1 MBA Studies, Chinese University of Hong Kong Sample Application 1. Follow all the instructions listed on this sample application and on the CUHK website. 2. After you submit the application online to CUHK, print the application and it to UCEAP. 3. Keep a copy for your own records. Note: Use Internet Explorer. 2 Do not click the submit button until you have completed all sections of the application. Your personal details must match your passport. Do not use nick names. Your citizenship is the same as the country that issued your passport. It is not your ethnicity. If applicable. A permanent or parental address that you will have access to prior to departing the U.S. No P.O. Boxes. Your student visa will be mailed to this address. Click the next button at the bottom of the page when you are ready for the next section.

3 IMPORTANT! Click Exchange Student. If you select study abroad student, you will be charged additional fees by CUHK and required to submit additional paperwork. Select your class level for the period you will be in Hong Kong. Year 3 = Junior Year 4 = Senior Year 5 = Graduate Student Period of Study (Must match your UCEAP application): Fall Program = IASP: SEP - DEC (1 st Term) Year Program = IASP: SEP - MAY (Full Academic Year) Spring Program = IASP: JAN - MAY (2 nd Term) MBA Fall Program = MBA: SEP - NOV (1 st Term) Level of Study Undergraduate = Associate Undergraduate Student MBA Program = Associate Postgraduate Student (Taught)

4 Click Yes. Select U.S.A., University of California, (campus). UC Systemwide Office of Education Abroad Program 6950 Hollister Ave, Suite 200 Goleta CA USA Your UC major. Your UC minor. IMPORTANT! This is your current cumulative UC GPA. Do not use transfer GPAs. This is your expected date of graduation. It must be after you have completed your UCEAP program or later.

5 List all other postsecondary institutions (i.e. community colleges, other universities than UC) in which you have earned academic credit. You may enter multiple institutions by clicking on Insert. Select your native language first. Then, click insert if you have studied other languages to add them to the list. Indicate travel outside of the U.S.

6 Enter your work experience. Click Insert to add additional employers.

7 Write a 300 word or less statement of purpose here.

8 IMPORTANT! Do not use the UCEAP Course Catalog to complete this section. Refer to the courses on the CUHK website. At CUHK, undergraduate courses are coded 1-4 while postgraduate courses are coded 5 9. The first numeral in the course code of undergraduate courses stands for the level of study. Normally, courses in the 1000 level are lower division courses while courses in the 3000 and 4000 levels are upper division courses. For courses in the 2000 level, the classification of division varies according to policies of individual departments. You must select at least four (4) courses that are offered the term you are at CUHK. Click insert to add more courses. Note: Economics courses require a strong economics and math background. Do not select economics courses unless you are an economics major. This section is NOT for course registration. It is to help CUHK understand your area of interest. Information about course registration will be sent to students about month prior to the start of the program.

9 UC exchange students with at least a 3.0 cumulative UC GPA are not required to submit recommendation letters. However, you must turn in the Application Record Sheet to UCEAP to confirm your nomination.

11 X MBA applicants select Not applicable - I am applying as postgraduate student at CUHK. CUHK does not provide housing for MBA students. Contact sandrachan@baf.cuhk.edu.hk to request information about off-campus housing options. Don t forget to: Click Submit. Once you successfully submit your application online, you will be prompted to complete a declaration. You will also receive a confirmation message from CUHK. Print a copy of the completed application and it to UCEAP. Once you complete your CUHK application and submit it online, you will be transferred to your My Application page. This is where you can access your Application Record Sheet (See PDC Item 5). This is the last section. Click Previous to edit any information.
