clinics in central Melbourne (the CBD). They assist people able to travel independently who are unfamiliar with Melbourne, or feel anxious using public transport alone. The service operates Monday to Friday 8am 6pm. Weekend or out-of-hours services can be requested depending on volunteer availability. The service is: Free Available to regional and metropolitan travellers Must be booked in advance. A minimum 48 hours notice is required Travellers must purchase their own public transport tickets. The Medical Companion will cover the volunteer's transport costs. Volunteers are not trained personal care workers. Trained personal care workers are on site at Travellers Aid Australia Flinders Street and Travellers Aid Australia Southern Cross to provide personal care when travellers are at the station. Volunteers accompany travellers in wheelchairs, as long as the traveller has a support person who assumes responsibility for loading, unloading and pushing the wheelchair when travelling. MCP volunteers are not required to push wheelchairs or conduct any heavy manual handling, as they have not been trained in manual handling procedures. : Anyone living in Victoria and travelling to a medical appointment in central Melbourne can use the service as long as they are able to adhere to the following: Must be departing from Southern Cross or Flinders Street Station Able to travel independently Able to walk unassisted or negotiate at least a few steps with the help of a walking frame Able to comfortably enter and exit a taxi, tram or train If travelling by taxi, able to safely secure themselves via a seatbelt (exemptions apply, a certificate is required) Over 16 years of age, unless accompanied by a parent or guardian Will not require medical treatment, intravenous therapy, oxygen or monitoring whilst using the service. : Phone: between 7.15am 9.45pm (7 days per week) Book online at least 48 hours in advance of your appointment at: If deaf or hard of hearing, please scs@travellersaid.org.au for a booking form or call the relay service on before calling to make your booking. 11 DEPARTMENT OF VETERANS AFFAIRS (DVA) REPATRIATION TRANSPORT SCHEME The DVA Repatriation Transport Scheme operates in two ways: the Veteran organises their own travel and claims reimbursement of costs from DVA or DVA arranges travel and pays the transport provider (taxi) directly. Eligible veterans, war widows/widowers (generally gold or white card holders, but check with DVA regarding individual eligibility) can claim travelling expenses under the Repatriation Transport Scheme (RTS) relating to: Visits to a health provider for medical treatment A disability claim An invalidity income support claim Treatment under the Australian participants in British nuclear tests (Treatment) Act The Repatriation Transport Scheme is comprised of a number of elements including the reimbursement of: Transport Accommodation Meals Medically required attendants. DVA may assist with travelling expenses for use of: A private vehicle Public or community transport Vehicle ferries Parking Road tolls Taxis or hire cars (where this is the most suitable and economical mode of transport) Booked car services Air travel (where this type of transport is necessary) Travel with a medically required attendant Ambulance travel Accommodation and meals. The Veteran will generally make their transport arrangements. However, in certain circumstances, transport (e.g. a booked car) may be arranged by DVA. DVA may arrange transport under the Booked Car with Driver Scheme (BCWD) for travel to approved treatment locations. Under this scheme DVA will arrange a suitable vehicle to transport entitled persons to their appointment on time. Veterans can arrange BCWD or have their health provider or nominated representative arrange transport on their behalf by contacting DVA directly or accessing the online transport booking system. : Phone: , or for country callers. 12 CENTRELINK MOBILITY ALLOWANCE You may be eligible for Mobility Allowance if you: Are aged 16 or more and Cannot use public transport without substantial assistance because of a disability, illness, or injury, and Need to travel to and from your home to look for work, do paid or voluntary work, or participate in study or training. Note: You can still qualify for Mobility Allowance if there is no public transport where you live. To be eligible for the standard rate of Mobility Allowance, you must also: Be undertaking vocational training, voluntary work, paid work, independentliving / life-skills training or any combination of these for at least 32 hours every four weeks on a continuing basis or Have an agreement to look for work through an employment services provider (e.g. Job services Australia or disability employment services) or Be participating in a disability employment services disability management service or Be receiving Newstart allowance, youth allowance or Austudy and satisfy the activity-test requirements associated with these payments. You may be eligible for the higher rate of Mobility Allowance if you are: Receiving Disability Support Pension, Parenting Payment, Newstart Allowance or Youth Allowance (job seeker) and Working for at least 15 hours a week on wages that are at or above the relevant minimum wage and either Undertaking job-search activities for work of at least 15 hours a week on wages that are at or above the relevant minimum wage, under an Employment Pathway Plan or agreement with an Employment s Provider (e.g. Job s Australia or Disability Employment s), or Working for at least 15 hours a week on productivity-based wages under the Supported Wage System. : Centrelink on , visit a Centrelink office or Date updated: 8 th March COMPANION CARD The Companion Card is issued to people with a significant, permanent disability, who can demonstrate that they are unable to access most community activities and venues without attendant care support. Attendant care type support includes significant assistance with mobility, communication, self-care, or learning, planning and thinking, where the use of aids, equipment or alternative strategies do not enable the person to carry out these tasks. It does not include providing reassurance, social company or encouragement only. The card contains a photograph of the cardholder and can be presented when booking or purchasing a ticket at events and venues, provided the cardholder requires attendant care support in order to participate at that particular activity. Participating organisations will issue the cardholder with a second ticket for their companion at no charge. Page 12

13 To be eligible to apply for a Companion Card, applicants must demonstrate that they meet all of the following eligibility criteria: Be a permanent resident of Australia, residing in Victoria and Have a significant disability and Due to the impact of their disability are unable to participate at most community venues and activities without attendant care type support and The need for this level of support will be life-long. Date updated: 8 th March VICTORIAN PATIENT TRANSPORT ASSISTANCE SCHEME The Victorian Patient Transport Assistance Scheme (VPTAS) subsidises the travel and accommodation costs incurred by rural Victorians and if appropriate, their escorts, who have no option but to travel a long distance to receive approved medical specialist services. To be eligible for assistance patients must meet all of the following criteria: Be a Victorian resident Live in a Department of Health designated rural region (Barwon-South, Gippsland, Grampians, Hume, Loddon-Mallee regions). Is receiving specialist medical treatment under specialty treatment codes , 102 or 115 from an approved medical specialist service registered with Medicare Australia and Need to travel more than 100 kilometres one way or on average 500 kilometres per week for a minimum of five consecutive weeks. Travel assistance includes: Subsidies for private car travel (a reimbursement of 17 cents per kilometre is paid where a private vehicle is used) Full concession fare reimbursements for public transport Air travel only if the journey exceeds 350 kilometres one way and a commercial flight is used Taxi travel only to the nearest public transport and only when a patient has no other means of transport available. Accommodation assistance is paid at a maximum of $35.00 per night, plus Goods and s Tax (GST) for a patient and an approved escort staying in commercial accommodation. To apply for VPTAS, all patients are required to complete a VPTAS Claim Form. The form requires the details of the approved medical specialist services to be verified by the approved medical specialist or an authorised representative. The specialist or authorised representative must also endorse the need for accommodation and the need for an escort. Page 13

Passengers will be invoiced at the end of the month of their journey. The Network Coordinator will advise of transport availability within 3 days but normally within 24 hours. Where possible, the Network will attempt to provide services for last minute requests. The preferred method of contact is via and bookings can be made online through their website. Carers and children can usually travel in the car, as long as this is discussed with the Transport Co-ordinator prior to travel.
