5 Principles fr Victrian alchl and drug treatment Intrductin Accrding t the C-ccurring Centre fr Excellence in the United States, a principle is a basic generalizatin that is accepted as true and that can be used as a basis fr reasning r cnduct (2007, p.1). Principles serve t guide the design f systems and the implementatin f service interventins, and infrm the ver-arching expectatins f the wrkfrce. Fr the alchl and drug treatment sectr and the health sectr mre bradly, a set f evidence-based treatment principles help t infrm service system imprvement. As such, treatment principles are particularly valuable in times f change. In the cntext f current departmental initiatives, including Alchl and Drug Treatment Refrms, crss-gvernment wrk n the Whle f Victrian Gvernment Alchl and Drug Strategy, and the develpment f a new Alchl and Drug Wrkfrce Develpment Framewrk and Implementatin Plan, it is timely t identify the principles f effective alchl and drug treatment. The purpse f this draft cnsultatin paper is t present an verview f the principles currently infrming alchl and drug treatment in Victria and ther jurisdictins, and t seek feedback n hw these might infrm the develpment f a Victrian set f alchl and drug treatment principles. This wrk is being led by the Sectr Quality team f the Mental Health, Drugs and Regins divisin f the Department f Health (the Department), in assciatin with alchl and drug sectr peaks. The cntributins f the Victrian Alchl and Other Drug Assciatin (VAADA), the Assciatin f Participating Service Users (APSU), Harm Reductin Victria (HRV), and ANEX are gratefully acknwledged. Cnsultatin paper This cnsultatin paper utlines the current Victrian alchl and drug cntext, identifies cntemprary evidence-based alchl and drug and ther treatment principles, and pses a set f verarching principles and cncepts fr cnsideratin. The paper has been infrmed by: A brief literature review A scping wrkshp held with alchl and drug service prviders and peaks at the March 2011 Service Prvider s Cnference Sectr feedback is nw sught n the alchl and drug treatment principles cnsultatin paper. All feedback shuld specifically address the questins in this cnsultatin paper and be prvided by Friday 5 Oct Sectr feedback will be crdinated by VAADA. Cnsumer, carer and family feedback will be crdinated by APSU. VAADA and APSU will cllate feedback n the cnsultatin paper and prvide a written respnse t the Department. The examples f alchl and drug principles belw prvide a useful starting pint fr cnsidering new Victrian alchl and drug treatment principles. United Natins Office n Drugs and Crime and Wrld Health Organisatin (2008) The United Natins Office n Drugs and Crime (UNODC) in partnership with the Wrld Health Organisatin (WHO) released a discussin paper Principles f Drug Dependence Treatment in This paper utlines nine precnditins fr evidence-based and respnsive alchl and drug treatment systems: 1. Availability and accessibility f drug dependence treatment 2. Screening, assessment, diagnsis and treatment planning 3. Evidence-infrmed drug dependence treatment 4. Drug dependence treatment, human rights and patient dignity 5. Targeting special subgrups and cnditins 6. Addictin treatment and the criminal justice system 7. Cmmunity invlvement, participatin and patient rientatin 8. Clinical gvernance f drug dependence treatment services 9. 7 Principles fr Victrian alchl and drug treatment Natinal Institute n Drug Abuse In the United States, the Natinal Institute n Drug Abuse (NIDA) released their revised Principles f Drug Addictin Treatment: A Research-Based Guide in The 13 principles state that: 1. Addictin is a cmplex but treatable disease that affects brain functin and behaviur 2. N single treatment is apprpriate fr everyne 3. Treatment needs t be readily available 4. Effective treatment attends t multiple needs f the individual, nt just his r her drug abuse 5. Remaining in treatment fr an adequate perid f time is critical 6. Cunselling individual and/r grup and ther behaviural therapies are the mst cmmnly used frms f drug abuse treatment 7. Medicatins are an imprtant element f treatment fr many patients, especially when cmbined with cunselling and ther behaviural therapies 8. An individual s treatment and services plan must be assessed cntinually and mdified as necessary t ensure that it meets his r her changing needs 9. Many drug-addicted individuals als have ther mental disrders 10. Medically assisted detxificatin is nly the first stage f addictin treatment and by itself des little t change lng-term drug abuse 11. Treatment des nt have t be vluntary t be effective 12. Drug use during treatment must be mnitred cntinuusly as lapses during treatment d ccur 13. Treatment prgrams shuld assess patient fr the presence f HIV/AIDS. Hepatitis Band C, Tuberculsis and ther infectius diseases as well as prvide targeted risk-reductin cunselling t help patients mdify r change behaviurs that place them at risk f cntracting r spreading infectius diseases NIDA als recently released a cmpanin dcument, Principles f Drug Abuse Treatment fr Criminal Justice Ppulatins: A Research Based Guide (2012). Cntinuity f care is essential fr drug abusers re-entering the cmmunity 10. A balance f rewards and sanctins encurages pr-scial behavir and treatment participatin 11. Treatment planning fr drug abusing ffenders wh are living in r re-entering the cmmunity shuld include strategies t prevent and treat serius, chrnic medical cnditins, such as HIV/AIDS, hepatitis B and C, and tuberculsis C-Occurring Centre fr Excellence (2007) Als in the United States, the C-Occurring Centre fr Excellence (COCE) released their Overarching Principles t Address the Needs f Persns with C-Occurring Disrders in The COCE dcument identifies 12 verarching cnsensus-based principles that guide system and clinical respnses t peple with c-ccurring disrders: 1. C-ccurring disrders (COD) are t be expected in all behaviural health settings, and system planning must address the need t serve peple with COD in all plicies, regulatins, funding mechanisms and prgramming 2. An integrated system f mental health and addictin services that emphasizes cntinuity and quality is in the best interest f cnsumers, prviders, prgrams, funders and systems 3. The integrated system f care must be accessible frm multiple pints f entry (i.e. n wrng dr) and be perceived as caring and accepting by the cnsumer 4. The system f care fr COD shuld nt be limited t a single crrect mdel r apprach 5. The system f care must reflect the imprtance f the partnership between service and science, and supprt bth the applicatin f evidence and cnsensus-based practices fr persns with COD and evaluatin f the effrts f existing prgrams and services. 6. Behaviural health systems must cllabrate with prfessinals in primary care, human services, husing, criminal justice, educatin, and related fields in rder t meet the cmplex needs f persns with COD. 7. C-ccurring disrders must be expected when evaluating any persn and clinical services shuld incrprate this assumptin int all screening, assessment and treatment planning. 8. Within the treatment cntext, bth c-ccurring disrders are cnsidered primary. 9. Empathy, respect and belief in individual s capacity fr recvery are fundamental prvider attitudes. 10. Treatment shuld be individualised t accmmdate the specific needs, persnal gals and cultural perspectives f unique individuals in different stages f change. 11. The special needs f children and adlescents must be explicitly recgnised and addressed in all phases f assessment, treatment panning and service delivery. 12. The cntributin f the cmmunity t the curse f recvery fr cnsumers with COD and the cntributin f cnsumers with COD t the cmmunity must be explicitly recgnised in prgram, plicy, treatment planning and cnsumer advcacy. Natinal Drug Strategy (2011) In Australia, the Natinal Drug Strategy (NDS) prvides a framewrk fr actin n alchl, tbacc and drugs. The framewrk utlines an verarching harm minimisatin apprach which includes three key pillars: supply reductin, demand reductin and harm reductin. These pillars are underpinned by cmmitments t: Partnerships acrss sectrs Cnsumer participatin in gvernance Building the evidence base, evidence-infrmed practice and innvatin Mnitring perfrmance against the strategy and its bjectives Develping a skilled wrkfrce that can deliver n the strategy (Cmmnwealth f Australia, 2011) Page 4

9 Principles fr Victrian alchl and drug treatment Western Australian Alchl and ther Drug Interagency Draft Strategic Framewrk (2010) In Western Australia, the Alchl and ther Drug Interagency Draft Strategic Framewrk prvides a guide fr gvernment sectr strategy, develpment and implementatin f alchl and drug plicy and services. Western Australian alchl and drug plicies, strategies and prgrams are supprted by the fllwing principles: Applying cmprehensive respnses t cmplex issues Prmting access and equity Supprting evidence based practice and applying innvatin Develping and maintaining effective partnerships Being respnsive t emerging issues Prmting sustainable change Furth Natinal Mental Health Plan In the Australian mental health field, the Furth Natinal Mental Health Plan (Cmmnwealth f Australia, 2009) identifies eight principles which underpin its five pririty areas fr actin. The plan s principles include: 1. Respect fr the rights and needs f cnsumers, carers and families 2. Services delivered with a cmmitment t a recvery apprach 3. Scial inclusin 4. Recgnitin f scial, cultural and gegraphic diversity and experience 5. Recgnitin that the fcus f care may be different acrss the life span 6. Services delivered t supprt cntinuity and crdinatin f care 7. Service equity acrss areas, cmmunities and age grups 8. Cnsideratin f the spectrum f mental health, mental health prblems and mental illness Natinal Standards fr Mental Health Services 2011 The Natinal Standards fr Mental Health Services 2011 (p.5) identifies eight key principles that have infrmed develpment f the Standards and are cnsistent with natinal mental health plicy. The principles include: Mental health services shuld prmte an ptimal quality f life fr peple with mental health prblems and/r illness Services are delivered with the aim f facilitating sustained recvery Cnsumers shuld be invlved in all decisins regarding their treatment and care, and as far as pssible, the pprtunity t chse the treatment and setting Cnsumers have the right t have their nminated carer(s) invlved in all aspects f their care The rle played by carers, as well as their capacity, needs and requirements as separate frm thse f cnsumers is recgnised Participatin by cnsumers and carers is integral t the develpment, planning, delivery and evaluatin f mental health services Mental health treatment, care and supprt shuld be tailred t meet the specific needs f the individuals cnsumer Page 5

10 Principles fr Victrian alchl and drug treatment Mental health treatment and supprt shuld impse the least persnal restrictin n the rights and chices f cnsumers taking accunt f their living situatin, level f supprt within the cmmunity and the needs f their carer(s) Victrian framewrk fr recvery-riented practice (2011) mental health sectr The Framewrk fr Recvery-Oriented Practice (Department f Health, 2011) identifies the principles, capabilities, practices and leadership that shuld underpin the wrk f the Victrian mental health wrkfrce (p1). Reflecting a grwing interest in, and uptake f, recvery-riented appraches in the mental health field and beynd, this Victrian dcument articulates nine dmains that reflect key aspects f recvery-riented practice. The dmains include: Prmting a culture f hpe Prmting autnmy and self-determinatin Cllabrative partnerships and meaningful engagement Fcus n strengths Hlistic and persnalised care Family, carers, supprt peple and significant thers Cmmunity participatin and citizenship Respnsiveness t diversity Reflectin and learning Victrian alchl and drug principles In the absence f Victrian-specific alchl and drug principles, Victria has previusly drawn n the widely accepted and utilised US NIDA principles (2009). This is cnsistent with the apprach f ther Australian jurisdictins, such as New Suth Wales 1. Feedback frm a March 2011 wrkshp with a small grup f Victrian alchl and drug service prviders and peaks (n=14), identified a number f key issues critical t the develpment f Victrian alchl and drug treatment principles. Participants reprted that alchl and drug treatment principles shuld guide and infrm the sectr, reflect brad, universally-accepted ntins and be dynamic t accunt fr advancements in knwledge ver time. Participants reprted that Victrian alchl and drug treatment principles might reflect the fllwing specific dmains: Treatment effectiveness Accessibility and equity Service quality and cntinuus imprvement Cnsumer, carer and family participatin Service respnsiveness Wrkfrce capacity Accuntability and evaluatin Cntinuity f care Hlistic, strengths-based appraches t care 1 NSW Health listed an earlier editin f the US NIDA principles in their Drug and alchl treatment guidelines fr residential settings (2007). 11 Principles fr Victrian alchl and drug treatment Evidence and best practice In the cntext f key Victrian develpments in the alchl and drug sectr, including a new Whle f Victrian Gvernment Alchl and Drug Strategy, a new Wrkfrce Develpment Strategy and brader Alchl and Drug Refrm, the identificatin and articulatin f Victrian-specific alchl and drug treatment principles is bth timely and critical. The principles will sit within a new framewrk fr quality and safety develped as part f brader treatment refrm. This framewrk will set ut the key quality requirements f the funded service sectr and will infrm the develpment f new perfrmance mnitring systems. This cnsultatin paper presents a set f prpsed alchl and drug treatment principles that have been infrmed by the internatinal and natinal examples utlined abve. Yur feedback n the set f principles and their key cncepts is welcmed. Each prpsed principle is briefly described. A number f key cncepts are listed t reflect the intent f each principle. Yu are invited t draw n yur knwledge and experience t cnsider each prpsed principle and its cncepts, and prvide feedback via the questins that fllw. Ten cre alchl and drug treatment principles are prpsed, based n the literature. The prpsed principles address the fllwing cre issues: 1. The nature f addictin 2. Treatment accessibility 3. The Natinal Drug Strategy (2011) identifies supply reductin, demand reductin and harm reductin as key pillars f a harm minimisatin apprach. In the Victrian treatment sectr, harm minimisatin appraches are reflected in the prvisin f safer using infrmatin, needle and syringe exchange prgrams, brief interventins and a range f ther wrk fcussed n enhancing the safety f clients. Key cncepts: a) the safety f clients is paramunt b) interventins range frm infrmatin and educatin t the prvisin f safer using equipment c) delivered in treatment settings as well as thrugh harm reductin services 4.1 Is a principle abut harm minimisatin apprpriate? 4.2 Have yu any cmment abut the key cncepts prvided? 5. This includes respnding t c-ccurring cnditins such as mental illness and wrking with frensic and ther specialist treatment systems. In sme literature, a hlistic service respnse is a key cmpnent f individualised care. Hlistic care reflects a brad understanding f the life circumstances f individuals in treatment and recgnises the imprtance f respnding t diversity in the treating ppulatin. Hlistic practice is cgnisant f the age, gender, race, ability, lcatin, scial, cultural, ecnmic, family, and relatinship circumstances f the individual. It requires cultural cmpetence, gender sensitivity, capacity t wrk acrss different ages and stages f life and t wrk with client, family and ther peple in caring relatinships. Evidence-based practice Principles fcussed n evidence-based practice articulate a need fr rbust and timely knwledge transfer frm research and practice t the field f direct service delivery and treatment. Sme evidencebased practice principles are high level and systems-riented, identifying the imprtance f using the best evidence available, while thers identify specific clinical interventins knwn t be effective.
