1 NHS rth West Transparency Project Reduction of Pressure Ulcers and Falls - How have we been doing? Patient and Staff Experience- What have they been saying? Liverpool Heart and Chest Hospital is one of the largest specialist cardiothoracic hospitals in the UK providing world class heart and lung services. For the 6 th Year we have been rated as top in the UK for overall patient care in the CQC inpatient survey. We are one of a number of rth West hospitals who want to be open and transparent with our patients. This is how a modern NHS hospital should be open and accountable, to the public and patients, driving improvements in care. As a member of the Transparency Project we continue to work with patients and staff to further reduce the harm that patients sometimes experience when they are in our care, and have made a commitment to publish a set of patient outcomes, patient experience and staff experience measures. Each month we collaborate with colleagues to optimise what we have learnt and as a result have made some changes to the Transparency collection template for April: Two extra questions have been added to the patient experience questionnaire, which is answered by 10 patients in the same ward whenever a pressure ulcer of grade (stage) 2 or above is recorded or a patient has fallen and suffered moderate or severe harm. The new questions are: o On reflection, did you get the nursing care that mattered to you? o If a friend or relative needed treatment, would you recommend this ward? In February Liverpool Heart and Chest Hospital cared for 1263 patients 0 patients suffered a fall in our care (rated moderate and above) 5 patients suffered a pressure ulcer in our care( Grade 2 and above)

4 Patients were very happy with the Nursing care provided to them We have a very strong safety focussed culture. As a result of this for the past two years we have set up a very positive reporting system and backed this up by appropriate training and education to allow staff to recognise every incidence of pressure ulcer irrespective of severity. This commitment to safety is a key component in ensuring our patients receive the best care possible. What our Nurses said about the care they provided: Statement 1 - I would recommend the ward as a place to work Statement 2 - If a friend or relative needed treatment, I would be happy with the standard of care provided by the ward 81% 78% 19% 22% Statement 3 - I am satisfied with the quality of care I give to Patients/Service Users 85% 15% In 2010, we launched a vision for how patients and families should experience care at our hospital from April 2013 onwards. Through this we set out an ambitious plan and we are implementing a number of improvements such as releasing more of the ward managers time to support staff and quality assure care, introducing regular comfort checks for patients and introducing a patient and family centred care model What did we learn about the care we have provided to patients?

Original risk assessment correct? Falls PU's Once again we achieved 10 compliance against the first four Indicators. Our focus on safety and quality means that we continually learn from every patient experience. By collecting this data, we can continue to make improvements so that even minor harm as a result of pressure damage can be reduced and avoided You said-we did! Transparency study supports our Patient Experience Vision by actively listening to our patients and their families and acting upon what they tell us. It also allows us to test out new ideas before we implement them. For the past two months we have focussed on the introduction of a Skin Inspection Record and care plan for all patients in our hospital. This enables the nurses to look out for early warning signs of skin deterioration and to put in early appropriate measures such as the use of pressure relieving cushions and mattresses to prevent any pressure ulcers developing.
