1 Instructions for Completing/Updating TFS Online Forms Please use the following instructions to help you enter or update your student and family information online. This information includes important household, emergency contact, medical, permissions and consents, and more. It can be accessed by you and updated at any time during the year should something change. Accessing Online Forms for the First Time Click on this link (or the one that was provided in the online forms you received) and log in using your Parent Portal username and password. If you are a new parent, this is the and password you created when completing the online application process. You will see the name(s) of your student(s) in the list below the welcome message. Click on the Select link beside the desired student, then click on the large Go to Forms button that appears directly below. You will be taken to a Welcome page. Please take a moment to read the important information on how to complete the online forms, then click on Save and Next when you are ready to proceed. Student Information In the Student Information section, please enter or verify your student s preferred name and enter his/her cell phone number, if applicable. When finished, click on Save and Next.
2 Household Update It is vital that our contact information for your student s household(s) and each parent/guardian be accurate so that we can contact you by mail, phone or . Household and parent contact information (address, , phone number) feeds directly into the School Directory so that the school, faculty and other parents can reach you. All additional information is confidential. In the Household section, enter or verify that your household information is accurate and complete. Please pay close attention to phone numbers and addresses, and be sure that Publish in Web Directory and Publish in Print Directory are selected so that faculty and other community members can find you (you may deselect these options as well, but doing so will prevent your information from showing up to others). In the Parent/Guardian Personal Data sections, enter or verify that all applicable information fields are complete and accurate. Emergency Information It is even more vital that we have accurate emergency information for your student, including emergency contacts, doctor and dentist contact information, and medical insurance information should your child need emergency treatment when we are unable to reach you.
3 In the Emergency Contacts sections, please enter or verify that all emergency contact information for up to three emergency contacts is complete and accurate. In the Physician/Dentist Information section, please enter or verify that all contact information for your doctor and dentist is complete and accurate. In the Insurance Information section, please enter or verify that information on your insurance company and policy and group numbers is complete and accurate. Medical Information The school relies on complete and up to date medical information on your student. The school nurse uses this information as needed in order to provide treatment of minor injuries, to assess illnesses, and to access the permissions needed to administer over the counter and prescription medications to your student as required. This information also accompanies faculty chaperones and coaches to field trips and away sports games, so that accurate treatment for your child can be sought in an emergency. The first portion of the form provides a list of medical alert conditions, medical history information, and allergies pertinent to your student. There is also a place to indicate your preference of hospital should emergency treatment for your student be required.
4 In the ALERT section, please click the radio button next to any of the conditions that affect your student. Use the text boxes to the right to enter any details that would be useful to the school nurse. In the Allergies section, please click the radio button next to any of the allergens that affect your student. Use the text boxes to the right to enter any details that would be useful to the school nurse. In the Hospital section, please click the radio button next to your preference of local hospital for your student should emergency treatment be required, or enter an alternate choice in the Other box. The second portion of the form provides space for you to share additional information about your student s medical and psychiatric history, followed by a treatment release form and a medical release form that will allow school staff to administer certain medications/treatments to your student and seek emergency treatment should you not be able to be reached.
5 Click any of the boxes that apply to your student (previous operations or serious illnesses, psychiatric treatment, disabilities or chronic illnesses) and provide details in the text boxes below. In the Treatment Release Form section, be sure to click on the permission box that grants the school permission to seek emergency treatment for your child in the event that you cannot be contacted. In the Medication Release Form, please click the radio button next to the medication types you will allow the school to provide to your student, in accordance with the guidelines provided. Be sure to click on both permissions boxes, one that grants the school permission to administer approved medications/treatments, and another that releases the school from any liability related to such medication/treatment administration. I mmunization Records Please note that you are REQUIRED by state law to provide an up to date copy of your student s immunization records to the school. This information may not be provided online and must be received by the school nurse prior to the student s first day of school. Permission Forms
6 The school needs parent/guardian permission for students regarding photo consents, field trip participation, certain car driver/passenger consents, and off campus permissions for seniors and second semester juniors. In the Student Photographs section, please read through the information and indicate your preferences by clicking on the desired Yes/No radio buttons. In the Field Trip Consents section, please read through the information and indicate your preferences by clicking on the desired Yes/No radio buttons.
7 In the Student/Driver Passenger Consents section, please read through the information and indicate your agreement by clicking on the desired Yes/No radio buttons. Parents of seniors and juniors only: In the Permission for Off Campus Privileges section, please read through the information and indicate your preferences by clicking on the desired Yes/No radio buttons. Note that with this section of the Permissions Form, both student and parent must read and indicate agreement. Acceptable Use Policy Students are expected to use technology on campus in accordance with the school s Acceptable Use Policy.
8 Please take a moment to read over the policy with your student before clicking on the Yes radio button to indicate that both parent and student have read and agree to the policy s terms. Submit Online Forms Data Once you have completed and saved information in each section of the online forms, a red box will appear. To complete the online forms process, you must click on the Submit Forms button. A thank you screen will appear, indicating that your forms have been submitted. Save/Print Out PDFs of Your Online Forms Once you have submitted your form data, you can save and/or print out a PDF version of each forms section for your records by clicking on the PDF icon next to the desired section name at the bottom of the thank you screen. Completing Forms for Another Student Once you have clicked on Submit Forms for your first student, and the thank you screen has appeared, you can click on the To complete a form for another child, please click here link to begin the process again for another student.
9 Thank you for taking time to provide/update your student and family information online. You will be asked to do this before the start of each school year at TFS. Please note that you can update any of this information throughout the year via the password protected Parent Portal, using the Household Information tab at the top left of the screen to access each section of data, then using the options at the left to access various form screens to update.
