2 Benefits Open Enrllment will be frm Oct. 28, 2013 thrugh Nv. 8, 2013, with electins effective Jan. 1, This is the ne time each year when yu can make changes t yur medical, dental, visin and flexible spending accunt electins withut experiencing a qualifying change in status such as marriage, divrce, r the birth/adptin f a child. There will be a number f imprtant changes t SAS benefits in 2014 including: Enhanced dental and visin cverage. Flexibility in cverage electins. Mderate changes t the medical plan. This bklet prvides a cmprehensive verview f SAS benefit changes which will take effect in January This bklet als cntains detailed infrmatin intended t help emplyees better understand the many cmplexities f health care refrm and their impact, bth immediate and lng-term, t SAS benefit plans. Details f the changes are utlined in the 2014 Highlights and Changes sectin f this bklet. Please review the infrmatin t understand the changes and hw they affect yur health care benefits and premiums. If yu d nt actively enrll r make changes during Open Enrllment, yur current benefit electins will carry frward t 2014 with the fllwing exceptins: Flexible Spending Accunts (FSAs) If yu wish t participate in a Health Care and/r Dependent Care FSA fr 2014, yu must actively enrll r re-enrll during Open Enrllment. FSA electins frm 2013 will nt carry ver t the 2014 plan year. Wrking Spuse/Dmestic Partner Premium If yu currently waive the $100 mnthly Wrking Spuse/Dmestic Partner Premium and are eligible t cntinue waiving the premium fr the 2014 plan year, yu must submit a new waiver during Open Enrllment. Infrmatin abut the premium can be fund n the Benefits Open Enrllment website at sww.sas.cm/hrweb/ OpenEnrllment.jsp. Yu can cmplete the waiver nline while making yur Open Enrllment selectins thrugh Wrkday. If yu d nt have access t a cmputer, cntact the Benefits Department at t request a paper waiver frm. Starting in 2014, we will be making changes t ur medical, dental and visin plan benefits and enrllment ptins. These changes are being made t: Cntinue leading in cmpetitive emplyer subsidized benefits. Manage SAS health care csts at a sustainable level. Avid triggering the excise (Cadillac) tax under health care refrm. Remain cmpliant under all federal and state laws and regulatins. We are asking yu t d yur part by living a healthy lifestyle, aviding unnecessary health care csts and being a thughtful cnsumer by evaluating ptins and the assciated cst when yu need health care services and treatment. SAS Rising Health Care Csts SAS health care plan expenses have increased by ver 21% in the last tw years and are estimated t increase by an additinal 12% in 2014 (ttaling $116 millin). In cmparisn, the natinal health care trend is estimated between 6% and 7% fr 2013 and SAS has dne its best t shield ur emplyees frm the full extent f the cst increases by cntinuing t pay 87% f health care premiums. SAS will cntinue t be vigilant in ur effrts t balance csts while maintaining the cmprehensive benefit fferings that have helped t make SAS an emplyer f chice. Key Drivers f SAS Health Care Cst Higher pricing Includes the rising cst f prescriptin drugs, prviders, hspitals, medical supplies and advances in medical technlgy. Aging wrkfrce As we age, we generally need mre care and spend mre n health care services. 1

3 Cmprehensive health care benefits with a lw emplyee cst share SAS has a histry f lw cpays, n in-netwrk cinsurance and very lw deductibles. When cnsumers have little r n ut-fpcket expense, there can be lack f awareness n the impact t the cmpany s bttm line. High cst claims These claims typically reflect very serius medical cnditins and treatments. SAS high-cst claims increased frm $10 millin in 2011 t $19 millin in Health Care Refrm In March 2010, the federal Patient Prtectin and Affrdable Care Act (PPACA), mre cmmnly knwn as Health Care Refrm, was signed int law. It implements extensive changes and cmprehensive health insurance refrms that will rll ut ver the next fur years and beynd. These changes significantly impact bth individual and emplyer-spnsred health plans. While health care refrm shuld prvide access t insurance fr everyne in the United States, it cmes at a cst. SAS is nt insulated frm these csts, and like many emplyers, must make changes t ur health care benefits t help ffset the csts f health care refrm cmpliance. SAS health care csts have increased by an estimated $5 millin annually as a result f the implementatin f health care refrm. Additinally, the annual impact is estimated t increase t $6.5 millin fr the plan years. Unless we are able t manage ur health care csts at a mre sustainable level, SAS will trigger a 40% emplyer excise (Cadillac) tax starting in The main drivers f SAS increased health care cst related t health care refrm are: 2011 emplyer mandate t cver adult dependents Emplyers are required t ffer cverage t adult dependents up t age 26 regardless f student r marital status remval f essential benefit limits This prvisin required emplyers t remve annual and lifetime limits fr essential health benefits extensin f preventive services under wmen s health Emplyers are required t cver additinal preventive services fr wmen at a $0 cst share. This includes wellwman visits, varius types f screenings and FDA-apprved cntraceptin methds transitinal reinsurance fees Health care refrm requires grup health plans t pay a per-cvered-member fee in rder t help t fund health care exchanges pening in excise tax n high value health plans By prviding cmprehensive benefits, SAS is expsed t a 40% federal excise tax n high cst plans knwn as the Cadillac tax. Starting in 2018, SAS may be required t pay millins in taxes if the cst f ur health care passes certain threshlds. Althugh the excise tax des nt start until 2018, SAS must begin t make changes nw s we dn t trigger the tax in cming years. Fill yur prescriptins at the HCC Pharmacy On average, ingredient csts fr prescriptins filled at the HCC Pharmacy are 20% less than at retail. Als, the HCC Pharmacy is the nly pharmacy that can fill a 90-day prescriptin in persn, ffering cnvenience as well as lwer csts fr yu and the cmpany. 4 2014 Open Enrllment Highlights and Changes 1. Health Care Cverage Enrllment Medical, Dental and Visin May Be Elected Separately Starting in 2014, emplyees may enrll separately in medical (includes prescriptin), dental and visin cverage, r any cmbinatin f the three. Eligible dependents may be enrlled in any cverage ptin in which the emplyee is enrlled. Fr example, an emplyee may enrll in Emplyee Only medical cverage, Emplyee + Family dental cverage and Emplyee + Spuse visin cverage. Mnthly premiums will be determined by the type f health care enrllment (medical, dental and/r visin) and the level f cverage selected (Emplyee Only, Emplyee + Spuse, Emplyee + Family). Mnthly premium infrmatin is utlined belw. If n actin is taken during pen enrllment, emplyees will be autmatically enrlled in the same level f medical, dental and visin cverage in effect n Dec. 31, Full-Time Emplyee Mnthly Premium Cntributins The full-time emplyee mnthly cntributins fr medical, dental and visin cverage are listed belw. Each cverage ptin will have a separate mnthly premium cntributin based n the level f cverage elected. The $100 mnthly Wrking Spuse/Dmestic Partner Premium will nly apply t medical cverage. Medical Benefit Changes Starting in 2014, SAS will be asking emplyees t share a little mre in the cst f medical care by implementing a lw in-netwrk cinsurance and by increasing deductibles and ut-f-pcket maximums. These changes are being implemented t help keep premiums lwer fr everyne cvered under the plan and t encurage cvered members t be mre cnscientius cnsumers when accessing health care services. Althugh the verall cmbined health care premiums (medical, dental and visin) will increase in 2014, SAS will cntinue t absrb the majrity f the ttal cst f health care by paying 87% with emplyees paying 13%. This cst share distributin remains unchanged frm T help emplyees make better-infrmed health care decisins, Blue Crss Blue Shield has develped an nline Cst Estimatr tl. The tl allws emplyees t: Find and review quality in-netwrk prviders. Cmpare the cst f diagnstic tests r treatment ffered by varius prviders and facilities. See an estimate f their ut-f-pcket expenses. By accessing data n quality, cst, and ut-f-pcket expenses, emplyees are empwered t cnsult with medical prviders when determining the health care services they need. The limit f ne cranial band per lifetime remains unchanged. Hearing aids will be cvered as fllws: Birth thrugh age 22: In-netwrk: Cvered at 95% after the deductible. Out-f-netwrk: Cvered at 80% after the deductible. Replacements available every three years. Ages 23 and up: In-netwrk: Cvered at 95% after the deductible, up t a $1,500 maximum (per ear). Out-f-netwrk: Cvered at 80% after the deductible, up t a $1,500 maximum (per ear). Replacements available every five years. What are cpays, deductibles, cinsurance and ut-f-pcket maximums? Cpay Fixed payments fr primary care ($20), specialist ($30), urgent care ($30) and emergency rm ($100) visits. Cpays d nt apply t the annual deductible. Annual Deductible The fixed amunt yu have t pay each plan year tward certain medical expenses befre SAS begins paying. An individual deductible is the amunt paid fr ne cvered member, and a family deductible is the amunt paid fr all cvered members in the family. Cpays and mnthly premiums d nt apply t the annual deductible. Cinsurance A way emplyees and SAS share the cst f care. Fr certain in-netwrk services, SAS pays 95% f the cst nce the deductible has been met. Fr Example: A cvered member with a $250 deductible and 95% cinsurance is respnsible fr paying fr the first $250 f medical care cst, then 5% f the subsequent csts until reaching the annual ut-f-pcket maximum. Out-f-Pcket Maximum The limit n the ttal deductible, cpays, and cinsurance a cvered member will pay fr eligible expenses under the SAS Health Care Plan during a plan year. Dental and Visin Benefit Changes T cntinue leading in cmpetitive emplyer-subsidized benefits, we are pleased t annunce enhancements t ur dental and visin benefits starting in Dental Benefit Enhancements The dental maximum benefit is increasing frm $1,800 t $2,000 per patient/per year. The maximum benefit applies t all cvered dental services with the exceptin f rthdntia and TMJ treatment. Cverage fr majr services is increasing frm 50% t 60%. Majr services include, but are nt limited t: inlays, nlays, crwns, gld fillings, bridgewrk, implant services and repair r re-cementing f crwns. 7 5. ID Cards Medical All cvered emplyees will receive new Blue Crss Blue Shield medical ID cards with up-t-date benefit infrmatin fr the 2014 plan year. Yu shuld receive yur new ID cards prir t Jan. 1, Prescriptin Cvered emplyees will nt receive new prescriptin ID cards frm PharmAvail unless dependents are added r drpped frm medical cverage during Open Enrllment. If yu add r drp a dependent, yu will receive a new set f ID cards prir t Jan. 1, Dental and Visin Emplyees will nt receive separate dental and visin ID cards. Dental and visin eligibility infrmatin will cntinue t be listed n the back f the prescriptin ID cards. FSA Debit Benny Cards Only new enrllees t the Health Care FSA will receive a new set f Benny cards fr Current enrllees may use their existing cards fr the 2014 plan year. A new set f cards can be requested frm Flres & Assciates at a cst f $5.00. Flexible Spending Accunt (FSA) Infrmatin SAS ffers tw Flexible Spending Accunts (FSAs): The Health Care FSA (fr full-time and part-time emplyees) and the Dependent Care FSA (full-time emplyees nly). An FSA allws yu t pay fr eligible ut-f-pcket health care and dependent care expenses with pretax dllars. By spending pretax dllars, yu reduce yur taxable incme by the amunt yu cntribute and therefre pay less federal incme and Scial Security taxes fr the year. Yur state and lcal incme taxes may als be reduced which may give yu a greater net incme. Hw an FSA Wrks Participatin in the Health Care and/r Dependent Care FSA is ptinal. If yu decide t participate, yu shuld estimate yur utf-pcket health care and/r dependent care expenses fr the upcming year t help determine hw much yu shuld cntribute. Nte that the changes t the 2014 Medical Plan may increase yur ut-f-pcket expenses. FSA cntributins will be deducted frm each paycheck n a pretax basis in equal increments and credited t yur FSA ver the curse f the year. When yu incur an eligible health care r dependent care expense, yu pay fr the expense then submit a claim fr reimbursement frm yur FSA. If yu are enrlled in the Health Care FSA, yu will als have the ptin f using an FSA Electrnic Payment Card which is described in further detail belw. Health Care FSA Yu may cntribute up t $2,500 per year in a Health Care FSA t reimburse yurself fr eligible ut-f-pcket medical expenses incurred by yu r yur dependents. Examples f eligible expenses include: Medical and prescriptin cpays and cinsurance. Deductibles and amunts ver the medical plan s Allwed Amunts. Glasses, cntacts and visin crrectin surgery (i.e., LASIK). Out-f-pcket dental expenses (excluding csmetic prcedures) and rthdntia payments. Participants in the Health Care FSA receive an electrnic payment card (debit card) called the Benny Card. The Benny Card may be used t pay fr eligible ut-f-pcket expenses at health care prvider ffices, medical clinics, hspitals, dental ffices, hearing and visin care centers and pharmacies. This cnvenient payment ptin is designed t be used in place f the traditinal paper claim, saving yu time. Hwever, yu may still submit traditinal paper claims fr reimbursement, if preferred. Dependent Care FSA Yu may cntribute up t $5,000 per year ($2,500 if married and filing separately) in a Dependent Care FSA t reimburse yurself fr child care expenses prvided fr yur dependents. The Dependent Care FSA is nt t be used fr reimbursement f dependent medical expenses. Reimbursements are made nly up t the amunt that yu have paid int yur accunt at any pint during the year. Examples f eligible expenses include: Daycare and preschl tuitin. Befre- and after-schl care. Nanny services. Day camps. Track-ut care (year-rund schls). Adult daycare services. Dependents must be under the age f 13, r f any age that lives in yur husehld and is incapable f self-care. 8 Using the Dependent Care FSA if Yu Als Receive a Daycare Subsidy r Daycare Prvided by SAS The Dependent Care FSA has an annual maximum cntributin f $5,000. Any dependent care benefits that yu receive frm SAS (thrugh the value f n-site daycare, the reginal child care subsidy r the cmmunity child care subsidy) will reduce yur $5,000 annual maximum cntributin n a dllar-fr-dllar basis. If the value f the emplyer-prvided subsidy is less than $5,000, yu may elect t cntribute the difference between that value and the $5,000 maximum t yur Dependent Care FSA. Fr mre infrmatin n SAS subsidized daycare, including guidance n hw t determine if yu are eligible t cntribute t the Dependent Care FSA, yu may review the Flexible Spending Accunt mem lcated at finance.na.sas.cm/tax. Imprtant Infrmatin Abut Flexible Spending Accunts Yu shuld carefully cnsider the amunt yu elect t cntribute n an annual basis due t the fllwing FSA rules, based n IRS requirements: N Changes: In mst cases, changes t yur FSA electins can nly be made during Open Enrllment. Midyear changes are nly allwed if yu experience a Qualifying Change in Status. Separate Accunts: The Health Care FSA and the Dependent Care FSA are tw separate accunts. Yu cannt use yur health care cntributins t reimburse yurself fr dependent care expenses r vice versa. Use It r Lse It: Any unused amunts remaining in yur accunt at the end f the year will be frfeited. Yu will have until March 31 f each year t file claims fr expenses incurred n r befre Dec. 31 f the previus year. Verify yur 2014 electins and premiums and print yur cnfirmatin statement fr yur recrds. This is the nly cnfirmatin yu will receive t verify yur enrllment electins. 7

9 Changes Yu Can Make During Open Enrllment Yu may make the fllwing changes t yur health care cverage during Open Enrllment: Select desired cverage fr medical, dental and visin cverage. Beginning in 2014, yu may separately elect cverage in the medical (includes prescriptin), dental and visin plans.
